On Jan 15th2014, at 04.07 ocal time , Perkins Generator# 2 at NUA-1 suddenly tripped due to sensor speed control open breaker indication then causes ESP NUA #1 tripped.

On 19 January 2014 Turbine Generator #3 at FF tripped Unsafe action :N/A and caused all ESP wells in Oseil #1, #2 and #4 area condition: N/A tripped as wells and some equipments in FF. The turbine Generator # 3 black-out suddenly tripped due to low supplied fuel gas to turbine generator, auto fuel switching was failure to transfer from gas to diesel & Turbine Generator #2 tripped was under frequency indication

1. Check alarm indication on TG-3 & TG2 2. Replaced pressure transmitter 80-PT-242 with new one. Ok 3. Check gas module both TG# 2 and TG#3 (Found nothing problems on gas module) 4. Check Voltage regulator system of TG#2 OK (without load and with load on diesel) in parallel but no tested yet when stand alone with full load.

Descripition On dec 02, 2014 at 18.00 LT Mr.Ibrahim kalderak as SL Crew change bus driver , driving his bus to Bula for crewchange , when arrive at the boom gate , some one was left at MPF , he decided to moving back to MPF . Without any consideration, suddenly he reverse his car at the boom gate area , in the same time , Triton B , driven by Pak Jamal Paloh was positioned behind the bus , finally, left side Triton B door hit by the bus and cause dented on the both vehicle body. No body injured in this incident. (Please see the illustration attached for more detail)

On January 8th, 2014. LCT Maju Jaya was berth in Jetty unsafe action : N/A unsafe Cargo, the unloading activities was held due to bad condition: Stroms or acts of weather. One strong wave slammed the LCT against Jetty nature edge and causing big impact to the jetty cargo edge. The LCT cannot be released from jetty cargo due to stranded cause by low tide condition. This incident causing damage to the edge of jetty cargo. (picture attached)

On Jan 10, 2014 at 04.45 local time. Karsono the driver has some trouble to start the Bus engine and he asked to another person in rig site to help him push the bus to lit the engine.There are miss communication between the driver and 2 persons he had asked for help until the bus pushed forward and move freely without control because the driver was not inside the cabin yet. the Bus then stopp after strucked against stationary object (parked Vacuum Truck) in rig site area. nobody injured caused by this incident.

Contain the leaks from vacuum truck engine starter); Inadequate area for parking fuel tank, move the bus to safe position (parking in sloping area); Inadequate identification of critical safe befaviors Job Factor : Inadequate repair / maintenance (broken engine starter not resolved properly); Inadequate removal/ replacement unsuitable item (Bus has been recommended to be replaced but not performed yet); Inadequate communication between workgroup (in adequate information and Communication between bus driver and oter persons involved in incudent)

On January 12, 2014 around 22.30, Arifin driver of Unsafe action :Violation by Ranger L 9744 V went out of lofin project site to drove individual one of employee to his house/ lodge then he planned to Unsafe condition: N/A visit his familiy in kobisadar. On intersection of road to kobisadar, he fell on micro-sleep and loss his control to the vehicle, until strucked the obelisk fence caused major damage to the vehicle and fence. no one injured caused by this incident

On Jan 26 2014 at 14.20 local time ,Mr. Arman Hukom as Unsafe action :N/A Unsafe Personal factor : N/A Job FEC Dalenta sandblaster , while adjusting 2" condition: Inadequate Factor : Inadequate standard/Specification sandblasting discharge hose in New MPF skim pond area equipment , the connection between discharge hose and sandblasting engine nozzle suddenly loose , it cause the silica sand from the loosing hose spraying to his face directly , it cause sporadic wound on his face, when the incident occurred he used the proper PPE 3M Mask double cartridge and safety goggle , immediately he brought to Wayhul clinic to got the medical treatment by the CSEL Doctor as First Aid Case .

1. Install extra fender (tire fender) to protect impact 2. Move out the LCT from jetty cargo area once have chance (high tide) condition

1. Keep implement not allowing any boat to be along side during bad weather (strong wave)

1. Reinforce safe behaviour of all method carrying for job 2. Vehicle inspection conducted to all vehicle in drilling site and following to other site in csel area, reject for any unsafe found 3. Reinduction all involved persons, refresh for safe behaviour and appropriate working procedure

1. Remove the vehicle to safe position 2. 1. Night driving permit to be Secure the main road make it accessible. implemented in lofin project site, acknowledge by Lofin project PIC 2. Personal warning and safety behaviour induction/ briefing.

On January 07 , 2014 at 10.00 Local time , Mr. Rasul as ASN HE operator , operate Excavator Hyunday 320 LC 2008 as additional HE from ASN to support land clearing activity ,while hauling the material to dump truck , suddenly the 1,5 " size of Boom hydrolic hose was loose from its clamp then the hot hydrolic oil spraying from the hose to the hot surface of Turbo & mufler of the engine that positioned behind the hydraulic hose , immediately it cause the fire around the engine mufler ,no any Fire Ext. on the HE it self , the operator calling Hasan Dawangu as ASN HSE Officer, then he brought the Fire extinguisher 6 kg as much 3 unit from FEC Site office , supported by Mr. Mukhlis and Tasori from FEC Dalenta , all of them went to well pad immadiately to extinguish the fire . No body injured in this incident.

1. Calling ASN HSE for help , supported by FEC Dalenta personnel in FEC Lovin # 2 site office area 2. Brought Fire Extinguisher 3 ea from FEC site office to extinguish the fire 3. Brought the excavator to ASN workshop for repair.

1. Provide FE to prevent the Fire on each HE in Lovin # 2 wellpad area 2. Provide schedulled PM for HE equipment in regular basis 3. Provide the proper HE in Lovin # 2 wellpad for construction area 4. Reinspect all hydraulic hose of ASN HE in Lovin # 2 wellpad area

On January 25 , 2014 at 11.04 Local time , Mr. Jarno as ASN HE operator , while commencing to operate the Excavator Catterpillar PC 300 , ASN No. 35-012, as additional HE from ASN to support land clearing activity ,suddenly the 2 " size of hydraulic cooler hose was broken, then the hot hydrolic oil sucked out by radiator blower and sprayed to the hot surface of the engine & mufler , immediately it cause the fire around the engine mufler ,no any Fire Ext. on the HE it self ,at the same time , Mr. Ali Paittaha as FEC Dalenta site spv passing beside the smoked excavator , he decided to took the Fire extinguisher 6 kg as much 1 unit from FEC Site office and extinguish the fire . No body injured in this incident.

On January 27 , 2014 at 10.15 Mr. Karim Rumeon as BMS Truck driver was inted to go to Bula , starting from HSE camp , when passing the three junction of OM Office area , he was unaware that the left side of his truck has struck the rear side body of Triton # 6 that park beside OM office , it cause dented on the rear side body of Triton # 6. No body injured in this incident.
