OCCUPATIONAL HEALTH & SAFETY POLICY General Statement Management MES-TSW JO regards health and safety at work as a matter of foremost importance and will ensure that all reasonably practicable steps are taken for the continuing health, safety, and welfare of our employees and any other persons who may be affected by our construction activities. Company will allocate the resource necessary to ensure full implementation of this OHS Policy. MES-TSW JO re-affirms project site commitment to health and safety and gives effect to the policy through the safety management system which: Ensures compliance with relevant statutory legislation and safety requirements. Promotes safe working practices through regular use of the Construction Safety Manual (CSM) Provides monthly safety statistics to measure the safety performance. Sets health and safety objectives. Engages the cooperation of employees at all levels through consultation, training, personal examples, the promotion of safety awareness and regular involvement of safety department staff at all stages of project from inception through design, planning, and execution to completion and maintenance. The responsibility of Project Site Management in implementing this OHS Policy is regarded by MES TSW JO as fundamental to the company’s main objectives of operation in an efficient and profitable manner. A safety manager with responsibility for health and safety is appointed by the company. He will ensure the provision of an effective organization and arrangement for the implementation of the OHS Policy. Responsibility of monitoring compliance with this OHS Policy rests with the Safety Manager and his Project Safety Officer. This is accomplished by regular site inspections from the project management staff, reports by safety supervisors, and by planned schedules of safety audits conducted by company auditors. This OHS Policy will be reviewed annually as a minimum and will be revised as often as may be appropriate. MES-TSW JO

SCOPE The safety precautions contained in this OHSS Plan will serve as standard reference for all construction employees and other personnel concerned, to make the new Tanjung Jati B Coal Fired Power Plant Expansion Project Unit 3 & 4 (2 x 6600 MW) work site as safe place as possible for all workers during the course of the entire construction period.

OBJECTIVES o
Structuring the project HSE Department within the Project Organization to ensure that all Health Safety & Environment aspects are conscientiously and actively managed.
o
To organize Safety Steering Committee comprising of key supervisory staff to coordinate all Health Safety & Environment (HSE) activities within our project in order to enhance general safety performance.
o
To develop education and training programs throughout the construction period so as to instruct our workforce in maintaining safe and healthy working conditions.
o
To implement and conduct daily toolbox meetings among the workforce aimed to prevent and reduce accidents. The most cost-effective away of doing this is by concentrating upon human factors.
o
To discuss all incident and accident reports arising from unsafe actions, unsafe conditions, near misses, serious and minor injuries including first aid cases.
o
To promulgate site safety auditing amongst employees and subcontractors aimed at improving safety and preventing accidents.
o
To create safety awareness with all staff personnel including line supervision and subcontractors by promoting group discussion and problem solving sessions on all issues pertaining to improved safety, efficiency and relationship.
o
To ensure standardization and continuity of efforts toward safety.
o
To enhance communication up and down the line on safety matters.
SAFETY AND ENVIRONMENTAL TARGETS MES-TSW JO shall strive to complete his work with a zero fatality record. Safety will be an issue in every meeting on all levels. In taking steps to ensure a safe working environment, MES-TSW JO Management and Subcontractors shall aim to/for: a. No Fatality b. Reducing Lost Time Incident (LTI) and accidents. The rationale behind this is that all accidents are preventable. c. Provide a safe working environment so that all employees, staff personnel and subcontractors are adequately protected.

RESTRICTED AREA o
The area within the boundaries of the Tanjung Jati –B- Expansion Project MES-TSW JO Project Construction Work site is classified as a “RESTRICTED AREA”.
o
All visitors, newly recruited employees and workers are required to report to the Security Department at the gate entrance for security check prior to entry into the project / construction office or the restricted area.
o
The construction site and the entire restricted area is classify as “HARD HAT AREA”.
o
Wearing of appropriate personal protective equipment (PPE) such as helmet, protective footwear, safety belt and protective eye goggle are mandatory and also a standard safety procedure when a person is exposed to any risk or hazard within the construction surroundings.
o
During the construction period, commissioning or handing over of site facility, no one shall enter any part of the CLIENT’S PREMISES or PLANT FACILITY including the work site, other than for the purpose of carrying out the work. All personnel needing access to the restricted area will be required to obtain a Special Security Pass issued by Project Management and to show it, if requested.
IDENTIFICATION OF PERSONNEL o
All employees engaged at the project construction site must wear identification name badge.
o
For employees of Subcontractors, Identification badges will indicate the names of employees and the subcontractor’s company they belong to.
o
Identification badges will be given to all employees by MES-TSW JO when they have completed safety induction training.
o
All personnel employed and working on the Project construction site are required to submit three (3) copies KTP size photographs to MES-TSW JO for processing of identification badge.
HEALTH AND SAFETY AWARENESS PROGRAM 6.1
Safety Induction Safety officer is responsible for conducting safety induction for new employees (at all levels) that being designated in the project before starting activities for the first time. Safety officer shall inform the new employees all potential hazards that may present in the premises, how to control hazard, and how to prevent incident. Safety Induction shall be documented mainly the attendance list and induction topics. All subcontractors shall submit details of its employees (key person, staff, and workers) to MES-TSW JO for safety induction and issuance of identification badges.
6.2
Toolbox Meeting Attendance of Toolbox Meeting is a group of workers involved directly in the work-place. Toolbox Meeting can be conducted at any time with relatively short duration of time about 5 to 10 minutes and taken place in the premises. Toolbox Meeting should be conducted either by supervisor or safety officer in simple way related to daily activities with emphasize to OHS to

be implemented in every works carried out. Toolbox Meeting being conducted shall be documented mainly the attendance lists and the topics being discussed. Topic for Toolbox Talks should include but not limited to: a. Good preparation of work. b. Identification of hazards of activities being carried out. c. Deviation from relevant work standards, practices, procedures, legal requirements, etc found out during inspections. d. Incident with its chronology and prevention action to be taken. e. Instructions and information from project management or client. f. Use of safe equipments and tools. g. Safe materials storage and house-keeping. h. Legal law, procedures, standards, etc. i. Tidy up work area and tools/equipments at the end of work shift.

Safety Mass Meeting Safety mass meeting is attended by managements and employees of main contractor and subcontractors. Safety mass meeting is held regularly at least once a month. A topic of speech related to OHS Philosophy or implementation shall be communicated to all employees by management representative. Safety mass meeting shall be documented mainly the attendance lists, the topics of safety speech, and photographs.

Health and Safety Promotion and Campaign. Safety Committee shall regularly promote and campaign for OHS. Company shall grant award for subcontractor or employee with the best OHS performance.
6.5
Health and Safety Trainings Safety Officer shall generate health and safety training. Training shall be conducted regularly in a certain period of time. Health and Safety Training shall include but not limited to the following: System of Good Housekeeping, Working at Elevated Heights, Personal Protective Equipment, Occupational Health and Safety in Construction, Fire Prevention and Control, Fire Drill and Evacuation, Incident Investigation and Reporting, First Aid Treatment, Safe Use of Equipments, Safe use of hand-tools. Project management shall ensure that all his personnel and Sub-Contractor’s personnel under its employment have been given the necessary safety training required by law and be familiar with MES-TSW JO OHS Plan prior to the start of the work.
ALCOHOL AND DRUGS MES-TSW JO Project Management shall ensure that at any times during the performance of the work, its personnel are neither under the influence of nor partake of any alcoholic liquor, drugs or other intoxicating substances, other than for bona fide medical reasons. Anyone found under the influence of alcoholic, drugs or other intoxicating substances will be removed from site.
8.
MEDICAL EVACUATION & EMERGENCY RESPONSE 8.1
Medical Evacuation MES-TSW JO shall establish medical arrangement with the relevant hospital authorities which are located nearby the project site in the event of an emergency. In particular, the Project management will ensure that all it staff / personnel are familiar with the Essential Medical Evacuation and Emergency Response Procedure. A schedule showing who is responsible, what to do in case of emergency, and whom to inform will be placed at the site entrance and the office building at highly visible locations.

An emergency first aid facility and ambulance shall be provided by others. If in the early stages this facility is not functioning each subcontractor shall establish adequate facilities to serve this purpose, such as: a properly equipped first aid kit in a defined and marked location; an employee trained in first aid and identified as such; a vehicle that will serve as an ambulance in the event of an accident. 8.2

Chief Chief of Emergency Response Team shall be responsible for the whole process of emergency activities including trainings for the team, simulation, and drill. Chief of Emergency Response Team shall lead the team in coping with the emergency situation. Chief of Emergency Response shall take decision to call for help from fire department, help from hospital, or help from police station. Chief of Emergency Response Team shall make reports of emergency situations. Evacuation Group Evacuation Group shall arrange evacuation process and guide people to the assembly point. Evacuation Group shall coordinate with First Aid Group to evacuate injured people. Evacuation Group shall try to cope with the emergency situation and coordinate with chief to call for help from related department. Evacuation Group should try to save assets. Evacuation Group should inventory assets and its defects. First Aid Group First Aid Group shall check if any person injured in the incident. First Aid Group shall give first aid to injured person and evacuate him to save place. First Aid Group should escort injured person to hospital if need to get further treatment in hospital. First Aid Group should inform the family of the victim if necessary. First Aid Group shall prepare report of injured persons. 8.3
Identification of Potential Emergency Condition Hazard identification, risk assessment, control measure and its application shall refer to OHS Plan. Emergency condition differs into emergency situation occurring accident with potential of minor injury, major injury, permanent disability, or casualty or total property damage.

Emergency Response Plan Emergency Response Team shall establish operation method and job description for the team. Emergency Response Team shall identify the needs of equipments and facilities. Project Management shall ensure all employees, staff, and management fully aware of emergency response through conducting sufficient communication and trainings regularly. Emergency Response Team shall establish schedule of inspection of emergency response readiness, schedule of training and re-assessment of emergency response readiness, reporting the readiness of emergency response. Emergency Response Team shall ensure the provision of sufficient emergency telephone numbers and address as follows: policlinic, hospital, fire department, police station, electricity department, and rescue department.
DUTIES AND RESPONSIBILITIES 9.1
Site General Manager Site General Manager shall provide his employees under MES-TSW JO Project Management with a safe working environment and promote safety consciousness among all parties concerned at all times. He shall: Be responsible for the implementation of the OHS Policy on the project. Ensure that OHS Policy is understood and carried out by all site employees. Assist the Safety Officer on site with the preparation of monthly safety meeting. Actively encourage all employees in their efforts to promote positive OHS attitude on site. Ensure that the causes of accidents are investigated and that the necessary corrective measure are planned and followed up to prevent reoccurrence. Ensure the coordination of safety procedures is strictly complied with and adhered to by the Client’s nominated subcontractors as well as MES-TSW JO own subcontractors. Formulate OHS audit schedule in consultation with MES-TSW JO Safety Manager and ensure that the audits are carried out as planned. Instruction and training is made available and provided to all employees in order to promote safe use of equipment, safe working methods and safe behavior. Provide personnel protection gear to all employees for the various work condition applicable to them. An emergency plan and casualty evacuation plan will be provided. All employees are aware that pollution of the environment must be avoided. Keep him self and the Safety Officer informed and acquainted about the local rules and regulations, updating of Company’s Head Office safety instructions and maintain liaison with client and local Government authorities. Investigate, analyze and report all incidents and accidents.
9.2
Project Safety Officer / Supervisor MES-TSW JO General Site Manager shall nominate his Safety Officer / supervisor to assist him in executing his safety programs in every respect. The Safety Officer and Safety Supervisor must be sufficiently capable to oversee the construction and safety activities at the site. He shall be responsible for the safety, health, environment, and security aspects of the project at the construction site. He reports directly to the Project Manager, advises and supports him in matters of site safety policy, security and pollution. His Job Responsibilities includes: Implementation of the approved OHS Plan. Coordinating with the MES-TSW JO Safety Manager in establishing a comprehensive accident prevention program within the respective work groups.

Co-ordinate all supporting activities such as induction courses, safety training, toolbox meeting, weekly safety meeting instruction to the contractor’s supervisors and foreman in scheduling daily toolbox meeting. Approve the use of all safety equipment in accordance with proper industrial standards and subsequence purchase and maintenance. Ensure that all workers are properly trained and briefed by their supervisors on the hazards pertaining to their respective work assignments and assure that necessary precautionary measures are taken to avoid injuries. Insure that daily tool box meeting are being conducted .and attention is given to younger, newly engaged, or illiterate workers. Ensure that the workforces are properly informed of possible work hazards and the precautionary measures to be implemented and inspected by the supervisor. Conduct, investigate and report all incidents and accidents with the accident investigation team. Inspecting the construction area daily and as often as possible and identify appropriate corrective actions on any unsafe acts & practices. Prepare daily inspection reports and submit audit reports to the Construction Manager so that the necessary corrective measures will be implemented. Approve and maintain all first aid supplies and medical equipment. Inspect and approved equipment such as scaffolding, lifting gears, power tools, welding sets etc. Implement a tagging system to identify both useable and unfit installations and equipment. Maintain and update the site Emergency and Casualty Plan with the Safety Manager. Organize regular safety drills. Keep and maintain all safety performance (man-hours) records including an incident / accident logbook. Attend to all Safety related matters. 9.3
Construction Manager The Construction Manager is responsible for the execution of the construction works as well as safety in regard to working methods and safety procedures. He has the daily supervision over all personnel, activities and operations at the work site and is responsible for security, pollution control, safe working methods, safe equipment and safe behavior of employees. In order to accomplish his task and duties in matter of health and safety, he will act in close co-operation with the Project Safety Officer. He shall: Organize work methods / safety procedures to protect workers as far as practicable against risks of accidents and injuries to health. For operation entailing an increased potential risk ( i.e. heavy lifting ) he will see to it that the necessary lifting plans, method statements and safety briefing will be made available or applied in time and he will take care that proper supervision and safety precautionary measures are taken prior to the execution of such activities. Ensure that all machines, appliances, vehicles or other equipment conform to all official safety regulations and requirements applicable to them. If there are none, it will be so designed or protected in such a manner that they can be operated safety. Provide adequate supervision to ensure that workers perform their work in a safe and healthy environment. Carry out safety audits and site inspections on the work site on a daily basis. Co-ordinate work interfaces that are done jointly by a number of persons and organize meetings aiming to instill co-operation and mutual understanding among all for the avoidance of risks ; and ensure that hazardous work be specially supervised by a qualified or competent person.

Only assign qualified and competent supervisors and workers who are suited by their age, physique, state of health and skill. Ensure the instruction and notices should as far as practicable be in the language of the workers employed, being Bahasa Indonesia and/or English. Make available at the site office, for the use of workers, copies, drawings, or summaries of national or local regulations and where appropriate, instruction and notices relating to protection against accidents and injuries to health, or post such texts up in prominent positions at suitable places. In case emergency situation, he will take the leadership of emergency and evacuation operations and adhere to the Emergency and Evacuation Plan in as much as is practicable. Ensure that regular safety drills, such as personal transfer over water and man overboard drill, man down due to fall etc., are being conducted at suitable intervals to test and enhance the medical evacuation operation. Do not permit machines, equipment or workplaces in which dangerous defects have been found to be utilized until the said defect had been remedied or corrective action taken. Adjust or coordinate construction work, based on the detailed work planning and schedules taking into account a risk analysis, as well as safety factors, particularly during adverse weather and rough sea conditions for offshore work. Take immediate corrective actions when so requested by the client. 9.4
Superintendent & Supervisory Personnel The Superintendent and Supervisor shall instruct his workers to comply with the following: Before beginning work, workers should examine their workplaces and the equipment that they are to use and should forthwith report to their superintendent / supervisor or any other competent superior in regard to any dangerous defect that they may find. Worker should make proper use of all safeguards, safety devices and other appliances furnished for their protection or the protection of others. Except in emergency, no worker, unless duly authorized, should interfere with, remove, alter, or displace any safety device or other appliance furnished for his protection or the protection of others, or interfere with any method or process adopted with the view of avoiding accidents and injuries to health. Workers should not interfere with equipment such as machines and appliances that they have not been duly authorized to operate. Workers should not sleep or rest in dangerous places such as scaffolds, garages, or in the vicinity of hot work areas; storage places for dangerous or toxic substances; at locations of running machines or vehicles and heavy equipment. Workers should acquaint themselves with and obey all safety rules and health instructions pertaining to their work. Workers should refrain from careless or reckless practices or actions that are likely to result in accidents or injuries.
9.5
Subcontractors Subcontractors shall inform MES-TSW JO names of their employees and obtain MESTSW JO Project Management approval prior to their assignment. Subcontractors shall ensure that their employees be informed of, and have the opportunity to participate in, the safety activities required by MES-TSW JO and which subcontractors are obliged to initiate in pursuant to the contract. Subcontractors shall take active steps in the implementation of site safety regulations, as well as work procedures which are identical to those required from MES-TSW JO. The project Safety Department shall regularly check the subcontractors to ensure compliance with safety requirements.

MES-TSW JO Project Management may refuse access to its work site to any of subcontractor’s personnel who, in its opinion, do not comply with MES-TSW JO working standards for safe and good workmanship, for reason of poor work attitude, lack of cooperation, lack of skill or insufficient training and experience; any or all of which that might jeopardize safety discipline or pose a safety risk to them and others. 10. SAFETY STANDARDS & ADMINISTRATIVE REQUIREMENTS 10.1 Regulations and Standards MES-TSW JO with commitment to implementing OHS Plan shall in the execution of work be fully responsible for the compliance with relevant local laws and regulation, standards, and codes of practice. 10.2 Project Management Obligations Providing well arrangement of work areas, equipments, work environments, and work methods in order to protect employees from any accidents. Ensuring machines, vehicles, and other means needed in the project complying with health and safety regulation, and those means shall be safely operated. Ensuring employees working under adequate supervision in order to supervise employees working in health and safety manner. Designating competent person as safety officer who due to his position in the organization authorized to supervise all task being carried out in order to prevent any possible hazards and accidents. Providing suitable tasks for employees considering their skill, age, sex, and physical condition. Ensuring all employees received safety orientation and informed about hazards of their tasks and the efforts of prevention, contractor shall provide signage, warning boards, and any other necessary steps. Ensuring Safety officer taking responsible for regular checking of work-areas, equipments, all means of accident prevention, work-environments, and safe work methods. 10.3 Occupational Health and Safety Organization MES-TSW JO that employs minimum 100 employees or in case of demanded by project situation shall establish Safety Committee. MES-TSW JO shall: a. Provide Safety Committee with necessary facilities for their activities. b. Consult with Safety Committee in all aspects related to occupational health and safety in the project. c. Take practical steps to set effect of recommendation issued by Safety Committee. Subcontractors joined in MES-TSW JO project shall cooperate in establishing OHS activities. 10.4 Incident Report Every dangerous incident or accident shall be reported to related party. Incident report shall cover the statistic that: a. Showing record of accident of each task, each employees, and b. Showing pictures of accident and the causes. 10.5 Occupational Health and First Aid MES-TSW JO shall ensure all employees undergo medical check-up.

a. Before starting the work for the first time (Medical checkup before starting the job emphasize to physical and individual health) b. Periodically depending on the risks of the tasks. Data of medical checkup shall be recorded and documented for reference. Emergency response and first aid plan shall be prepared involving all related local components as first aid officers, equipments, means of communication, and traffic rout. First aid in occurrence of accident or illness shall be carried out by doctor, nurse, or competent person trained in first aid. Adequate first aid equipments and first aid kits shall be provided in the premises with condition of free from dust and in an appropriate humidity, etc. First aid kit shall contain, but not limited to, adhesive bandages, gauze pads, large-triangular bandages, sold pack, iodine antiseptic, eye lotion, medicine, scissors and cotton clip, etc. Contains of first aid kits shall be checked regularly so that they remain complete, first aid kits shall not be empty. If employees designated in areas where hazards of drowning or poisonous gases considered present, means of rescue shall be located close to their work areas. Quick response shall be well prepared to evacuate ill or injury employees to the nearest hospital.
11. SAFETY INSPECTION (PRE-MOBILIZATION & PERIODICAL SAFETY CHECK) o

12. MACHINERY & EQUIPMENT 12.1 General Project Management shall provide and operate its equipments in compliance with the company safety requirements or Local Legislative Safety Pertaining (Building Operation and Works of Engineering Construction (Regulation) 1985. 12.2 Manually-Operated Chain Hoists Chain hoists shall be of larger/adequate capacity than the regular work requires. Supports of the hoist shall be strong enough to carry the load to be handled. 12.3 Heavy Equipments Heavy equipment shall be operated only by authorized person. Operators of mobile heavy equipment and trailers shall be licensed by the Local Authority. Periodic inspection shall be made to ensure that the condition is maintained at all times. Operators shall be responsible for proper care and cleanliness of the equipment assigned to them and shall make reports of defect or unusual conditions found therein. Operators shall be in good physical and mental condition and shall be capable of holding responsibility for safe operation of their equipment. The operator shall not allow any unauthorized persons to operate the equipment assigned to him nor shall he allow such persons to ride on the equipment while it is moving. Operator shall never leave equipment with the engine running. When equipment is left overnight, it shall be locked, blocked, and otherwise barricaded so that unauthorized persons cannot get in to start or move it. Equipment that is electrically operated shall be properly grounded at all times. No

equipment shall be operated where there is danger that it may contact high-voltages lines. Equipment shall not be oiled while it on operation and shall not be refueled while the engine running. Employee shall not get "ON" or "OFF" moving equipment. 12.4 Manual and Powered Tools Manual tools or power tools shall be in good condition. Tools shall be regularly cleaned, inspected and maintained. Tools shall be regularly inspected. Do not use if the tool is damaged. Use the correct tools in performing the job. Power tool shall only be operated by competent person. Portable electric tools shall completed by proper guards. Tools shall be placed in safe place and proper position to avoid tripping hazard. Power tools shall be hoisted or lowered by hand line, never by the cord to avoid any damage of it. 13. SUBMISSION OF WORK PLANS Subcontractors will be employed at least one week prior to the commencement of the project construction work. They must submit a copy of a total work plan to MES-TSW JO Project Management for review and approval. The WORK PLAN should include: List of personnel to be engaged in the Work Organogram including safety organization chart, indicating names of supervisory staff and foreman and number of personnel to be mobilized. Entire work schedule and / or work procedure Manpower and equipment mobilization charts. List of safety protective equipment to be supplied for each job profession. 14. 14.2 Individual Responsibility Responsibility of Chairman: a. Chairman shall plan, develop, review, and implement health and safety programs, procedures and standards. b. Secretary shall advise senior management about the duty care of the company and the responsibility of employees in order to achieve compliance to health and safety regulations, procedures, and standards. b. Secretary shall be aware of and keep up to date all relevant health and regulations, codes of practice, guidance notes and safety procedures etc. for reference and to provide advice and recommendations regarding their application. c. Secretary shall initiate health and safety committee meeting at least once a month. d. Secretary shall arrange workplace inspections to ensure that safe working methods are maintained with compliance to health and safety regulations, with authority to introduce alternative arrangements or improvements if considered necessary. e. Secretary shall prepare reports of accident investigation includes the near-miss accidents, unsafe conditions and acts, etc. and make rectification recommendations to prevent re-occurrence in the future. f. Secretary shall liaise with government inspectors, insurance companies, and other authorities as required. g. Secretary shall provide necessary training as first aid training, fire fighting training, fire drill and evacuation training, safe use of equipments training, etc. h. Secretary shall ensure that new employees attending induction training before commencing work on the premises. i. Secretary shall recommend Best Safety Performance – Subcontractor. j. Secretary shall recommend reprimand, suspension, fines, dismissal of health and safety regulation violators to chairman of committee. k. Secretary shall stop the operation in cases where endangering the employee, clients, subcontractor, visitor, and properties. l. Secretary shall ensure emergency response procedure is available and understood by employees, especially in obtaining medical and ambulance services in case of serious injury.

Responsibility of Non Executive Members: a. Non executive members shall attend health and safety committee meeting and contribute advices related to safety issues in the project. b. Non executive members shall educate employees through performing personal example of observance to health and safety regulation in the premises c. Non executive members shall perform safety behavior and wear PPE at all time in the premises. d. Non executive members shall participate in health and safety trainings and seminars. Responsibility of Executive Members a. Executive members shall establish and organize operation under their control to ensure that work is carried out in a safe manner complying with acceptable standards with minimum risk to all persons, property equipment and materials. b. Executive members shall be aware of and observe the requirements of the Health and Safety at Work Act., construction regulation, other relevant statutory requirements, and approved-codes of practice, guidance notes, and safety procedures appropriate to the operation under their control. c. Executive members shall carry out instructions of operation under their control with particular reference to safety procedures and keep records of each inspection. Ensure that statutory registers and records etc. are completed accurately as required by regulations and other instructions. Arrange for any remedial or improvement work to be carried out without delay. d. Executive members shall ensure employees in the premises under their control have received adequate working instructions, commencing tasks in safe area, and means of prevention of accidents and unnecessary risks are carried out. e. Executive members shall ensure subcontractors and their employees observe adequate safety procedures and statutory regulations, shall review any confusion about areas of responsibility, and shall liaise with clients or other representatives to ensure the safety of all persons affected by the works in the premises. f. Executive members shall ensure that PPE is available and being worn by employees as required by statutory regulations. g. Executive members shall encourage the observance of health and safety regulation by personal example. 14.3 Safety Committee Meeting Safety Committee Meeting shall be held weekly and attended by: Project Manager, General Site Manager, Construction Manager, Safety Manager, Safety Officer, and Subcontractors Site Manager, and Subcontractor Safety Officer. Subcontractors shall be responsible for maintaining and enhancing the safety awareness of their staff and laborers, by introducing their own daily tool-box meeting and Weekly Production Meeting which should include safety matters as the first item of the agenda. 15. GENERAL TECHNICAL REQUIREMENTS Premises and Equipments 15.1 Gate and Exit Way Gate and emergency exit shall be made in the premises. Gate, exit way, equipments, and accessories in those places shall be well maintained. Gate shall be provided with safety sign posts in order to remain the people entering premises obligated to follow safety regulation. Gate and exit way shall not be obstructed by any materials, equipments, activities, and etc.

15.2 Illumination Illumination is need of lights in work area to carry out activities effectively. In case of natural illumination is not adequate, appropriate artificial illumination shall be provided in the whole area including all passage-ways. Means of illumination shall be safely fixed and able to provide adequate illumination, and shall be maintained by competent persons to ensure no occurrence of hazards related to those means. Natural or artificial illumination shall be arranged effectively to avoid dazzling lights. 15.3 Ventilation Adequate ventilation for fresh air circulation shall be provided in closed work-area. If necessary to prevent health hazard due to dust and gases contaminated air, exhaust fan shall be provided. In case of elimination of hazardous dust and gas is technically impracticable, employees shall be equipped with appropriate Personal Protective Equipment (PPE) to prevent hazardous gas penetrating their respiratory system. 15.4 Housekeeping Waste and unused materials shall be moved to a safe particular place. All protruding nails shall be removed or bent to prevent accident as punctured by nail. Small materials and equipments shall not be scattered on the floor to prevent slipping and tripping hazards. Waste of materials, rubbish, and equipments shall not be accumulated in work area. Work areas and passage-ways that slippery due to oil spillage shall be immediately cleaned or poured by sands. Easily moveable tools and equipments shall be returned to storage after being used. 15.5 Fire Prevention and Fire Extinguisher Occupied work areas shall be equipped with adequate fire extinguishers. All supervisor and some workers shall be trained to use fire extinguishers and designated as fire marshals. Fire marshals shall attend work area during working time. Fire extinguishers shall be well maintained and checked regularly in certain period by competent persons. Means of fire fighting as water-hoses, portable fire extinguisher, and all accesses to those means shall be well maintained and free from any obstructions. Means of fire fighting shall be positioned in easily seen and reachable places. Means of fire fighting shall be provided at least one number for: a. In a room where combustible materials being stored. b. In every place where hot work being carried out. c. On every level/floor of under construction building with presence of combustible materials. Means of fire fighting made from dry chemical materials shall be provided for: a. Places with presence of flammable liquid. b. Places with presence of oil, gasoline, gases, and heating appliances. c. Places with presence of asphalts and asphalt-boiler. d. Places with presences of electric hazards. Means of fire fighting shall be checked and maintained regularly to avoid technical defects. Means of fire fighting containing chlorinated hydrocarbon or carbon tetrachloride shall not be used in narrow or closed spaces. . Heating Appliances Heating-appliances as charcoal stove shall only be used in a well ventilated area. Heatingappliances with open flame shall not be positioned near exit way. Appliances with risk of fire as gasoline stove and charcoal stove shall not be put on wooden floor nor any other floor made from combustible materials. Plastic sheet, canvas materials, etc. shall not be placed close to heating-appliances with open flame and to be placed in safe area to avoid any fire hazards.

Flammable Materials Flammable materials as sawdust, oily clothes, and unused pieces of wood shall not be accumulated on work area. Overall uniform sprayed with oil shall not be kept in a closed area. Chemical materials that reactive to water shall be kept dry. Inside building, waste of oil shall be placed in closed-cans. No smoking and no naked flame close to flammable materials. Flammable Liquid Flammable liquid shall be stored, transported, and used in a particular safe way to avoid occurrence of fire. Fuel for heating-appliances shall not be stored inside building or equipment, except stored inside fire-proof designed can or equipment. Fuel shall not be stored close to doors and exit way. Inspection and Supervision Regular inspection shall be carried out in places where fire hazards considered exist. Those include places close to heating-appliances, electricity installations, store of flammable materials, areas with hot work as welding. Personnel in charge for fire watchman shall standby during working time or even resting time. Emergency Information and Signage Information board shall be fixed in attractive and strategic places with information of: a. Nearest fire alarm. b. Telephone number of nearest Local Fire Department Fire Exit and Assembly Points Adequate fire exit shall be provided on the premises with clear signage displayed on the wall. Fire exit shall be free from any obstructions and in good housekeeping to prevent tripping and slipping hazards. Assembly points shall be arranged in a large-safe area outside the premises and marked with clear signage. Assembly points shall be free from obstructions and hazards as falling objects, fire, falling, hitting by vehicles, etc. 15.6 Protection from Falling Objects Effective means shall be fixed to protect employees from falling objects. Scaffolding materials, waste of materials, tools and equipments shall not be thrown from height as they may become a danger for other people. In case of materials and equipments are not safely moveable from height, necessary steps of precaution shall be taken as installation of fences, warning and signage, or provision of separate access in order to avoid people harmed in accident. Employees are prohibited to enter silo, warehouse, storage of fuel/coal, with exception: a. Holding authorization to b. Outlet of silo shall be closed with protection c. Employees shall use full body harnesses with lanyards anchored to firm and strong object/holder. d. Employees shall work under strict supervision. 15.7 Protection for Falling Person/Safety Barriers and Railings Safety barriers and railings to protect slab openings, wall openings, high position working areas shall be made from good and strong materials and construction. Rail, rope, or connection chain shall be fixed in the middle between upper toe board and lowest part of the upper railings. Posts in adequate numbers shall be installed to ensure stability. Toe-board shall be free from sharp edges and to be maintained properly.

Open Slab and Opening on slab Opening on the slab shall be protected using cover, barriers, and other effective means. In case those means of protection need to be removed to allow materials or personnel movement, when the movement finished those means of protection shall be reinstalled or replaced immediately. Cover of slab openings shall be safe for people and if necessary for vehicles moving on it. It shall be on the same level with the slab and not to obstruct the traffic. Openings on the wall Openings on the wall which is with potential of falling person hazards shall be protected with barriers. Small openings on the wall shall be protected with toe-board. Elevated Work-Area Work-area which is more than two meters high above the floor shall be completed with safety railings. Elevated work-area shall be completed with particular access as ladder. If necessary to avoid hazard of working in elevated place more than 2 meter, use full body harness. Prevention of Falling into Water Employees presented in an area with hazards of falling into water and drowning shall use life jacket or buoys. Means of rescuing people falling into water shall be provided in the vicinity and easily be accessed. . 15.8 Noise and Vibration Hazardous noise and vibration exposed to employees shall be reduced and considered under acceptable limit. In case of noise cannot be reduced, employees shall use means of hearing protection. 15.9 Avoiding Unauthorized Person Premises shall be fenced. Unauthorized person shall be prohibited to enter premises, unless escorted by authorized person and wearing appropriate personal protective equipment. 15.10 Toilet Toilet is sanitary facility covering bath room, WC, and wash basin to be used by employees during work-time. Toilet for ladies shall be separate from toilet for gents. Toilet shall be cleaned by house-keeper regularly. Toilet shall be free from insects and rats. Water used in the toilet shall be clean and appropriate with toilet needs. 15.11 Smoking Area Non-smoking policies in the premises shall be implemented strictly to avoid fire hazard. Smoking area for smoking person shall be provided by contractor outside or inside the premises. Smoking area shall be well ventilated and if necessary to be completed with exhaust fan. Smoking area shall be completed with sand-box for cigarette buts. Adequate means of fire fighting shall be provided in the smoking area. Smoking area shall be cleaned regularly to avoid presence of waste and combustible materials. General Construction Work 15.12 Scaffolding All scaffolds shall be erected or dismantled by competent person. All scaffoldings or temporary floors materials shall meet the requirements in strength and rigidity. Scaffoldings or working platforms shall be securely fastened to the building or structure. All runways, scaffolds, or other working surface more than two meters high above an adjoining surface shall be effectively guarded with strong railings. Such railings shall be

minimum one meter high. Suspended scaffolds shall have railings, intermediate railings and toe boards. Scaffoldings shall not be used for the storage of materials. Lifting heavy materials shall not be carried out from ladders of scaffoldings. Scaffoldings shall not be overloaded beyond their working capacity. Workers assigned to work in the scaffoldings or temporary floors shall wear safety harness with lanyards anchored to lifelines. Life-lines shall be in sufficient length to allow workers moving freely in commencing their works. Scaffoldings shall be inspected regularly. Scaffoldings and platforms erected by subcontractors shall be carefully inspected and certified before used. Supports or braces of scaffoldings shall not be removed until the works completed. Workers shall not jump to or from scaffoldings, drop heavy materials from scaffoldings, throw objects from or to scaffoldings. Workers shall not carry out any tasks during high wind with speed of 50km/hour. A safe means of access to the scaffold, either by stair or permanent ladder shall be provided. 15.13 Ladder Ladders used shall be in accordance with standards. In placing a ladder, the distance from foot of ladder to building against which it is leaning shall be approximately one-fourth of the length of the ladder. In other words, if a 12-feet ladder shall be placed, the foot shall be three feet away from the building. Ladders with weak rungs shall not be used. Ladder shall not be placed in front of door without first locking the door or designating a watchman. Stepladder shall be fully spread before used. Workers shall grip side rails while using ladder. If it is not practical to grip side rails, workers shall grip rungs securely with both hands while descending or ascending. Workers shall not work on a high ladder during wind with speed of 50 km/hour. Workers shall not carry any tools in both hands when climbing or descending. Workers shall face ladder when climbing or descending and keep eyes on rungs while climbing, there might be broken rungs. Workers shall ensure their shoes cleaned and not slippery before climbing or descending. Workers shall not lean too far to one side of ladder. Ladder shall not be loaded by more than one person at one time. Ladders shall not be placed against window glass nor leaned against unsteady backings such as loose boxes or barrels. 15.14 Materials Handling and Storage Safe load limit for frequent lifting of compact objects is 25 kg for average male worker. If worker is doubt about weight of the object, a test lift shall indicate whether or not it is within his lifting capacity. If the object is too heavy for him, he shall ask for assistance. A similar help shall also be sought when lifting bulky or unhandy load or when the object lies in an awkward position. When lifting heavy objects, worker shall make sure that his footing is secure, assume a squatting position with his back erected and then raise the object by straightening legs. This technique shall bring legs muscles into use and lessens back strain. Workers shall ensure their hands free from any oily, greasy, or slippery substance, and fully grip the object before lifting it. Workers shall keep front end high and rear end low especially at corners or in places where vision is obstructed when carrying long materials such as pipes, lumbers, ladders, etc. Before lifting and carrying heavy object from one point to another, worker shall first inspect route to be taken, make sure there is no obstruction or slippery substance spilled on the floor which might cause him tripping or slipping, and shall ensure there is sufficient clearance. from hoist way, and floor openings. Stairways, passageways, gangways, and access ways shall be free from materials, supplies and obstructions at all times. Storage yards shall be free from unusable combustible materials. Sources of ignition shall be prohibited in areas where flammable liquids are stored, handled and processed. Suitable warning and "NO SMOKING" signs shall be posted in all such areas. Rubbish or other combustible materials shall be kept from areas where flammable and combustible materials are stored, handled or processed. Spillage of flammable and combustible liquids shall be cleaned up immediately. Accumulation of combustible and flammable liquids on floors, walls, etc. is prohibited. 15.15 Housekeeping in the Workplace Materials shall be stored in an orderly manner and properly secured to prevent them from falling. Employees shall observe the standard procedures and instructions on materials handling applicable to their crafts. Materials shall be stored in such a way as not to obstruct fire exits, fire protection system, vehicular traffic, electrical boxes and stairways. Remove or bend all protruding nails. Cracks, splinters, ruts and breaks on the floor shall be reported and/or repaired as soon as they are discovered. Spillage of oil, grease or other slippery substances shall be wiped off or removed from ramps, pathways, shower rooms, floors, etc. Leftovers or cuttings on the job such as lumbers, re-bars, steel, welding butts, etc., shall not be left around where they shall pose a tripping and/or stored if still usable. Flammable liquids, oily rugs and other highly combustible materials shall be stored properly in separate and safe place. It is the responsibility of the supervisor/foreman to see to it that the working area is kept clean and tidy. Waste or trash drums/cans shall be placed in strategic places in the work area. 15.16 Painting Paint soaked rags shall not be left in lockers or in coverall pockets. They shall be spread out to dry or put into metal containers. Paint in which turpentine has been used as thinner shall not be used on hot surfaces. This might cause vapors to ignite or the fumes emitted might suffocate workers. Spray hose shall be securely fastened to a scaffold so it shall not become loose and hit or drag a man off. Adequate ventilation to prevent the accumulation of flammable vapors to hazardous levels of concentration shall be provided in all areas where painting is done or paints are mixed. No smoking, open flame, exposed heating elements, or other sources of ignition of any kind shall be permitted in area or rooms where spray painting is done. Workers shall clean their skin thoroughly of any coating materials. Do not use thinners to remove paint from hands or skin. Use only recommended creams and cleaners. 15.17 Electrical Safety Employees shall not wear rings, watches, or carry keys, lighters, or other similar metallic objects while working on electrical equipment. Employees shall wear suitable and required personal protective equipment as safety shoes, hand gloves, etc. and it shall be dried. Employees shall be instructed in the use of electrical equipment and shall be prohibited from tampering and blocking circuit breakers, using wrong fuses, or by-passing fuses. Employees shall not work on, or in proximity of unprotected live circuits unless adequate safety measures have been taken (Permit to Work) Electrical switchgear shall be labeled to identify the equipment controlled. Adequate warning signs shall be placed in all electrical equipment. Water shall not be used on electrical equipment fires. When possible, electrical equipment shall be “OFF” before attempting to extinguish an incipient stage fire.

Electrical installation and all electrical equipment shall be periodically inspected and tested by the authorized persons to assure continued satisfactory performance and to detect deficiencies. The power supply source shall be disconnected before repairs or services are performed on power tools. Electrical tools and equipment shall be adequately earthed Authorized personnel shall be allowed to perform all electrical works. Reparation of electrical equipments shall be carried out by authorized personnel. Portable electrics tools and extension leads shall be fitted with proper plugs and sockets and shall not be connected by inserting bare wires into the sockets. Electric leads shall be preferable hung in safe positions, not lying on the ground, and especially not in wet area. 15.18 Crane Operation Cranes, hoists, and etc. shall be operated and maintained in a safe manner to comply with local standards. Cranes and lifting gear shall have current test certificates. Authorized and competent persons shall act as banks men and shall use the hand signals prescribed by acceptable Standards. Banks men shall know color code for the use of slings etc. Licensed, well trained, and experienced operators shall operate cranes. Operators shall test hoist, travel, slings, brakes, etc. at the start of each shift. Operator shall not allow anyone to ride on load and hook chains during lifting work. Banks men shall arrange loads correctly balanced prior to lifting operation. Operators and banks men shall aware of obstacles within lifting radius. Banks men shall never use point of hook for lifting. Banks men shall ensure lifting area is clear and all workers in the vicinity aware of loads being lifted. Banks man acting as signaler shall stand in a secure position where he can see the load and can be seen clearly by operator. Signaler shall face the operator if possible. Signaler shall ensure signals given distinctly. 15.19 Concrete Work Employee shall avoid to ride concrete-buckets or to work under concrete-buckets while operated. Employees shall be required to wear proper PPE to prevent cement burns. Employees applying cement, sand, and water mixtures through a pneumatic hose shall wear face protection in addition to safety glasses. Employees designated to place or tie reinforcement steel at height more than 2 meters shall use proper scaffolding/ladder and safety harness. 15.20 Abrasive Wheel Employee shall be strictly prohibited to operate an abrasive wheels unless he has been trained and authorized by his employer to undertake this duty. Abrasive wheel shall not be fitted other than to the correct spindle. Abrasive wheel especially on portable equipment shall be operated carefully to avoid damage or crack. Abrasive wheel shall be guarded to avoid accident related to it. Employees shall not wear loose uniform, necklace, watch, ring, and long hair to avoid entangled hazard. Employees shall wear proper eye protection while operating abrasive wheel. 15.21 Excavation Safety barriers shall be installed around excavation to prevent entry of unauthorized person. Safety signpost warning deep excavation shall be placed around excavation where it can be easily seen. Heavy vehicle shall not pass near edge of excavation as it may cause collapse of edge and vehicle. Employee shall wear appropriate PPE for working inside excavation. Safe access to excavation shall be provided for employees prior to commencing any work in the excavation. stacked with safe distance from edge of excavation as engineer recommendation. Sufficient Illumination shall be provided for the excavation. Means of pumping water out of excavation when necessary is to be provided. 16. IONISING RADIATION o Any work involving ionizing radiation shall be executed as much as possible outside normal working hours. o A component radiation protection advisor and radiation protection supervisor must be appointed and procedures established to comply with statutory regulation and be at least as stringent as the guide lines set out in the Ionizing Radiation Safety Guide. The following points are noted : a. All activities involving ionizing radiation shall be conducted, by a minimum of two persons. b. Qualified / classified persons, authorized by the contractor. c. Personnel executing activities shall carry calibrated, personnel radiation metering devices at all times (which must be worn in the correct position). d. The work area shall be protected against unauthorized entry by adequate barriers or ropedoff by plastic tape including warning signs prior to starting work. Safe distance to be specified. e. Radiation matter evaluation and calibration intervals to be specified and recorded. Record shall be available on site. f. No radio-active material shall be left unattended unless stored in a designated, fit for purpose storage. g. Storage area shall be equipped with relevant warning signs and shall be locked to reduce the risk of unauthorized access. h. Established emergency response procedures to handle such events as damaged source containers and fires in the storage area. 17. WORKING ON OR OVER WATER 17.1
General Requirement. Where there is a risk of person falling from a structure into water, a secure barrier must be provided. This can be removed for access and movement of materials, but must be replaced back as soon as possible. Safety nets, if used, must be properly erected. If safety nets are not being used or some other forms of effective barrier are not in place, then safety harness must be used in combination with a life jacket. Warning signs must be placed near the edges of the working area. Adequate lighting must be placed near the edges of the working area. Special care must be exercised in inclement weather, particularly high winds. A life buoy / lifeline system must be to hand. A rescue vessel must be on stand-by within the immediate vicinity. Be aware of the possible source of fumes / gases that could render someone unconscious and take suitable additional precautions (e.g., respirators, increased support, and evacuation procedures).
17.2
Life jackets and Buoyancy Aids. Life-jackets should be worn for all works conducted from floating platforms or vessel (and working over water if no effective barriers are in place).

All life jackets shall be designed to turn and support a floating unconscious wearer face up. If there is a danger that the employee could be swept away by currents or may not be seen (if working at night) then the life jackets must be equipped with a whistle and a water activated light. Employee shall be given instruction on the use of life jackets, rescue equipment and all related procedures. All personnel shall work in pairs or more and be counted frequently. All equipment is to be checked on a regular basis. A man overboard or man in the water exercise should be undertaken prior to work starting. 18. ENTRY INTO INERT OR CONFINED SPACES o A confined space is an enclosed working place which has restricted access and inadequate natural ventilation. Typical confined spaces are barge manholes of water ballast tanks, sewers, shafts, funnels & cellars. o The three main hazards are suffocation (due to lack of oxygen), fire and explosion (flammable gases) and poisoning (overcome by toxic gases). o Entering a confined space will only be allowed when safety procedures are being followed in line with the “Entry into confined Spaces” safety procedure. o Employees designated in confined space shall be competent persons. o Employees designated to work in confined space shall fully understand the emergency response procedure in emergency situation. o Employee shall not work alone in confine space. There shall be one partner designated as watchman outside confined space who will act immediately in emergency situation exposing employee working inside confined space. o Confined space shall be provided with sufficient ventilation and illumination. o Confined space shall be free from fumes, oil, gases, and flammable materials. o Employees shall wear suitable personal protective equipment. o Confined space temperature shall be control by any means to obtain acceptable temperature for human body 19. ENVIRONMENT o MES-TSW JO and subcontractors shall pay due regard to the environment by preserving air, water, soil and plant life from adverse effect of the construction activities, and minimize any nuisance which may arise from such operations. o Spillage of oil or chemicals shall be avoided by containment around tanks and other practical methods, however should spillage occur it must be cleaned up immediately in view of the hazards from fire, slippery surfaces, toxic substances, etc.

o Appropriate safety precautions shall be taken during the clearing up operation. 20. WASTE DISPOSAL REGULATIONS o HSE Department will be the focal point for waste control and monitoring. They will advise the relevant departments of the approved waste disposal methods and shall keep a record of all inventories of waste and their movements. o Dense waste is all disposed materials in dense form included disposable from industrial activities. o Liquid waste is all waste in the form of liquid includes feces. o All parties generating waste must inform HSE department of their waste inventories at the end of each week. o Store, transportation, an annihilation of domestic waste shall comply with local legislation. o All hazardous materials and waste generated must be properly sorted, treated if necessary, and disposed of following the GOI regulations on B3 waste. o Before B3 waste can be removed or disposed of, the waste transporter must fill in the waste disposal application form and submit to HSE department for clearance approval. o The contractor transporters should be licensed. They should understand and be trained on waste handling safety and disposal procedures. o All non-toxic and non-hazardous waste including domestic waste must be dumped at approved local dumping grounds with permission from the local authorities. o All toxic waste generated shall not be dumped into the drainage system. o All toxic waste shall be neutralized where possible and stored in secure steel drums marked “POISON, KEEP OFF” and moved to a safe and secured location for disposal later following approved methods. 21. SAFETY IN THE OFFICE 21.1 Office Equipments Office furniture shall free from sharp edges / corners. Desk drawers shall be closed to prevent tripping over them. Plugs of all electric machines and equipments shall be removed from the sources during after office hours. Cables and wires shall be arranged in particular way to avoid presence of tripping hazard. All electric office machines shall be provided with equipment grounding before used. Liquid things or items shall not be placed on top of electrical or electronic equipments due to wetting sensitive parts thereof could load to equipment shorting, breakdown, and damaged. 21.2
Office Behavior Employee shall be prohibited running in office. Door shall not be pushed nor opened abruptly or slammed when closing. Nobody shall stand within the path of the door swing. Employees shall ensure able to see over and around when carrying a stack of materials and walking in office. Employees shall be prohibited scooting across the floor while sitting on a chair. Employees shall avoid leaning out from the chair to pick up objects on the floor since it

considered dangerous. Blade of paper cutter shall not be left in the upright positioned. Breakable objects shall not be left on the edge of desk or table where they can be easily pushed off. Materials and documents shall be filed neatly and firmly to prevent them from crumbling on the floor or fall over. The heaviest and largest pieces shall be on the bottom of the files.. 21.3
Office Housekeeping Office housekeeping shall be maintained regularly. Littering in office shall not be allowed. Any liquids spillage shall be wiped immediately. Broken glass shall be swept immediately and disposed off in a safe place. Tripping hazards such as defective floors, rugs, floor mats, electric cords shall be reported to the maintenance personnel for immediate replacements and/or repair.
22. PERSONAL PROTECTIVE EQUIPMENT 22.1
General MES – TSW JO Project Management shall ensure provision of qualified PPE and effective use PPE. Personal Protective Equipment (PPE) is means used by employee to protect a whole or parts of human body from potential hazards or work-related accident. Use of PPE is the last effort of hierarchy in controlling hazards after elimination, substitution, engineering control, and administrative control.
22.2
Face and Eye Protection Eye protection shall be worn by employee whose duties indicate the need for protection. Impact type safety goggles or glasses shall be worn during activities involving flying or falling objects or particles such as but not limited to chipping, grinding and hammering. Safety glasses or goggles are also required in areas that include but not limited to: use of compressed air to clean or blow out debris, potential dust hazards, cutting and tying of cables or wire rope, operations where there may be danger from spraying of oils and chemicals. Arc welding work requires the use of welding mask.
22.3
Head Protection Standard safety helmet shall be worn in job site. Before used, helmet shall be inspected for cracks, signs of impact or rough treatment and wear that might reduce the degree of safety originally provided. Those found damaged should be replaced. Alterations of any kind shall not be made on the helmets such as painting, etching, and cutting as these would impair the performance of the headgear.
22.4
Safety Belt/Harness and Lifelines Safety belt/harness is a device worn around portion of the body that will provide protection to wearer from a fall. Lanyard is a rope (could be manila or steel cable) suitable for supporting one person with one end fastened to a safety harness and the other end secured to a life line, structure or substantial object. Life line is cable or rope string horizontally or vertically from one substantial object to another providing a means of traveling between those two objects and maintaining full protection between those objects. All persons working on elevated structures such as steel-erections, steel-men, painters, welders, carpenters etc shall wear safety belt / harness or lifelines. Safety harness shall be securely fastened on rigid and firm braces, framings, life lines, and the like. Lifelines shall be tied to permit as little slack as possible and thus stop a man with minimum free fall. Safety harnesses and lanyards shall be worn and used in the following circumstances: a. On all stages, floats and any other type of suspended scaffolding b. On all scaffold with incomplete decking or incomplete guardrail

Foot Protection Employees shall wear foot protection appropriate for the activity and conditions. Alterations, mutilations, and defacing of safety shoes should be prohibited. Safety shoes should be kept clean.
22.6
Working Clothes Employees shall wear prescribed working clothes for a particular job. Loose clothes shall not be allowed while working around moving machines or energized lines or equipment.
22.7
Respiratory Protection Device Respiratory protection devices shall be used for activities that cannot be safely controlled by engineering methods, such as for highly toxic materials application; and when the working atmosphere is or may be oxygen deficient, if so, air supplied respirators shall be used. Supervisor shall ensure respiratory used suitable with the hazard present.
22.8
Hearing Protection Employee shall wear hearing protection as ear plugs or ear muff if exposed to noise above 85dB with exposure duration 8 (eight) hours. Hearing protection used shall be comfortable and able to reduce the noise to lower than 85dB. Hearing protection used shall not humping working activities carried out.
22.9
Hands Protection MES-TSW JO Project Management shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes. Employee shall wear appropriate hand gloves at all time when their hands are exposed to hazards that may harm their hands.
