Bulletin Boards TITLE

“Show me the way, the truth and the right”

OBJECTIVES To provide announcements and adequate information about health for the people of Balubad I that would help to increase their knowledge and awareness about their health. It also promotes good health and promotes quality life. This project can be a guide for all the people of Balubad I and they can put announcements in the bulletin board from their barangay, health center and school. The bulletin boards will be place in every purok of the barangay. DURATION The duration of the project is 3 days from January 23 to 26, 2007 BENEFICIRIES The beneficiaries of the project are the people of Brgy. Balubad I, Silang Cavite and even from other barangays. BACKGROUND & JUSTIFICATION During our first ocular survey in the barangay, we noticed that they had a problem about information in health. They don’t have an information area and the health center is newly established in their barangay. The bulletin boards will be place in 4 ideal spots in every purok of the barangay. The bulletin boards can be posted with pamphlets, charts and other materials. The bulletin boards can also increase the awareness of the people about health and to increase their knowledge for them to be able a self-reliant person. The bulletin board will also serve as an information area for the barangay. PROPONENT The proponents of this project are students from FEU, Institute of Nursing. The Groups 127 and 128 of BSN 732, specifically the students of Health Care Provider Committee. COMPONENT The component of this projects are the following: 1. ¼ Plywood 2. Coco Lumber 3. Nails 4. Vital Signs Monitoring Sheet TITLE

“BOOKLET-IN MO, IPAPAALALA KO”

OBJECTIVES The client would have their own vital signs monitoring booklet, for them to be able to have a record of their vital signs and they would be able to check if their vital signs increase or decrease. DURATION The duration of the project will be 3 days from January 16, 2007 to January 18, 2007. BENEFICIARIES The beneficiaries of the project are the citizen of Brgy. Balubad I, Silang, Cavite. BACKGROUND & JUSTIFICATION Most of the community people went to Brgy. Health Center to know their BP measurement and when they are asked what their last BP, most of them forgot their last BP measurement. Vital signs monitoring booklet will help them to check and know if their vital signs increase or decrease. Some of the community people want to know if their weight increase or decrease. Vital Signs monitoring sheet includes the measurement of weight, it may help to community people to recall their last weight measurement and they may also know if their weight are increase or decrease. PROPONENT The proponent of this project is the committee of Health Care Provider that composed of five students from Far Eastern University. Nameplates TITLE “Oh! Pangalan ko, banggitin mo baby!” OBJECTIVES The objectives of this project would include having nameplates worn at all times by every barangay health care provider or health care worker during duty for familiarization of each persons belonging to the barangay of Balubad, Silang, Cavite. These would also include easy rapport building of the barangay and the health care providers. DURATION The duration of the project would be a week in preparation of having their nameplates. BENEFICIARIES Health care team and Core group of barangay Balubad BACKGROUND AND JUSTIFICATION This project would ensure familiarization of the barangay people to the members of each barangay core group officials and this would establish a good communication between the barangay consultants and the core group members for better health assistance given to the community people. Operation Timbang TITLE

“GALUGAD, BALUBAD” OBJECTIVES This will be able to provide information and data for the health center that is in need to have a feeding program and other kinds of programs. Through this project we can identify who are malnourished children from ages 0-6 years old. The committee will have house to house in each four purok of the barangay. This project can also help the program of the barangay for children and to educate their community. DURATION The expected duration of the project will be 8 days. BENEFICIARIES The beneficiaries of the project are the people from Brgy. Balubad I, Silang Cavite especially children ages 0-6 years old and their mothers. BACKGOUND & JUSTIFICATION When we are in the health center, we recognized that the mother always asking multivitamins for their children. Also the midwife of Balubad II conducting a vaccination during that time suggests having an operation timbang in Brgy. Balubad I so that they could have statistics of malnourished children in the said barangay. PROPONENT The proponents of this project are students from FEU, Institute of Nursing. Pamphlet Stand TITLE “C’mon and get me, get me, get me” OBJECTIVES To provide information about health to the people of Balubed I that would help in improving the promotion of health and preserving life through leaflet and pamphlet in a patient-friendly stand. DURATION The expected duration of the project is two days only BENEFICIARIES The people who will benefit from this project is the community people specifically those who are visiting the health center in Balubad I BACKGROUND & JUSTIFICATION Based on the exposures that we had, we noticed that majority of the community people are not aware of the sign and symptoms of diseases, the prevention and cure that makes alarm and would lead to anxiety. A pamphlet or leaflet placed on a patient-friendly stand in an accessible area would help in encouraging the people to get one that would give them adequate information and would answer their questions and will prevent anxiety. Beautification of Health Center TITLE “Nasa Tao ang Gawa, Nasa Kahon ang Biyaya” OBJECTIVES The objective of this project would include the beautification of the barangay health center by providing medicine box, linoleum, curtains, doormat, shoe rack, medicines, donation box and also the painting of the barangay health center. The medicines will be arranged by storage so that it is easy to find and each will have a label. DURATION The duration of the project would be 7 days. Two days will be spent for the canvassing of the materials and the remaining days would be the preparation and beautification of the health center. BENEFICIARIES The beneficiaries for this project will be the citizens of the Barangay Balubad I from Pooc I to Pooc IV and also the neighboring barangays. BACKGROUND & JUSTIFICATION Before, Barangay Balubad I doesn’t have a health center, then the first group of students from Far Eastern University proposed a project of locating a site for a health center, then they decided to make the Barangay Out-Post as a health center in Barangay Balubad I. Then, the second group of students from Far Eastern University also came and they proposed a project of buying things / materials that will be needed in the health center. They bought a cabinet file where the files will be kept, they also provide pamphlets and posters from DOH and they also provide medicines. And then, our group came and we observed that all the medicines were kept in one storage cabinet without a divider, so we planned to have it separately and with division so that the medicines are easy to find and easily be monitored. We also observed that their barangay health center doesn’t look like a health center because its color doesn’t catch the attention of the people so we decided to paint it and to have plants around the center, we would like also to buy doormats, curtains and linoleum so that it will become more attractive. PROPONENT The proponent of this project will be the Health Care Provider Committee of BSN 732 Group 127 and 128 of Far Eastern University.
