Learning Experience Duration: 3 Weeks 2.1 Department Framework – Week 1 Department of attachment: A & E Date: From 8th to 12th October 2018 Summary of department attached.   

  

Patient acceptance on A&E uses Triage  P 1 Immediatly, P2 -30 minutes, P3- 1 Hour. Handling simulation of patients with ebola involving all relevant officers.  Handling patients from A&E to ICU Patient’s path  Registration Waiting Triage Vital sigh RP Case to see doctor Nursing Action Waiting spesialis/x-ray/lab/X-Ray result Sending patient in to the room The number of nurses in the A & E department is 24 people in addition Nurse Manager There are 3 shifts for the guard schedule, take care of people in the morning, take care of 6 people in the afternoon, 5 people at night, keep moorning from 7;00am to 14;30 pm keep your daytime from 2;00pm to 9;30 pm, watch night from 9 pm to 7.30 pm All nurses on duty at A&E 100% have an emergency certificate such as ATLS and 3 people have advanced certificates/emergency specialties

2.2 Learning experience. What have I learn that I can share in my hospital/organisation  Paper usage is very minimal, Documentation or recording of patients using computers starting from the patient's arrival until the patient is hospitalized.  Infection control is a priority for patients and for health workers.  Patient privacy is well guarded under the PDPA (Personal Data Protection Act), including when officers perform special actions.  Officers control information on diseases, drugs and actions given, always giving medical treatment to patients is always preceded by information or an explanation of the procedure before taking action to the patient.  Approval of action is always requested and signed before it is implemented.  Every mount there is a refresher on cases relating to emergencies with different cases and alternately presented by staff  Cleanliness and neatness of the place of work is considered.

3.1 Department Framework – Week 2 Department of attachment: 9 W Paediatrics Date: From 15th to 19th October 2018 9 W Paediatrics Department is the wards devoted to the care of children from 1 month to 14 years with various kinds of diseases including diseases that require isolation treatment.  Light code in front of the patient’s room : 1. White ( Patient Call ) 2. Green ( Nurse SN in the room) 3. Orange ( Nurse EN in the room )  Emergency codes : Blue (Medical Emergency ), Red (Fire ), Green (Violent Patient ) pink code only used for paediatric spaces 9 west if kidnapping occurs in children or babies.  At 9 West Paediatric, the nurse’s schedule is divided into 3 shift (morning shift, afternoon shift, and night shift), In one shift, one nurse ( SSN&SN) in charge of keeping 1 to 10 patient.  Before the rolling shift, briefing is done first about the number, case and implementation of patients to the handled by each nurse.  For patients who are allergic given a bracelet in the hand with orange and for parents who accompany the patient is given a green bracelet and for patients who are not allergic are given a green bracelet and parents who accompany the green colour as well.  The use orange bracelets on the patient’s companion as a sigh that is responsible for the patient can be father, mother or other family. 

Evaluation of patient condition using

 Nurses who take care of the morning work on nursing care with the PAGIE method : P = Problem A = Assessment G = Goal I = Intervention E = Evaluation And for the afternoon and evening the nurse evaluates with P = Problem E = Evaluation Unless there are new problem that arise the evening or evening service will make the PAGIE method  There is a hands rub in front of the patient’s room and for isolation room there is many mask n hands-on. It aims to make doctor and nurses easy and obedient to always perform hand washing procedure before and after contact with patients.  Every drug that has been ordered for the patient is put into a cupboard in the patient’s room and locked the key is held by nurse.  Every room that has been left by the patient will always be cleaned, be it the room or equipment available (vital sign monitor performed sterile procedure on the room by using a special spray). 

At 9 west paediatric also has an isolation room that serves to control the spread of infection from the threated patient in 9 west paediatric the isolation room has two doors that are opened alternately and also has air pressure that is located with the room outside.

3.2 Learning experience. What have I learn that I can share in my hospital/organisation  Every drug given both oral and injection under the care of nurse  The nurse’s have high discipline and work ethic.  The nurse’s care for all patients and maintain patient (family) privacy.  Before carrying out medical procedures the nurse’s always provides an overview of the procedures to be taken.  Prioritizing the quality of services, especially nursing care.  Visitor restrictions (visitor hour: 8 am – 8 pm, photography or video recording prohibited, and mobile phone is set to silent).  The medical procedures are carried out by doctors and certified nurse.  The cleanliness of the environment and the patient’s room are very good.  Know how to manage drugs according to patient needs.  Recording of patients using a WOW computer (work on wheel).

4.1 Department Framework – Week 3 Department of attachment: 9 East Department Date: From 22th to 26th October 2018 9 East Department is a ward that treats surgical cases with age limits for children 3 years and older and adult. Nursing work system in 9 East Department: -

Orange ( Nurse EN in the room )  Emergency codes : Blue (Medical Emergency ), Red (Fire ),  Code blue :  When code blue is acting or helping nurses, senior ners and doctor (will came just have to wait)  At 9 East, the nurse’s schedule is divided into 3 shift (morning shift, afternoon shift, and night shift), In one shift, one nurse ( SSN&SN) in charge of keeping.  Before the rolling shift, briefing is done first about the number, case and implementation of patients to the handled by each nurse.  The patient’s bracelet is white for the patient’s identity, green for the risk of falling  At any one time please limit visitor to two in general ward and one in the critical care and high dependency areas.  in this ward room is a pain nurse who educates patients at the time of surgery and wound nurses who observe wounds  There is a hands rub in front of the patient’s room and for isolation room there is many mask n hands-on. It aims to make doctor and nurses easy and obedient to always perform hand washing procedure before and after contact with patients.  The drug is not placed in the patient’s room but the medicine is put on the drug trolley after the drug is ordered before it is distributed  The nurse in change of giving medicine uses a special orange vest.  Morning and evening guard consists 6-7 people as coordinator and 2 team (each team consists of 3 people)

4.2 Learning experience. What have I learn that I can share in my hospital/organisation  the nurse’s explanation to the patient about the drug procedure and the action was very clear.  Collaboration of team (nurses,doctor,lab officers,etc)for recovery of patients goes very well  The nurse’s have high discipline and work ethic.  The nurse’s care for all patients and maintain patient (family) privacy.  Prioritizing the quality of services, especially nursing care.  The medical procedures are carried out by doctors and certified nurse.  The cleanliness of the environment and the patient’s room are very good.  Know how to manage drugs according to patient needs.  Recording of patients using a WOW computer (work on wheel).

5.3 Scope and Limitation Summary of my strengths and limitations: Strength : - The local government (Kabupaten Tanah Bumbu) fully supports apprenticeship (nurse’s trainee) activities at Gleneagles Hospital Singapore. - The hospital provides an opportunity to be able to internship in several ward. Limitation : - Limitation in speaking English so that communication between nurses is not smooth.
