Per our phone conversation just minutes ago, please consider this my clarification to the request originally submitted on May 6, 2008 (attached): Per the federal government's criteria, accidents only need to be logged if it results in an injury requiring transport to a medical facility. I'm requesting not just information from those "accidents," but all "incident" reports, including those that do not require transport to a medical facility. 1) All incident reports of all Blue Line train/vehicle and train/pedestrian incidents involving individuals under the age of 25 (not including MTA employees in the act of work). Information on such reports may be redacted if found legally necessary to fulfill this request, but among the information that should not be redacted shall be: the location, time and date of the incident, and the age of all individuals involved (not including MTA employees in the act of work). 2) All accident reports of all Blue Line train/vehicle and train/pedestrian accidents involving individuals under the age of 25 (not including MTA employees in the act of work). Finally, unless the format of the MBL Quarterly Reports have changed since the June 2006, there has been a substantial redaction of information. Thus: 5) I am requesting all interoffice memos from Corporate Safety Department regarding the Metro Blue Line quarterly reports. All the other provisions of the original request submitted on May 6 on page 1-3 and the first paragraph of page 4 are applicable. Your prompt response is greatly appreciated.
