Kriteria Calon Surveior 1. Syarat-syarat umum : 1. Umur 35 tahun ke atas 2. Fit untuk melaksanakan tugas surveior (dibuktikan dengan surat keterangan sehat dari dokter Rumah Sakit) 3. Memiliki kemampuan di bidang kepemimpinan, komunikasi dan kerjasama dengan Tim 4. Seluruh calon surveior harus mendapat rekomendasi dari  Pengurus KARS, atau Surveior KARS, dan  Direktur Rumah Sakit, atau  Kepala Dinas Kesehatan Provinsi, atau  Ketua PERSI Pusat atau Daerah, atau  Pejabat Kementerian Kesehatan (minimal Eselon II) 5. Pengajuan lamaran hanya melalui email [email protected] dilengkapi dengan surat lamaran yang ditujukan ke Ketua Eksekutif KARS, foto ukuran 4×6 dengan background merah, curriculum vitae , dokumen penunjukan atau pengangkatan, ijazah pendidikan dan sertifikat telah mengikuti seminar atau workshop. 6. Menguasai komputer untuk program Words, Excell, Powerpoint, E-mail / Internet 7. Memahami peraturan dan regulasi di bidang perumahsakitan dan kesehatan 8. Tidak pernah terlibat secara permanen dalam masalah hukum

Kriteria Surveior Keperawatan 1. o

The personal attributes and skills of the surveyors A surveyor must has the following skills and abilities for:       

Decision Making Strategic Management Assimilation and Information Analysis Skills in teaching and training Understand standardization and its purpose Able to accept criticism and make self-correction Has extensive knowledge on health service system in Indonesia

General Qualifications of Surveyor: 1. Age 35 years and above. 2. Fit to conduct surveyor’s tasks (proven by doctor’s certificate from hospital). 3. Has competence in leadership, communication, and good teamwork. 4. Surveyor candidate must obtain recommendation from one of the following: o KARS’ commissioner, or KARS’ Surveyor, and o Director of a Hospital, or o The Head of the Province Health Office

Pengalaman :  Pernah bekerja di Rumah Sakit sekurang-kurangnya 5 tahun sebagai fungsional tenaga keperawatan dan,  Pernah menjabat sebagai jabatan struktural dalam bidang pelayanan keperawatan di Rumah Sakit dan,  Syarat lain :  Mendapat izin / rekomendasi atasan langsung dari Kepala / Direktur Rumah Sakit. Pernah mengikuti Workshop SNARS yang dibuktikan dengan sertifikat.

Able to operate computer for Words, Excel, PowerPoint program, Email/ Internet. Understand the rules and regulation in the field of hospital and health. Has never been convicted with permanent legal force.

. Requirement qualification for nursing suveyor Education: Nursing Specialist (S.Kp) / Magister Nursing (MKep) from Nursing Diploma 3 Experience : 1. Has worked in a hospital for at least 5 years as functional nurse, and 2. Has served in a structural position in nursing service field in a Hospital and 3. Other qualification: o Obtain permit / recommendation from

direct supervisor from the Head/Director of the Hospital 4. Have attended SNARS Workshop proven by certificate Requirement qualification for management surveyor

Kriteria Surveior Manajemen



1. 1.  

Pendidikan : Dokter atau Dokter Spesialis, dengan S2 Manajemen Rumah Sakit Pengalaman :   



Pernah menjabat sebagai Direksi dan Pernah bekerja di Rumah Sakit sekurang-kurangnya 5 tahun, atau  Pernah menjabat sebagai pejabat struktural 1 (satu) level di bawah Direksi Rumah Sakit dan bekerja di Rumah Sakit selama 10 tahun, atau  Pernah aktif sebagai Ketua Panitia/Komite Mutu di Rumah Sakit dan bekerja di Rumah Sakit selama 10 tahun, atau  Aktif dalam penyiapan akreditasi Rumah Sakit di Dinas Kesehatan Propinsi/Kementerian Kesehatan dan bekerja di Rumah Sakit selama 10 tahun Pernah mengikuti Workshop SNARS yang dibuktikan dengan sertifikat.

Kriteria Surveior Medis

1. Education: Doctor or Specialist Doctor, with Master Degree on Hospital Management 2. Experience:  Has served as the Director Board and has worked in a hospital at least 5 years, or  Has served as structural official 1 (one) level below the Hospital’s director boardand has worked in a hospital for 5 years, or  Has actively served as the Head of Committee/Quality Committee in a hospital and has worked in a hospital for 5 years, or  Has actively participated in hospital accreditation preparation in the Provincial Health Office / Ministry of Health and has worked in a Hospital for 10 years 3. Have attended SNARS Workshop proven by certificate. Requirement qualification for medical surveyor :

Pendidikan : Dokter Spesialis Pengalaman : Pernah bekerja di Rumah Sakit sekurang-kurangnya 5 tahun sebagai fungsional dokter spesialis dan pernah aktif pada Komite Medis. Syarat lain : tidak pernah mendapat sanksi disiplin oleh MKDKI dan di tindak etik oleh MKEK Pernah mengikuti Workshop SNARS yang dibuktikan dengan sertifikat.

1. Education: Medical Specialist / Specialist doctor 2. Experiences : o Have worked in hospital 5 years minimally as functional specialist doctor and active supported Medical Committee o Others requirement: Never got to disciplinary sanctions by MKDKI (Assembly Of Honor Of Indonesian Medical Discipline) and ethics followed by MKEK (Assembly honorary medical ethics) 3. Have attended SNARS Workshop proven by certificate
