Advice for EEA Applications : Czech Republic, Slovakia, Czechoslovakia (medically qualified) This advice sheet is aimed to assist your application for registration. It is for the use of nationals of Member States of the European Economic Area or those entitled to be treated as such.1 In addition to this advice sheet please read the guidance notes attached to the application form before completing your application. What the GDC will require: •

a completed application form;



a certified copy of your passport (see below);



a certified copy of your medical diploma/degree (see below);



a certificate of good standing;



a certificate which confirms that your qualification meets the minimum standards agreed under Article 1 of the second Dental Directive 78\687\EEC and



the fee.

Where your documents are in a language other than English, they must be accompanied by translations into English by a qualified translator. The translator must provide his/her name, address and certify in English, that the translations accurately reflect the texts in the original language. Please note that your full name must correspond on all documents. If your name has changed you must include documentary evidence (e.g. a certified copy of a marriage certificate). Application form -

Registration Details: this is your personal declaration. The details that you complete will be those details that will be entered on the Dentists Register if your application is successful.

Health and Character Declaration: another professional must provide the character reference, such as a doctor, (but not the doctor who completes the health certificate on the form) a dentist or a lawyer who has known you for over one year and who is not a close member of your family. A registered medical practitioner who is not a member of your family must complete the health certificate. Both certificates are only valid for three months from the date

they are signed. If they expire before an application is complete, new certificates will be required. -

Health and character self-declaration: this declaration should be completed and signed by the applicant.

Please read guidance notes attached to the application form for further information. Certified copies of documents -

The GDC will accept photocopies of your passport and your degree/diploma certified by someone entitled to practise law in their country or by an authorised officer of an embassy or by a notary (this does not include the police forces or civic officials).

The certified copy must confirm in writing, in English, that they have inspected the original document and that the copy is a true copy of the original document.

The certified copy must contain the name, profession, signature and address of the person certifying.

-

The person certifying must not be the applicant himself/herself, his/her spouse or a member of his/her family.

Certificate of good standing -

A certificate of good standing must be issued by the competent dental authority in the country where you last worked as a dentist. It should state that you are legally entitled to practice dentistry and that you have not been suspended, disqualified or prohibited from working as a dentist.

If you were last working in the Czech Republic, this certificate should be issued by The Czech Dental Chamber, Jecna 3, 120 00 Prague.

If you were last working in Slovakia, this certificate should be issued by Slovak Chamber of Dentists, Fibichova 14, 821 05 Bratislava.

This certificate is only valid for three months from the date it is signed and if it expires before the application is complete, a new certificate will be required.

The original certificate must be submitted since we do not accept copies of this document.

If this certificate is not in English it must be accompanied by an authorised translation.

Certificate of EEA compliance. This section applies to you if you are an EEA national and hold a medical diploma which was awarded in the Czech Republic, Slovakia or the former Czechoslovakia, and the training which led to the award of that diploma began before 1 May 2004.

Your qualification will be recognised for registration if you provide with your application form, a certificate from the dental regulator of the Czech Republic or Slovakia which states that you have: a) ‘effectively, lawfully and principally been engaged, in Slovakia or the Czech Republic, in the activities specified in Article 5 of the Dental Training Directive for at least three consecutive years during the five years preceding the date of issue of this certificate’

Or; that you have: b) ‘successfully completed three years of study which are equivalent to the training referred to in Article 1 of the Dental Training Directive’. And in addition to either (a) or (b) above, the certificate must also state that you: ‘are authorised to practise the activities specified in Article 5 of the Dental Directive under the same conditions as holders of the scheduled European diploma in relation to Slovakia or the Czech Republic’. -

If you qualified in the Czech Republic, this certificate should be issued from Ministry of Health, Palackeho nam. 4, 128 01 Praha.

If you qualified in Slovakia, this certificate should be issued from Ministry of Health, Health Care Directorate, Department of Education, Networks and Licensing, Limbova 2, 837 52 Bratislava.

The original certificate must be provided to the GDC.

If this certificate is not in English it must be accompanied by an authorised translation.

If you are unable to provide the EEA Compliance Certificate, your application will be assessed by a panel of independent dental experts for eligibility for registration in the UK. Frequently Asked Questions Q: Do I have to apply in person? A: No. The completed application can be posted to this office. Q: How long will my application take? A: If your documents are in order, we will aim to consider your application within 20 working days. This process will be delayed if you do not provide all of the information that we require. The Council reserves the right to request a personal interview with an applicant if the Registrar is not satisfied with an application. Q: Can I work in the meantime? A: No. It is illegal to practise dentistry in the UK before a registration number is issued. Q: How will I know when my registration is complete? A: We will send an Original Registration Certificate to your address. You may also search the Dentists Register on our website www.gdc-uk.org to check if your name is included in the Register.
