Adapted Physical Education Assessment Results Report Name: Student Program: Skill Builders Date of Birth: June 26th 2015 Age: 3 years Test Date: September 11th, 2018 Examiner: Owen Pearson Student is a 3-year-old bundle of energy who enjoys lifting heavy objects and moving things around. He also enjoys building things to knock them down and communicating using short words. Student has been diagnosed with developmental delay. Student enjoys physical education but has difficulties when given complex directions and loses attention easily. He enjoys making loud noises with balloons and listens well once we stop for a minute. Although Student is young still, he is a very caring individual who enjoys trucks, Star Wars, and Peppa Pig. He also likes to be rewarded by having time to explore the other equipment in the gymnasium. Student was assessed using the Test of Gross Motor Development -2 (TGMD-2) on Tuesday, September 11th, 2018 between the times of 6 and 7pm. The outcomes of his assessment are as follows – Your assessment results should be detailed and can following the criteria used in the assessment/test. Assessment tool: TGMD-2 TGMD 2 Loco-motor Subtest a. Run- Student demonstrated none of the criteria but seems like he will be able to run soon.

b. Gallop- Student demonstrated a lead foot and rhythmic 4/8 movement. He did not leave the floor with both feet or have proper arms. c. Hop- Student was not able to hop on one foot but could hop and then gain balance afterwards.

d. Leap- Student mostly did a series of jumps for this skill.

e. Horizontal jump- Student was very straight while jumping and kept hands up for the most part.

Slide- Student was not able to slide. He would turn towards me and still gallop with his lower half. Subtest Total Score:

TGMD-2 Object Control Subtest a. Striking a stationary ball- Student did not take part in this assessment. b. Stationary dribble- Student attempted the skill and could not keep the balloon in one area.

Suggested Recommendations: Based upon the results of the assessment (total score) (Ages/Stages Checklist/TGMD-2/Brockport Fitness Test) it is suggested that Marc Sanderson work on the following activities across these domains:    

Cognitive: Marc Sanderson will be able to recognize various colors. Affective: Marc Sanderson will share equipment at each lab session. Psychomotor: Marc Sanderson will demonstrate 2 locomotor skills at control level. Fitness: Marc Sanderson will be able to participate without stopping for 3 minutes.

The areas that are identified in the Pre-assessment (raw data) and assessment report will be used to develop the Block Plan, The IEP, and implemented on each subsequent Lesson Plan during the semester.
