Part I : เลือกคำาตอบท่ีถูกต้อง 1. ผู้ป่วยมาท่ีห้องฉุกเฉินด้วยอาการชักมานานประมาณ 10 นาที ยังมีอาการชัก ตาค้าง กัดฟั น ถามตอบไม่รู้สึกตัว ข้อใดควรปฏิบัตท ิ ่ีห้องฉุกเฉิน 1. เตรียมยา Valium 2. เตรียมอุปกรณ์ใส่ tube 3. จับตะแคงหน้า 4. ผูกมัดผู้ป่วยติดแน่นกับเตียง 5. ให้ O2 support 6. เปิ ดเส้นให้ IV fluid 7. ให้กัดผ้า gauze ป้ องกันการกัดลิน ้ 8. พลเมืองดีโทรแจ้งมายัง รพ ส่ิงท่ีทา่ นควรปฏิบัติเม่ ือไปถึงจุดเกิดเหตุคือ 1. ประเมิน conscious เป็ นลำาดับแรก 2. ใส่ Hard collar protect c-spine 3. เปิ ด iv fluid 5%DN/2 4. ใส่ OG tube 5. Check deformity and bleeding

3. ผู้ป่วย U/D Liver cirrhosis ญาตินำาส่ง รพ ด้วยอาการเพ้อ พูดจาไม่รู้เร่ ือง

PE : E4V4M5 pupil 2 mm RTLBE Flapping tremor, present sign of chronic liver stigmata จงให้การวินิจฉัย : Hepatic encephalopathy

4. ผู้ป่วย U/D DM c HT c Dyslipidemia มา รพ ด้วยแขนขาอ่อนแรงข้าง

ซ้าย พูดจาไม่รู้เร่ ือง ญาติให้ประวัติ มาถึงห้องฉุกเฉินผูป ้ ่ วยถามตอบรู้เร่ ือง การตรวจร่างกายปกติ DTx normal จงให้การวินิจฉัยท่ีเป็ นไปได้มากท่ีสุด : TIA

5. ผู้ป่วยมาพบแพทย์ท่ี รพ ด้วยอาการถ่ายเหลวเป็ นน้ำาหลายครัง้ คล่ ืนไส้อาเจียน เหน่ ือยเพลีย กระสับกระส่าย ถามตอบไม่รู้เร่ ือง PE : V/S BT 37 PR 120 RR 26 BP 60/40 GCS E4V3M5 pupil 2 mmRTLBE Others : unremarkable ส่ิงท่ีท่านควรปฏิบัตค ิ ือ 1. เปิ ด iv Fluid โหลด 5%DN/2 2. O2 canula 3. เตรียมอุปกรณ์ใส่ Tube 4. เตรียม Valium 5. NPO 6. DTx stat 6. ท่านได้รับแจ้งเหตุว่ามีผป ู้ ่ วยหมดสติ ไม่รู้สึกตัวท่ีหมู่บ้านแห่งหน่ึง พอท่านไปถึง

ท่านจะมีวิธใี ดในการประเมินว่าผูป ้ ่ วยเสียชีวิต(Brain death) แล้วหรือยัง 1. คลำาชีพจร 2. ดู Pupil 3. ดู corneal reflex 4. ดู doll eye sign 5. ดูอุณหภูมิร่างกาย 6. ดู muscle rigidity 7. ดู Gag reflex

Part II : assess The Glasgow coma scale Situation : You are the patient and the doctor is examining you 8. - You will have your eyes open normally - You will speak normally and answer questions appropriately. - You will be able to follow commands like, “show me two fingers.” GCS : E4V5M6 9. - You will have your eyes open normally - You will speak normally, but you are confused. You don’t know exactly what happened to you, and you give the wrong month when asked. - You will be slow to follow commands like, “show me two fingers,” but you will be able to do so. GCS : E4V4M6

10. - Your eyes will be closed, even if someone pinches you. - You will make no sound, even if someone pinches you. - You will not follow commands. You will not move, even if someone pinches you. GCS : E1V1M1 11. - Your eyes will be closed, but you will open them briefly if anyone calls loudly. - You will not answer questions. If anyone pinches you, you will groan or yell, but not say any recognizable words - You will not follow commands, but if someone pinches you, you will quickly push his or her hand away GCS : E3V2M5

12. - Your eyes will be open. - Your speech won’t make much sense. In answer to a question like, “what month is this?” you will say something like, “he, umm, he . . . My jacket. I don't . . . - You will not follow commands, but you can movement by yourself GCS : E4V3M5

13. - Your eyes will be closed, but you will open them if anyone pinches you. - You will not say any recognizable words. If someone asks you a question, look confused and only say, "ummm , ahhh" without opening your mouth. - You will not follow commands. If someone pinches you, pull away GCS : E2V2M4
