STUDENT SAFETY PLEDGE SHEET/SAFETY PERMIT Your Name (Please Print)_________________________________ Who is enrolled in the Industrial Technology Program, will as a part of his/her class experience, operate power equipment, providing that his/her parent or guardian gives written permission. It is understood that each student will be given proper instructions both in the use of the equipment and in correct safety procedures concerning it, before being allowed to operate it himself/herself. The student must assume responsibility for following safe practices, and we therefore ask that he/she subscribe to the following safety pledge: Follow the teacher’s safety instructions at all times. Do not behave unsafely in the technology room, lab area or around any tools or equipment. Pay close attention to what you are doing at all times. Do not let others distract you while you are using any classroom tools or equipment. Always wear safety glasses and other eye protection in any situation where you are working with hand tools, power tools and other equipment or hazards that could cause eye injuries. Ask your teacher for help. Never use any tool or equipment without permission by your teacher. Do not use a tool unless the teacher has instructed you in its use. Do not follow the advice of anyone other than the teacher when using tools or equipment. Remember to ask the teacher for help if you are not sure about an operational procedure. Be careful not to wear loose clothing, jewelry or other items that could get caught in a tool or machine. Always use the guards on each machine. Keep hands and fingers away form all moving parts of tools and equipment. Keep hands and fingers away from all cutting instruments/blades/drills, etc. Report all injuries to your teacher at once.

STUDENT TECHNOLOGY PLEDGE SHEET Your Name (Please Print)_________________________________ This student's Public School District has developed the following technology guidelines for the Industrial Technology Department. By adhering to these guidelines, educational experiences can be enhanced. 1. Access, receipt, transmission, or display of offensive material or messages including, but not limited to, sexually explicit images, messages, cartoons; use of ethnic slurs or racial epithets; defamatory, abusive, obscene, profane, and/or threatening language; inaccurate information; encouragement of use of controlled substances; or illegal material are prohibited. 2. Use of technology for solicitation, commercial, religious, political, personal or any other non-educational activity is prohibited. 3. Use of technology must occur within the parameters of school district policy and local, state and federal law and must not disrupt the educational process. This includes attempts to gain unauthorized access from within or outside of the District, to proprietary computer systems including, but not limited to: individual computers, network file servers. Adherence to copyright laws is mandatory. 4. Introduction, transmission or creation of any computer "virus" using School District technology is prohibited. 5. Any financial obligations incurred by persons using District Technology are the responsibilities of that user. 6. If a student in grades kindergarten through grade 12 applies for an account to an on-line technology service such as "Internet" through the District, the student's parent/guardian must sign a "User Account” agreement with the institution providing the service. The parent/guardian signature indicates that the parent/guardian recognizes that guidelines for that service exist, the child will follow those guidelines and approval for the child to access an on-line service using school district technology has been given. 7.
Persons who violate any of the District guidelines are subject to disciplinary measures.
