Every action and decision performed by this position is founded on respect for the residents, for the promotion of their dignity, individual freedom, choice and lifestyle, and meaningful quality living. The goal of this position, as is for all of the Continuing Care team, is to create a feeling of home and belonging for all who live in Yukon Continuing Care facilities by being responsive to the uniqueness of each resident. Section 2 - General Summary As a member of the Transdisciplinary team, the Licensed Practical Nurse provides care services to support Continuing Care residents and clients according to the Continuing Care philosophy of care and within legislated scope of practice. The Licensed Practical Nurse is team leader on designated shifts. Section 3 - Organizational Structure Describe, in narrative form, the place of this position in the organization, including peers (those positions reporting to the same supervisor as the topic position), subordinates, and supervisor's supervisor. This position is one of several indeterminate and auxiliary Licensed Practical Nurse positions who report to a Program Manager, who reports to the Director, Continuing Care (15-711). Peer positions are Registered Nurses, Nursing Home Attendants, Therapy Assistants, Domestic Aides, and Kitchen Helpers.

Section 4 - Principal Duties and Responsibilities (limit to 10 Duty Statements) 1. Provides care to residents promoting their physical, emotional and spiritual well being, according to established philosophy of care, policies, procedures and legislation and individual resident need. This is achieved by: • Respecting and promoting individual choice and lifestyle and the feeling of home and belonging. • Developing therapeutic relationships with residents and clients, and communicating with residents with varying communication capabilities. • Assessing through observation, interview, palpation, inspection, auscultation, collection, and testing of specimens. • Participating in planning care. • Providing appropriate interventions according to care plan and best practice. • Administering medications and treatments. • Processing and verifying written orders. • Assisting residents with Activities of Daily Living. • Supporting, planning and/or leading daily leisure and productive activities that contribute to the quality of life for clients. • Documenting on health records, and sharing health information appropriately with other team members and disciplines. • Evaluating client focussed goals and effectiveness of care. 2. Performs and contributes to the development and well being of the Transdisciplinary team by: • Developing harmonious relationships within care teams and across all programs and departments. • Managing conflict in a timely and appropriate manner. • Actively supporting the Transdisciplinary care approach through demonstrated practice. • Maintaining effectiveness when experiencing change in work tasks, equipment or work environment. • Providing leadership by demonstrating tasks to colleagues, training other employees in work procedures, establishing daily work priorities and schedules, participating with program manager or Registered Nurse in employee performance evaluation, providing ongoing feedback. • When designated team leader, providing organizational support such as replacing staff as required; assigning work and reviewing quality/quantity of work. 4. Communicates and demonstrates philosophy of care to residents, community members and colleagues. 5. Maintains professionalism by: • Continuous professional development and improvement in personal performance and quality standards. • Preceptoring, mentoring, orienting and training of co-workers and students. • Other duties as related to the philosophy of client care and program goals.

Section 5 - Contacts Describe, in narrative form, the working relationships inherent to the success of the job. ♦ This position is part of a transdisciplinary team (Registered Nurses, Licensed Practical Nurses, Nursing Home Attendants, Domestic Aides, Kitchen Helpers, Cooks, Physiotherapists, Occupational Therapists, Recreation Therapists, Therapy Assistants, Speech Language Pathologist, Social Workers, First Nation Liaison Worker, residents, Program Manager and other Support Services). This position coordinates care, provides feedback, exchanges information, and participates in the development of a plan of care on a daily basis. ♦ Daily contact with residents to exchange information, provide care, teach and counsel. Also to assist with activities of daily living in a manner that respects their privacy and dignity. ♦ Daily contact with volunteers, visitors, families, and the general public to exchange information, teach, and counsel. ♦ Weekly contact with physicians, pharmacists, and hospital staff as well as other professionals such as dentists, optometrist, and speech pathologist to exchange information. Section 6 - Problem-Solving List examples of problems, issues, or complexities most typically encountered in the job AND describe the actions this job takes to resolve each problem listed. 1. When delivering care to residents, the position is responsible for ensuring that the Continuing Care philosophy, Continuing Care policies and procedures are carried out, while ensuring they are working within the Scope of Practise as outlined by the Yukon Nursing Assistant Registration Act. 2. Position must use safe and proper techniques such as back safety when performing resident transfers, handling sharps. 3. Position must use clinical skill to observe and report changes, response to treatment in residents’ health status, and communicate progress and concerns with the transdisciplinary team and contribute to the care planning. 4. The position is required to make recommendations and consult with the Transdisciplinary team as to specific care of the residents, e.g. Treatment of a pressure area. Final decisions are made by the Registered Nurse/Team Leader, Program Manager, and/or physician. 5. Prioritize the order in which the position carries out their day to day duties 6. Where unsafe or emergency situations (i.e. fire alarm) occur, this position performs their role, while implementing Fire/Disaster/Emergency procedures.

Section 7 – Specific Accountabilities 1.

Describe final decisions regularly made for which the incumbent is held accountable. The incumbent is accountable for making those decisions affecting some nursing care provided to residents, such as dressing changes, administration of medications, taking of vital signs. When team leader, accountable for making decisions related to resident care, staffing and security of the program.

What are the expected end results of this position, i.e. what are the impacts of performing the Principal Duties and Responsibilities? Those decisions would impact on the safety, health and quality of care the residents receive. When team leader, decisions impact team functioning and security of program.

Section 8 - Budget This position has no direct financial accountability. However, the incumbent must be sensitive to the high cost of supplies and equipment and extra staffing, and conduct activities accordingly. Section 9 - Working Environment Describe in narrative form the position's working environment which can be generally favourable or include exposure to undesirable conditions. 1.

Physical Effort

Position assists with transferring and repositioning residents weighing between 100-300 pounds 15-20% of the time, although there are lifts that can aid with the transfers. Position stands, stoops, bends, walks, sits, reaches, and pushes (wheelchair, beds, lifts, gerichairs), approximately 75-80% of the day. Position lifts boxes (supplies) up to 25 pounds approximately 5% of the time; 2.

Physical Hazards

There is a potential to exposure of infectious agents 100% of the time in the handling and disposing of body fluids or treatment of residents with infections, but attention to task at hand and following procedures such as standard precautions, reduce that likelihood.

Discomfort

The temperature of the facility is maintained at approximately eighty degrees Fahrenheit, which may cause some discomfort. There is some exposure to animals in the Pet Therapy Program and to second-hand smoke from residents, which may also cause some discomfort. There is a high level of emotional residents/clients/family members. There is a high degree of emotional stress caused by dealing regularly with situations of loss, terminal illness, crisis, depression, abuse and unpredictable behaviour. There is some potential for exposure to emotional, physically and verbally abusive, dissatisfied residents/clients, and/or family members. Exposure to strong and unpleasant odours or fumes; regular critical deadlines; constant interruptions; instructions from more than one source; high degree of stress related to various emergencies and critical decision-making
