LETTER OF CONSENT
I, _____________________do hereby allow myself to be a part of the study after hearing the thorough explanation of the student nurses of Lorma Colleges.
Without hesitations, I will allow myself to be physically assessed and be interviewed regarding my case. I will cooperate in any terms that would further clarify the study provided that it would not aggravate my existing condition and would not, in any form, invade my privacy.
