Indications-Dilantin is indicated for the control of Contraindications/warnings/interactions - Sinus bradycardia, SA generalized tonic-clonic (grand mal) and complex partial node block, 2nd or 3rd degree heart block. Caution in pt with severe (psychomotor, temporal lobe) seizures and prevention cardiac or respiratory disease. Interacts with alcohol, gabapentin, and treatment of seizures occurring during or following omeprazole, (may increase phenytoin drug levels). Antidepressants neurosurgery. may cause additive CNS depression. M echanism of action-Rx limits seizure propagation byCommon side effects-CNS: ataxia, agitation, confusion, drowsiness, altering ion transport, may also decrease synaptic dizziness, dysarthria, dyskinesia, extra pyramidal syndrome, H/A, transmission. insomnia, weakness.EENT: diplopia, nystagmus. CV: hypotension, tachycardia. GI: gingival hyperplasia, N/V, constipation, drug-induced hepatitis. Derm: hypertrichosis, rash, pruritis. Hemat: Agranulocytosis, aplastic anemia, leukopenia, megaloblastic anemia, thrombocytopenia. MS: osteomalacia. Misc: allergic reactions including Stevens-Johnson syndrome , fever, lymphadenopathy. Lab value alterations-May increase serum alkaline phosphatase, GGT, and glucose levels. Monitor serum folate concentrations periodically during prolonged therapy. Nursing ConsiderationsAssess for phenytoin hypersensitivity syndrome, (fever, skin rash, lymphadenopathy). Vital signs. Interactions with other patient drugs, OTC, or herbal medicines-Gabapentin and omeprazole may increase drug levels.
