IDENTIFYING DATA: JM, 28 years old, Male, Married, Bank teller, Filipino, Roman catholic, Born in Manila on May 20, 1989, residing at Batasan hills Quezon City. Admitted first time in FEU-NRMF on September 10, 2016. CHIEF COMPLAINT: Heartburn HISTORY OF PRESENT ILLNESS: The present illness started 1 week prior to admission when the patient felt indigestion which was present 30 minutes to 1 hour after meals and with reclining. This was accompanied by epigastric tenderness 5/10 on the pain scale. No medications taken and consult made. 3 days prior to admission, patient still felt indigestion and epigastric tenderness but increased in severity, with a grade of 7/10 on the pain scale present after meals and waking him most nights. This was accompanied by regurgitation of acidic fluid and heartburn. No medications taken and consult made. Few hours prior to admission, patient presented with similar symptoms of indigestion, epigastric tenderness, heartburn. Regurgitation of acidic fluid that is blood streaked was noted and this prompted the patient to seek consult in FEU NRMF Medical Center where he was subsequently admitted. PAST HEALTH HISTORY: - Had Chickenpox and Measles during childhood - History of Surgery for ACL tear FAMILY HISTORY: - Mother, 55, diagnosed Hypertensive - Father, 58, apparently well - Wife, 27, apparently well PERSONAL AND SOCIAL HISTORY: Patient is married for 2 years. Regular exercise such as jogging for 30 minutes every morning, playing basketball once a week. The patient is a 4 pack year smoker. He is an occasional alcoholic beverage drinker, once a month, consuming 2 bottles of san Miguel pale pilsen. Fond of eating fried, salty and spicy food, and rice. He takes Multivitamins everyday. He drinks 3 cups of coffee everyday and sleeps 6 hours a day from 11 pm to 5 am, interrupted sometimes by abdominal discomfort. Lives in a 2-story house that is well ventilated. Drinks potable water. The household observes segregation of refuse/litter with disposal and collection twice a week. No history of illicit drug use and sexually transmitted infections. BP: 120/80 mmHg HR: 90 bpm RR: 20 cpm Temp: 37.0 C Weight: 60kgs Height: 5’6 or 167 cm PHYSICAL EXAMINATION and LABORATORY FINDINGS: NORMAL
