Form 137 for transferees 4. NSO Birth Certificates 5. K to 12 Grading System 6. Other matters 1. Meeting called to order at 1:00 pm by Grade 9-B adviser, Ms. Grace T. Antaran. 2. The following is agreed new set of Homeroom PTA Officers for school year 2018-2019: President: Melanie T. Ingeniero Vice-President: Maripel T. Tilar Secretary: Jennifer B. Aguelo Treasurer: Janet B. Cadiz Asst. Sec. Treas.: Salvadora P. Romero Auditor: Claudio Gamale Bus. Manager: Benny Ascaño P.I.O: Erupeñeta Duromal Peace Officers: Eleuterio Alarcon Ligaya Flores Leonisa Cahilig 3. The adviser reiterated the following school rules and regulations:     

Male students are not allowed to have long hair, in short, they must observe proper haircut. They must not also wear earrings in school. All students must come in school wearing their complete uniform excluding Wednesdays. Students’ tardiness and absences will be recorded by the adviser. Successive absences without excuse letters will be subject for home visitations. There will be quarterly meetings and parents are encouraged to attend to know the statuses of their children in school. Use of cell phones during class hours is strictly prohibited.

4. The adviser reminded the parents of the transferees to follow up the form 137 of their children from their previous schools as well as, the parents who have not submitted NSO birth certificates. 5. K to 12 Grading System was introduced and explained to the parents: Written works ------ 30% Performance -------- 50% Quarterly Assessment ---- 20% 6. The adviser reminded the parents to attend in the next meeting. 7. The meeting adjourned at 1:45 pm.
