1. HEALTH SAFETY ENVIRONMENT Penerapan konsep HSE dalam area pertambangan adalah tanggung jawab semua karyawan dan managemen sehingga perlu ada komitmen dari management PT. KONUTARA SEJATI mencari solusi, dalam penyelesaian permasalahan-permasalahan HSE. Oleh Karena itu, diperlukan koordinasi dan kerjasama diantara semua unit kerja PT. KS dalam pembahasan masalah HSE agar penerapan program-programnya dapat dilaksanakan dengan baik.

1.3 Incident dan accident a. Kebakaran Lv. Daihatsu rocky dan Lahan di area Blok 16 Ha. Menurut kronologis kejadian, LV dalam keadaan terparkir dengan kondisi, ban tersangkut, handle tidak berfungsi. terdapat 2 saksi mata yang berada tidak jauh dari lokasi kebakaran sedang istirahat dan tiba-tiba merasakan hawa panas dan

mendengar letupan-lutupan, api telah membesar di depan kiri LV dan api dengan cepat masuk ke dalam body LV. dan menghanguskan lahan disekitar mobil. Penyebab kebakaran : masih dalam penyelidikan tetapi sumber api di duga dari LV akibat (Arus pendek, kelistrikan dll) Akibat yang ditimbulkan : Mobil LV hangus terbakar dan area sekitar Di lahan 16HA( 50X100 M²)

b. Korsleting Aki Lv.Hilux Merah Saudara A sementara mengemudikan LV kemudia tiba –tiba LV berhenti (Mesin Mati). Saudara A membuka kap mobil dan muncul asap disekitar Aki. Saudara A mematikan stop kontak Lv. Penyebab kebakaran : Besi penahan Aki terlepas dan menempel di kutub positif aki sehingga mengakibatkan korsleting. Adanya spek Aki yang tidak sesuai dengan dengan jenis penggunaan. Akibat yang ditimbulkan : Aki rusak.

Di Minggu pertama di bulan November ini Baik produksi Ore dan kegiatan penjualan Ore (Barging) Cukup baik, Di bulan ini ditargetkan produksi dan barging dapat mencapai 50,000 Wmt. Kegiatan Produksi Ore minggu ini mencapai target 77.80% dengan rata-rata produksi sebesar 1945 mwt. Sedangkan barging mencapai 111% (Sangat baik)

2.3 Mining Progress Penambangan masih dilanjutkan di 2 blok yaitu 3.7 Ha dan 8.4 Ha, dibeberapa titik telah dijumpai fresh rock sepert, front 6A,6D dan Sebagian 6C.yang telah menandakan level bottom ore, penambangan di lahan 3.7 untuk menghabiskan cadangan sisa yang hanya di estimasi 2000-3000 ton menurut estimasi lapangan. Penambangan front 7A bagain utara lahan 8.4 Ha tidak bisa dilanjutkan karena tdk prospek (data test pit) sedangkan Progress penambangan saat ini berada di lokasi 7B bagian barat (Tidak masuk dalam pit boundary) Estimasi cadangan sisa di lahan 8.4 Ha sebesar 63,084 Ni 1.91% Fe 32.25% (Survey tgl 30/10/2018) Dari data hasil eksplorasi di lahan 16 Ha, terdapat prospek area sebesar 2 Ha, diperoleh data cadangan sebesar 72225 Ni 1.85% COG 1.7 COG 1.7 OB ORE

2.4 Kendala Produksi Adapun kendala yang dihadapi tim produksi dilapangan adalah sebagai berikut: a. Kemampuan ALkon/pompa yang terbatas dalam memompa air di dalam front sehingga menghambat kegiatan ore getting b. DT dan unit alat berat yang masih sering breakdown c. Debu tambang, diperlukan penyiraman yang intense di sekitar tambang dan office d. Jumlah LV yang tidak memadai untuk mobiltas kegiatan dilapangan. e. Tidak adanya suryeyor untuk pengukuran dilapangan.

Dari grafik di atas diketahui performa UA dan EU dari unit compactor dan grade sangat rendah karena tingginya waiting intruksi. Mulai Bulan november sistem sewa unit tersebut menjadi sistem HM, dari sebelumnya sistem bulanan. Sedangkan MA Excavator cukup baik 76% sedangkan Dump truck 64% karena banyaknya kerusakan DT. (Waiting Spartpart)

Secara keseluruhan waktu kerja alat minggu ini sebesar 51%,

Breakdown dan waiting

intruction penyumbang terbesar dari waktu stanby.

Untuk unit alat berat (Excavator, grader, compactor) hanya bekerja sebesar 47% dari total plan kerja waktu efektif 10 jam/hari.

Sedangkan Untuk dump Truck pada kegiatan hauling hanya bekerja 55% dari total waktu efektif 10 jam/hari.
