Abstract This document has been produced by the AWS Website working group to gather information on pupilʼs opinions for a website accessible for all students at Akeley Wood Senior School. The website built would be accessible in and out of school but only for pupils of the school. This Questionnaire is one of two used to gather user opinions of both staff and pupils, any information will be taken into account when designing the website. Introduction - (for form teachers) Welcome, please can you ask your form the following questions. Most questions will use a scale from 1-10 (see example below), for these please take a majority decision from your form. For other questions please detail the main points raised. 1

Majority Answer If you find there is something that is missing on this questionnaire that should have been included please use the blank page at the end of this questionnaire. This questionnaire was designed to be completed within 1 morning registration and all completed questionnaires can be simply returned with your register. This questionnaire consists of 4 sections Introduction - (for pupils) Welcome, this questionnaire is designed to gather pupil opinions for a website that will be built for pupils of the school. What this website would show is largely up to you and as such we are asking for your suggestions. This questionnaire will be answered by one form per year group to make sure that all pupils of the school are included. Those suggestions that are popular and suitable for a school website will be aimed to be included where possible. We are currently aiming that this website would be completed by the end of the winter term . Page 1/5
