I hereby warrant and covenant that I have the legal capacity to enter into this contract and will be bound by its terms. I have read and understand SLM Studio’s Registration Agreement on the back of this form describing its policies on Tuition Payments and other related fees throughout the season. I voluntarily enter into this legally binding contract and agree to uphold the terms of this Agreement for the 2010-2011 season. I also understand that all deposits are non-refundable. In the event of injury or accident, I hereby authorize my child to receive any necessary emergency medical attention while present at SLM Studio/SLM Studio II, if parents or emergency contacts cannot be reached by telephone. I also hereby release, indemnify and hold harmless SLM Studio and its faculty members, representatives, and principals from all liability for claims resulting from injury or damages occurring while my child is present on the premises of SLM Studio or any claims or actions for injury or damages resulting from or related to this Agreement.

Slm studio Registration agreement Fall Tuition & Fees: Tuition for the regular school season is based on the number of hours per week in class. Tuition can be paid in monthly, semester, or yearly payments. Monthly tuition payments are based on 16 lessons throughout the semester and divided into payments. Monthly payments remain the same regardless of the number of classes in a month. Monthly payments are due by the 1st of each month and semester payments are due by the 1st class of each semester. Payments received after the 10th of each month will be charged a $10.00 late fee. Please put your child’s name and class time on all checks. All checks need to be made payable to SLM Studio. ______________ Initial

Withdrawal/Refund Policy: There is no refund policy once lessons have begun. A student should not register for study if he/she believes that schedule changes will interfere with regular weekly attendance. Once registered, the student has contracted the teacher’s time for the duration of the semester and is responsible for the full tuition for the entire semester. Inability to find time to practice, changing one’s mind, or unexpected changes in the student’s schedule are not acceptable reasons for a withdrawal request. Subject to the sole discretion of SLM Studio and its faculty members and representatives, students may withdraw and receive refunds prorated for the number of lessons received only in the event of prolonged illness or moving away from the area. If a student needs to with draw, a withdrawal form must be filled out in the office with a two week notice prior to withdrawal. Billing to the account will continue unless this form has been completed. ______________ Initial

Absentee Policy: Regular attendance is essential for your child’s progress in music. Unfortunately, SLM Studio cannot refund accounts for classes missed, regardless of illness or other reasons. Make-up lessons are at the discretion of the student’s teacher. In the event it becomes necessary for the studio to cancel a lesson, a make up lesson will be rescheduled. If a student misses that appointed make up lesson, it will not be rescheduled. Lessons do not carry over from one semester to the next semester. ______________Initial

Tardiness & Missing Books: Students who are late for a scheduled lesson or forget their lesson books cannot be guaranteed their full lesson length. _____________ Initial

Fall Registration Fee:

A Registration Fee is due upon registration of a student along with the registration agreement to reserve a position in class. The fee is $35.00 for the 1st child, $25.00 for the 2nd child, and $20.00 for the 3rd child. (Non-refundable).Fall Registration fee will carry over for students enrolled in both Fall 2009 & Spring 2010 classes. _____________Initial

SLM Studio Tuition & Fees Overdue Policy: • Any accounts not paid by the 10th of each month will be assessed a $10 late fee. • Students with accounts past due longer than 90 days will not be allowed to participate in class •

unless a payment agreement has been made with the office. All returned checks will be charged $35 NSF Fee.

The person who has affixed their signature to the registration form and has agreed to be bound by the terms of this contract, including payment for all tuition and fees, agrees that, in the event of default, he/she will be responsible for all costs of collection for past due accounts, including court costs and reasonable attorneys fees. _____________Initial Please mail this form along with registration fee to:
