Cruz, Davao Del Sur after having been subscribed to an oath, depose and say: 1. That I am hired as WELDER PIPE for QATAR KENTZ WLL, a registered principal/employer of RENSOL RECRUITMENT AND CONSULTING, INC; 2. That as a WELDER PIPE, there will be a trade test in the jobsite to determine my skills and qualifications for my job; 3. That I fully understand, recognize and agree that as a WELDER PIPE pursuant to my Master Employment Contract with QATAR KENTZ WLL, I may be terminated for failure to pass the required trade test in the job site; 4. That should I be terminated for failing to pass the trade test in the jobsite, I will not file any civil, criminal and administrative case against RENSOL RECRUITMENT AND CONSULTING, INC. its officers/ staff and QATAR KENTZ WLL and its officers/staff from any liability of whatsoever kind and nature to any government in this regard; and 5. That voluntarily I execute this Affidavit of Awareness and Consent without any reservation on my behalf.
