Quality Site Observations 2.1 United Precast Concrete (UPC) Open NCR BSBG noticed that target dates tabled on previous meetings by AFC are getting revised and extended week in and week out in respect of the first meeting wherein the dates were initially proposed and are delayed from thereafter and as it currently stands. New proposed target dates and updated status of the NCRs are reflected in below table.

Pending SOR’s & NCR’s till date

NCR: BSBG noted and tabled the pending NCRs which are open either due to no proposed corrective action given or the corrective actions have been agreed with BSBG but the implementation at site is still incomplete. Refer to NCR register. SOR: BSBG noted that a total of 21 Nos. of SOR are open since September 2018 up until December 2018. Most of the SORs were responded by AFC with corrective action and were returned by BSBG with noted acceptance. From thereafter, no further responses were made on records. Refer to attached SOR register. BSBG noted that the escalation from SOR to NCR is mandatory, due to failure to respond and close the related SORs as agreed in the last meeting.

Recurrent Quality Issues found during weekly walkthrough

BSBG noted that below issues which are again prevalent during the site walk. 1. Inadequate Protection of Screed 2. Incorrect material used: Hollow blocks used instead of solid blocks 3. Unacceptable block cut pieces 4. Restraint post incorrect rebar size for external blockwork 5. Soffit cleaning not done prior succeeding works i.e. MEP services installation 6. Quality of concrete cast (stiffener columns and beams) 7. Blockwork CJ not as per IFC drawing. 9. Inadequate and worse without lateral and head restraint for blockwork 10. Hand mixing blockwork mortar 11. Incorrect blockwork accessories used for external blockwork Additional Item found: 12. Secondary steel member for MEP supports commenced without approved shop drawing, MIR for steel members, Calculations etc. 13. Reinforcement steel stacking in Zone 5 2.4

Focus Area of NCR Close-out: Zone 1

Parallel to above item 2.1, BSBG noticed that target dates tabled below by AFC are getting revised and extended on similar account in respect of the first meeting. The target dates proposed were missed subsequently hence are revised and extended by AFC whose new target dates are given below heretofore. BSBG remarked that delay of rectification and response is unacceptable and must be sorted out immediately pursuant to the contractual obligations. NCR

External Audit BSBG confirmed that an external audit will be undertaken to review AFC compliance with project QMS and its effectiveness. Time and date to be confirmed – Note.

Quality Issues found 3.1

Joint Quality Site Walk-Through BSBG noted that joint quality report no. 2 and 3 were responded last week. BSBG to review and return by next meeting. AFC committed to submit report no. 1 responses within the week. BSBG to verify and discuss the response by next meeting. Report for the latest conducted site walk is herewith attached

3.2

Construction Team not in attendance during Site walk BSBG highlighted that that no representative from the site team has responded and joined for the recent site quality site walk held on 17 Feb 2019 despite incessant calls from BSBG Engr and Muthu (AFC QA/QC Engr). The site walk was intended to assist and provide the site team proactive measures to avert any recurrent issues thus diminishing the issuance of further NCRs and SORs of persistent nature but will not be effective unless otherwise their involvement is fully materialized. AFC have noted that internal instructions were given, and all PM’s are now required to attend the site walks from next week onwards.

4.0

Activities based from 2 week look ahead BSBG noted that dry wall partitioning, tiling, secondary steel supports installation, painting for internal finishes, cladding, and EIFS system installation are some of the activities to commence shortly. AFC to conduct a review on the status of required submittals prior to commencing to avoid any SORs/NCRs.

5.0

AFC Internal trainings/toolbox AFC noted that 2 nos. of internal toolbox talks were conducted for blockwork. BSBG noted that training records has to be maintained and available for future audits.

6.0

External Training/toolbox AFC to share any details of external (supplier) training workshops with BSBG by next meeting

AFC Quality Personnel AFC QA engineer will back from leave on 27-02-2019
