Note that PEA can look like any rhythm (any organized electrical activity), but if no pulse it is PEA

Course Ventricular Fibrillation

Note the rate is > 100 bpm

Reentry Supraventricular Tachycardia

This is a regular, narrow complex tachycardia without P waves with a sudden onset and cessation

Monomorphic Ventricular Tachycardia

Note the rate <60 bpm. Could be physiologic or symptomatic depending on the patient.

ขั้นตอนการทำา ACL S ณ รพ.นาเชือก

ทีม ACLS. ประกอบด้วย 1. D = Doctor 1 คน 2. N = Nurse 3 คน N1 = In chart N2 = ผู้ร่วมอยู่เวร N3 = ผู้ที่ถูกตามให้มาช่วย 3. A = Aid 1 คน 4. W = Worker 2 คน

เครื่องมือและอุปกรณ์

1. ประเมินการหายใจ ถ้าไม่หายใจ ช่วยหายใจ 2 ครั้ง โดย N 1 ส่วน N 2 notify แพทย์ และตาม N 3 ทันที

2. ประเมินชีพจร โดย N1 ถ้าไม่มีชพี จร ให้กดหน้าอกโดย W1 สลับกับการ ช่วยหายใจ 30:2 ทำาติดต่อกัน 5 ครั้ง นับเป็น 1 รอบ ( 2 นาที )แล้วประเมินชีพจรซำ้า ( ให้ W1 สลับกับ W2 ในกรณีที่ทำาต่อไม่ไหว)

3. ติดเครื่อง defibrillator โดย N2 ในระหว่างที่หยุดเพื่อประเมินชีพจร แล้วเตรียมอุปกรณ์สำาหรับใส่ท่อช่วยหายใจ

4. N3 ทำาการให้สารนำ้า โดยใช้ 0.9%NSS โดยไม่ขัดจังหวะของการกดหน้าอก ส่วน N2 เตรียมเครื่องวัด BP

5. เมื่อแพทย์มาถึง ถ้ามี indication ในการทำาdefibrillation ต้องทำาก่อน โดย shock ที่พลังงาน 360 จูล 1 ครั้ง ถ้าไม่ทำา defib ให้ใส่ท่อช่วยหายใจ หลังใส่ให้กดหน้าอกด้วยอัตรา 100 ครั้ง/นาที และบีบ Ambu bag ด้วยอัตรา 10 ครั้งต่อนาที ไม่ต้องสัมพันธ์กัน

6. แพทย์ประเมินชีพจรทุก 1 รอบ ทำา defib ตามข้อบ่งชี้ โดยใช้พลังงาน 360 จูล สลับกับการ CPR และให้ยาตามข้อบ่งชี้ โดย N2 เป็นผู้เตรียมยา ส่วน N3 ทำาหน้าที่วัดความดันโลหิต และ suction

ทา electrode gel หรือ cream หรือ saline soaked gauze บน paddleก่อน ระวังอย่าให้ cream ไหลมาเชื่อมกันระหว่าง 2 paddle

7. ทำา CPR ต่อจนกว่าจะมีการเต้นของหัวใจกลับมา หรือไม่มีการตอบสนอง จึงทำาความเข้าใจกับญาติ แล้วพิจารณาหยุด CPR (แต่ไม่เกิน 30 นาที )
