CHAPTER 8: EMERGENCY RESPONSE PLAN 8.1

LIST OF LIKELY EMERGENCIES

Emergencies and occupational risks that are likely to occur at the project site during the construction are: (a) Slope Failure; (b) Spillage during handling or transportation; and (c) Silt/ mud flood.

8.1.1

Slope Failure

A slope failure is phenomenon that a slope collapses abruptly due to weaken self-retain ability of the earth under the influence of a rainfall. Because of collapse of slope, personnel must know the location and proper use of the first aid kits. The contractor’s site personnel should immediately report the accident to the Environmental Officer or by calling direct the emergency respondents such as the JKR, Police Station, Hospital, and Fire Station.

8.1.2

Spillage during Handling or Transportation

An elevated bund lined with concrete (110% of tank volume capacity) surrounding the diesel skid tank area must be constructed to contain any accidental oil spillage and to limit the spread of any potential spillage. The spilt oil then should be contained or stored in drums prior to disposal as required in the EQA or can be recycled for other uses.

Spills should be prevented through compliance with correct operating procedures, and through regular inspection. If any accidental oil spillage occurs into the water body, the contractors or site personnel should immediately report the spillage to the Environmental Officer or by calling direct the emergency responders and JKR.

Hazardous (HAZMAT) of Taiping Fire and Rescue Station is available for cleaning purpose. The affected equipment, machine or tank should be isolated or shutdown whichever is applicable. The Occupational Safety and Health Department must be notified through a proper channel within 24 hours.

8.1.3

Silt/Mud Flood

Large projects usually involve extensive land disturbance involving the removal of vegetation and reshaping the topography. Such activities make the soil vulnerable to erosion. Soil removed by erosion may become airborne and create a dust problem or be carried by water into natural waterways and pollute them. Due to the soil erosion of the exposed and loose earth, there will be deterioration of water quality in the surrounding water bodies due to siltation and sedimentation. Siltation and sedimentation in the water bodies can result in mud or silt floods and flash floods in the downstream area during heavy downpour.

The impact on the environment is particularly high when work is on highland, on slopes, near coast areas, rivers, and lakes. The contractor’s site personnel should immediately report the accident to the Environmental Officer or by calling direct the emergency responders such as JKR, Police Station, and Fire Station (Jabatan Bomba & Penyelamat).

8.2

SPECIFIC EMERGENCY PROCEDURES

8.2.1

Cleaning Procedure and Waste Management Method

The immediate action to be taken for cleaning the wastes and the method for wastes management prior disposal are as indicated below.

Cleaning Procedures: 

Identify the size and nature of pollution;



Identify the type of pollution;



Identify the resources needed to contain or to clean it;



Identify the dean up personnel to carry out the cleaning duty;



Identify the suitable protective clothing;



Identify the location to dispose the collection waste; and



Manage the transportation of waste for disposal.

If the conditions worsen, the help of the nearest Fire Department (Jabatan Bomba & Penyelamat) and the Police Station will have to be search. If the situation then goes out of control, the evacuation signal would have to be activate after which complete evacuation of personnel must be execute. If Personnel are not actively involved, he will evacuate from the area. In case of contaminated materials run off from the containment area, effort should be made to preventing release to the sewage water system using absorbents or pumps.

8.2.2

Transport Emergency

The transport emergency response procedures are design to mitigate the effects of hazardous waste spills during transportation. The lorry driver should inform the Prisma Dinamik Sdn. Bhd. about any accidents during the transportation. After being informed about the accident the office, the main contractor will immediately contact the Police Department, Fire Department (Jabatan Bomba & Penyelamat), Majlis Perbandaran, JKR or DOE through the numbers tabulated in Table 8.1 below.

All spills will be considered as hazardous. No one will approach the spills until the identification of spill had known. In all cases, the personnel at the accident site should wear protective clothing. The safety precautions will be as follows: 

Always approach a spill from upwind;



Do not touch the waste materials;



Remove all possible ignition sources (e.g. running engine);



Do not smoke; and



Restrict access to the area.

The most effective way to control spill results from transport related accident is to contain it. This act can facilitate on-site clean-up operations and prevent of contamination of water sources. In case of spill on land, earthen dikes built with the help of shovels or bulldozer; or the land is excavated to pond the waste. Pumps will be used to recover the waste solvents. For the part too little to be pumped up, and absorbent will be used to absorb the spilled waste. Action need to be taken to return the environment to its conditions as before the accident. Actions that might be taken under this phase will be replacement of contaminated earth and replanting of vegetation.

8.3

RESPONSIBILITIES

When personnel observe any emergency incidents; he should alert on site personnel of possible dangers and provides for an orderly stop to operations in the affected area or evacuation of necessary. It shall also signal the emergency response team to take necessary action.

Each personnel in the emergency response team will have its own responsibilities as stated below:  Emergency Contractor or Representatives The contractor representatives involved at site will be responsible to initiate the first line of action in all emergencies. However, if the situation goes beyond his control, JKR Hulu Langat will be informed immediately.  DMIA (as Main Contractor) DMIA Sdn. Bhd. shall direct all control and rescue operations in the event of a disaster or emergency. Immediate mobilization of a trained ERP team comprising of internal personnel is required. Subsequently, steps to liaise with external agencies for assistance should follow. from DMIA Sdn. Bhd. shall direct the control and rescue operations with the assistance of the Jabatan Kerja Raya.  Jabatan Kerja Raya Jabatan Kerja Raya (JKR) shall direct all control and rescue operations in the event of a disaster or emergency. Immediate mobilisation of a trained ERP team comprising of internal personnel is required. Subsequently, steps to liaise with external agencies for assistance should follow.

In the event that measure failed, and external areas at risk of being affected, especially if it extends to public areas, then depending on the nature of the disaster, the senior officer from JKR shall direct the control and rescue operations with the assistance of the Senior Corporation representative.  Project Manager Project Manager shall ensure that the site is prepared for an emergency disaster by regular monitoring, checking and testing the organization, individual, procedure and equipment available for the respective purposes. This range from simple checks of the components and the emergency response arrangements to full-scale simulations involves the active participation of external agencies and emergency services.

To revise disaster vulnerability on continuous basis, revise plans and resources as required. A post disaster plan shall also be provided.  Environmental Officer Environmental Officer assumes the responsibility of implementing the EMP at the Project Site under the supervision of the Project Manager. Internal audits will be carried out from time to lime to ensure that all pollution control measures are effective. The EO will advise the Project Manager on any environmental non-compliance issues reported during the site audits. He will also be responsible to supervise the implementation of corrective actions for the recorded NCRs. The EO will also assist the Project Manager in

preparing all environmental management related reports for the submission to relevant agencies.  Safety Officer Construction safety officers conduct regular site inspections, recording all violations, noting what remediation needs to occur to keep the project moving forward safety. If necessary, the construction safety officer suspends the project, pending necessary changes. He conducts safety trainings, both formally, such as new employee safety orientations or refresher trainings, and on the spot to all site workers. He produces a comprehensive report of each inspection for accounting purposes.

Accidents sometimes happened because there are not a sufficient number of workers on site at the right time, so a construction safety officer monitors work force to ensure there are no shortages. He communicates mandatory safety standards to all contractors during the pre-bid walkthrough stage and at pre-construction meetings so there are no questions about the rules before the job even begins. He reviews each contractor's safety plan and monitors it for compliance.

8.4

SCOPE OF THE EMERGENCY RESPONSE PLAN

Many of the emergency plans and procedures are link to safety and prevention of accidents. The scope of the ERP is therefore to include plans for safety of the workers that includes the following: i.

Sounding of alarm and immediate stoppage of work whenever an accident occurs;

ii.

Detailed instructions for notification to the proper authorities (including phone numbers etc.) when an emergency occurs;

iii.

Listing of the individuals responsible for the various procedures in emergencies; and

iv.

Communication plans to ensure all the site personnel aware of their correct responses in an emergency.

In the line of the scope of ERP and procedures, any personnel observes any emergency incidents; he should alert on-site personnel on possible dangers and provides an orderly

stop of operations in the affected area or evacuation of necessary. It shall also signal the emergency response team to take necessary action. Plan for control and safety steps presented as follows:-

i.

Injuries and fatal activities

In the event of a fatal accident, the in-charge person should immediately inform the Police and Department of Occupational Safety and Health (DOSH) who should visit the scene of an accident. The in-charge person will liaise with the police, DOSH, and Fire and Rescue Department to have joint inspections before the resumption of work.  Traffic Accidents In the case of traffic accidents within the Project site, the procedures for general injuries or fatal accidents are applicable. a) Immediately survey the traffic situation and redirect the operational vehicles away from the accident site; b) Sound the alarm and call the ambulance if there are persons injured; c) If the accident occurs with ensuing explosions, the BOMBA has to be called immediately. All entrance gates should be closed to any incoming vehicles, except for authorized vehicles such as the police and fire engines; d) Freeze all activities at Project site and evacuate if necessary; e) The SHO shall report all accidents to the PM during and after the accident; and f) He/she should also prepare an accident report for relevant authorities, the Resident Engineer (RE) and the Project Manager (PM).

Figure 8.1 until Figure 8.3 show the flowcharts for emergency response, and actions of serious accident involving Injury and fatal accidents.

ii. Fire and explosion Fire can occur during the construction phase. The following procedure applies in the case of fire or explosion. 

Before the fire alarm activated: a) Activate the fire alarm or shout “FIRE, FIRE” if in public places; b) Report fire location to the nearest personnel who has a walkie-talkie or any medium of communication; c) Call for medical help if there are persons injured; and d) Do not enter the Project site until it is declared safe.



When the fire alarm is activated: a) Assist the workers and any public to use the escape route; b) Direct the workers to the designated gathering area; c) Do not stop to collect personal belongings; d) Switch off all powered equipment; and e) Follow the instructions from the in-charge person.



Extinguish the fire: a) Attempt to use the nearest fire extinguisher to extinguish the fire if the height of the fire does not exceed the knee level. Alternatively, the alternative, evacuate immediately; b) Call the nearest available Fire and Rescue Department; and c) An in-charge person shall report all accidents to the Police and the Management.

iii. Oil/chemical and Hazardous Spillage a) Minor spills A minor spill is a spill small enough to be controlled, contained and cleaned up using readily available equipment and materials. b) Major spills A major spill demands immediate attention and immediate notification of both company personnel and appropriate authorities. For any major spill, before responding:  Notify others of the situation;  Do not allow anyone to walk or drive in the spilled material;  Get needed protective gear;  Be certain and all responding are aware and consider the spilled material have any special properties. (i.e. flammable, acid, high toxicity, reactions with other stored products); and  Shut down all power until it is determined what can be safely used. For a spill within any secondary containment:  Pump water or spilled product into stable tanks. Remove or restrain small equipment;  Remove or transfer material to another storage area;  Check valve. Plug holes with wood stakes or replace parts; and  Tape or strap over the leak. Transfer to another tank. When an incident results in an oil/chemical release of any amount, notify Emergency Management or Environmental and Natural Resources immediately upon gaining control of the spill. The agencies will provide assistance and guidance regarding the proper procedure, based on the amount and type of substance involved. Whenever possible, spilled material should be use originally intended. When either reuse according to the product label is not possible because of excessive contamination or cross-contamination with the incompatible material, the material must be disposed off in accordance with the DOE requirements. Depending upon the chemical involved,

disposal may be local landfill permitted to accept the material, a hazardous waste landfill, or land application of the contaminated soil. Do not begin use/disposal of spilled or contaminated material until DOE has granted approval. The steps are shown in Figure 8.5.

iv. Flooding Events Flooding events can occur during any stages of construction. This contingency plan is prepared for flooding events in two phases, which are: 

An indication of flooding i. In the occasion of non-stop heavy rain for hours, monitor the river water level time by time; ii. Notify the PE or SHO; iii. Evacuate the mobilization of workers and equipment’s from Project site; iv. Flooding control equipment to be in placed; and v. PE and SHO to control further flooding under the Project requirement.



Abrupt of flood i. In the occasion of non-stop heavy rain for hours, monitor the water level time by time; ii. If the water level increases drastically; iii. Site Supervisor/ site leader assist in mobilize and head counting of workers/ visitors at the nearest assembly point; iv. Call the police, ambulance or Fire and Rescue Department; v. Mitigate the situation and rescue the victims; and vi. SHO shall report all accidents to the PM during and after the accident. He/ She should also prepare an accident report for relevant authorities and PP.

Mitigation measures shall be carried out to minimise flood damages to the construction site and later during the operational phase to all the buildings and operational plants. Flooding can occur in a small area such as blockage of the drains causing back of runoff that affect the construction site. This situation can easily be rectified by unblocking the drains and site clean-up. Major floods due to the high water flow in the rivers during the monsoons can occur in the low-lying areas. In the event of a major flood, the flood response plan providing for flood evacuation of the workers shall be activated. The RE and the SHO during their planning has to provide information on the locations of the

Medical Emergencies Medical emergencies can occur at the work place such as heart attacks, fainting etc. The actions include (Figure 8.7): a) Call 999 for an ambulance immediately; b) Narrate the exact type of medical emergency if possible and indicate the exact place of injury (if the person is injured) and approximate condition or symptom; c) Call the management office to report the same; d) Take the first-aid team members to the place where the patient stays; e) Take care of the patient; and f) Keep calm and comfort the patient. The SHO shall report all accidents to the RE during and after the accident. He/she should also prepare an accident report for relevant authorities and the RE and PM.

vi. Other Types of Hazards and Emergencies These types of hazards include drowning, thefts and fighting at the construction site. Probability any of these accidents occur, the procedures activated are: a) Stop work and sound the alarm; b) Activate the emergency procedures; c) Call the police and ambulance; d) Try to search and rescue the person or persons if possible (in the case of drowning). If situations are complicated, wait for the police and the rescue teams to perform the necessary rescue actions; e) Cordon off the area of the accident to avoid situations from getting worse and provide accessibility to the accident site; f) Survey and report the accident and incident (in the case of thefts and fighting) to the relevant authorities; and g) Work should not resume until the RE and the PM of the contractor give permission.

vii. Command and control In the event of an emergency, all control and rescue operations should be under the direction of the in-charge persons. This will involve the key project proponent personnel in charge of the project with assistance from outside agencies.

When a major disaster occurs that involves the prolonged commitment of an outside agency, or when imminent danger extends into a public area, the relevant agency senior officer should take charge. The Project Proponent’s representative role will be assisting the senior officer by offering technical advice, facility control and other available resources within his power. The emergency control plan is summarized in Table 8.2.

This is to ensure that all areas are safe and that there is no risk of a repetitive or similar accident. ERT will carry out an investigation to find out the cause of accident and report should be submitted to the management, client and relevant authorities. In the event of a fatal accident, the in-charge person should immediately inform the police and Department of Occupation Safety and Health (DOSH) who should visit the scene of the accident. The organisational flow chart for Emergency Control /chain of command is shown in Figure 8.8. In the case of fire and explosion, the following procedures shall apply:  Trigger the alarm and call for medical help if there are any persons injured.  Activate the fire alarm or shout “FIRE! FIRE!”  Report the location of the nearest personnel who has radio or any medium of communication;  Extinguisher the fire;  If it is safe to do so, try to extinguish the fire using the nearest fire extinguisher. If the Fire cannot be extinguished or if it starts to spread, evacuate. PROTOCOLS FOR EMERGENCY RESPONSE PLAN

Many of the emergency procedures in the ERP are related to the safety of the workers at the Project site. Thus, all site activities will have to be carried out in a manner that minimizes risks as far as is reasonably practicable. This can be achieved by safe working practices. The main focus of the ERP shall be for the safety of the employees as the first priority when any potential hazards do occur. The focus of this ERP is on the provisions in the form of procedures for the smooth operations to handle the emergency, including search, rescue and recovery operations and other relief work. The main components in the ERP are:

i.

Types of emergencies: Potential hazards and threats to human lives, the Project and its surrounding areas;

ii.

Safety procedures: For all work activities when accidents occur;

iii.

Emergency organisation and responsibilities: Formation of a well-trained and well-equipped response team for fast and effective rescue operations when required;

iv.

A list of key personnel: Individuals responsible to organize and control operations in emergencies;

v.

Details of emergency services: Instructions for notification to the proper authorities (including contact numbers, etc.); and

vi.

Internal and external communication: A communication plan to ensure that all the site personnel aware of their duties in an emergency.

ELEMENTS OF THE EMERGENCY RESPONSE PLAN

The ERP shall be activated immediately when an emergency occurs. The main elements considered in the ERP are: a)

Types of emergencies and potential hazards and threats to the workers, Project site and its surrounding areas;

b)

Formation of a well-trained and well-equipped response team for fast and effective search and rescue operations when required; and

c)

Designated areas for all personnel to group together for safety purposes at an Emergency Assembly Point(s) until further notice by the Resident Engineer (RE) or the Safety, Health Officer (SHO) and Environmental Officer (EO).

8.6.1

Basis for Emergency Response Plan

The basis for the ERP program is to comply with the requirements or general intent of all relevant legal requirements pertaining to emergency management. These include the: a)

Occupational Safety and Health Act (OSHA) 1994 (Act 514);

b)

Environmental Quality Act 1974 (Act 127);

c)

Majlis Keselamatan Negara (MKN) – Arahan No.20 – Dasar dan Mekanisme Pengurusan dan Bantuan Bencana Negara;

d)

BOMBA Service Act, 1998 (Act 341);

e)

MS 761: 1982, Code of Practice for Storage and Handling of Flammable; Combustible Liquids (UDA 620.263-Sirim 402/2/8); and

f)

Definition of Terms Serious Accident – A serious accident is defined as a life-threatening incident or causing disabling injury to the worker(s). When a serious or fatal accident occurs on-site, certain procedures shall be followed to ensure orderly and proper actions be taken;

The situation is still within the control of the safety and security unit on- site. The emergency does not involve loss of lives or extensive damage to property. Limited or no help from outside is required. A more serious accident or emergency with potential loss of lives and extensive damage. This emergency will require outside help and includes calling assistance from Police, BOMBA, and hospitals. Accident or emergency that is more complex and serious and requires all the help that is available. Besides the Police, BOMBA and hospitals, Department of Occupational Safety and Health (DOSH) and DOE will have to be notified.

Awareness for Site Personnel

Work at the Project site exposes employees and the public to various types of accidents that may threaten their safety. Thus, the employees shall be made aware of their responsibilities to prevent accidents from occurring and to take prompt and positive safety actions when accidents do occur. Regular awareness campaigns among employees may occur at any time. During emergencies, all site personnel (sub-contractors, construction workers, etc.) shall assemble at the emergency assembly area upon hearing the siren for head count and then follow all directions from the assembly warden, the PM, SHO or any senior officer of the ERT.

INCIDENT OR ACCIDENT REPORTING

A proper incident or accident reporting procedure will enable the relevant parties to respond immediately to control the incident or accident for the protection of the workers, the public, property, and environment. The procedure will provide: a) Guidance on formal notification and reporting of an incident or accident required by DOSH and other authorities (i.e. DOE) for insurance claims on post damage losses; and b) Guidance for the authorities to perform follow-up inspection or investigations on the remedial actions taken in compliance with DOSH requirements.

8.7.1

Risk and Emergency Communication

It is necessary and judicious to inform all workers and staff of all the potential hazards and their associated risks at the Project site. The communication effort is essential for their understanding so that the procedures in the ERP can be adopted and practiced effectively. Staff training and familiarization tour of duty areas where this communication can be formally imparted. During the sessions, it is good practice to provide document handouts on all the procedures, rules and regulations that need to be complied with. All relevant instructions should also be placed in prominent areas of work to facilitate easy access by the workers and the general public.
