Take good care of your kidney By Dr. Benita Padilla, President, Philippine Society of Nephrology

A

RE you among those with hypertension who think they do not have to take medications to lower their blood pressure? “I feel well even if my blood pressure is 160/100 so I’m ok.” Sorry, you risk developing heart attacks, strokes, and kidney failure. You might even die within the next 5 to 10 years. But there is hope.

In 2007, more than 7000 people started dialysis in the Philippines. Among them, 42% developed kidney failure because of diabetes and 22% had kidney failure due to hypertension. These two diseases are the top causes of chronic kidney disease (CKD) in the world. CKD is the medical term describing permanent damage to the kidneys, which in its most advanced stage requires dialysis or transplantation to prolong life. It is a major problem—there are now more than 500 million people who have CKD in the world—and most of them don’t know it. Chronic kidney disease develops slowly and you often feel well until it is too late. This is why the International Society of Nephrology, and its member societies such as the Philippine Society of Nephrology, initiated the celebration of World Kidney Day every second Thursday of March.

How can I take care of my kidneys ?

The good news—even if you have hypertension or diabetes, you don’t have to end up with CKD. The bad news—there is no quick fix. Your blood pressure and blood sugar have to be normalized with proper diet, regular exer-

cise and regular medications. In short, be serious about “lifestyle change.” Give up smoking and you lower your risk for heart disease, stroke, lung disease and kidney disease without spending a centavo. In fact, you would have saved some money for medicines. Eat more vegetables and less hamburgers—and maybe you won’t need that cholesterol-lowering drug anymore. Walk more and ride taxis less—and maybe you won’t need that insulin shot anymore. Better control of hypertension and diabetes translates to better care of the kidneys.

Keep the pressure down

Keep your blood pressure low if you don’t want your kidneys to go. Believing that “my body is already used to a high blood pressure” is never correct. Your blood pressure goal is something you should know. For diabetics and those already with kidney disease, blood pressure should be kept below 130/80. For most others, blood pressure should not be maintained below 140/90. Ask your doctor whether you are achieving the correct blood pressure. Medications to control hypertension can cost as little as P25 a day. Dialysis costs about P30,000 a month. That means that one would spend the same amount of money paying for one’s medications for 10 years as paying for dialysis in 3 months!

Have your kidneys checked

The first simple steps to find whether your kidneys are happy are a routine urinalysis and a serum creatinine. These will set you back between P150 to P400 depending on where you go. Then your doctor can decide if you need other tests. Compare that with P30,000 a month for dialysis. Prevention makes sense, and saves cents!
