5. Dispatch Permit issued by the Ministry of Health, Labor and Welfare of Japan (with English translation). 6. Copy of Passport of the Company’s President/Authorized Representative. 7. Copy of Philippine Agency’s License/Certificate of Renewal issued by POEA. 8. Employer for Interview at the Office of the Labor Attache, Embassy of the Republic of the Philippines.
