Tata laksana case manager Alur Pasien masuk melalui ugd 1. Pasien cito tindakan : perawat ugd yg melakukan scoring 2. Pasien tidak cito : perawat ruangan ( ka- tim ruangan ) yg melakukan scoring Pasien masuk melalui poli : perawat ruangan (ka-tim ruangan) Setelah dilakukan scoring, pasien dengan kategori resiko sangat berat, dilaporkan ke case manager. Selanjutnya case manager mengevaluasi kembali keadaan pasien. Jam kerja case manager 07.00 – 14.00. Diluar jam kerja tetap bisa melaporkan/ menghubungi. Bila dalam masa perawatan pasien terjadi perubahan kondisi, maka dapat dilakukan scoring ulang.
