THE CAUSES OF LOSSES
Loss - Physical harm to people or damage to property. The losses vary in degree from negligible to catastrophic. Incident - An undesired event that could or does result in contact with a source of energy above the threshold of the body or structure.
Unsafe Act and Conditions - These are the immediate causes of loss incident. They are below the accepted safety, health and fire standards for loss prevention. The Existence of Environmental and personal factors. These are the basic causes of losses.
Symptoms of Substandard Practices Operating equipment without authority Failure to warm of danger Failure to secure Operating at an improper speed Making safety devices inoperable Removing safety devices Using defective equipment Using equipment improperly Failure to use personal protective equipment
Improper loading Improper placement Improper lifting Improper position for task Servicing or adjusting equipment in operation Horseplay Under influence of drugs
Symptoms of Substandard Condition Inadequate guards or barriers Inadequate or improper protective equipment Defective tools, equipment or materials Congestion or restricted movement Inadequate warning system Fire and explosion hazards Poor housekeeping; disorder Hazardous environment; gases, dusts, smokes, fumes vapor or lack of oxygen
Noise exposure Radiation exposure High or low temperature exposure Inadequate or excess of illumination Inadequate ventilation
Basic Causes of Losses
The Environmental Factors are: Inadequate design standard Inadequate purchasing standard Inadequate maintenance standard Inadequate work standard Normal wear and tear Abnormal use rate or method
Basis Causes of Losses
The Personal Factors are: Lack of knowledge Lack of skill or proficiency Improper or inadequate motivation Physical problems or incapacities Mental problems or incapacities Stress
Classification of Accidents Persons falling Handling objects Striking against objects Struck by objects Equipment Hand Tools Transportation Hazardous atmospheres Caught in or between Radiation
Over exertion Contact with electric current Entry of foreign bodies in eyes either loose or embedded Other specific causes
Classification of Breakdown Falls of person from heights to the ground from another level from scaffolding from ladders into holes, trenches, etc. Into water Falls to person on same level to the ground against objects
Handling objects from lifting from pulling or pushing handling materials electric shock Striking against objects protruding nails scaffold tubes and fittings stepping or kneeling on objects
Four Steps in controlling Accident
Identify/know the Hazard a. Inspection/observation b. Investigate c. Job Hazard Analysis Correct hazard, Eliminate Substitute materials Guard the machine
Preventive Control Measures/ Process in Accident Prevention
1. 2. 3. 4. 5. 6. 7.
Provide exhaust ventilation. Isolate the worker enclosed booth. Improve housekeeping. Evaluate by products and materials. Use administrative control. Educate workers. Provide personal protective equipment.
