Notes 2-10, Mike Tedesca

Video List of People/ Go's Identified: Lobby: Lt. Vinne Halloran, Ladder 8 55 Engine Greg Atlas 10 Engine Bill McGovern Batt 2 Bill McKin Squad 18 Rescue 1, David Weiss (w/ Terry Hatton) Chief Prunti, 2nd Batt Chief Burns 3 Engine Jay Jonas Ladder 6 Michael Warchola 5 Truck Al Turi 1 Engine Grezlak, Batt 4A? Steve King, Safety Chief King's Ass't Bobby Crawford After Collapse (on West St): Rescue 3 rig McNally, Depl4th O'Flaherty (comes from Marriott) Notes: Pfeifer: "Get an engine to team up" (SOP) sounds like "no higher than 70" *sounds like Burns goes to Ch 7 before going to Lobby 2; Pfeifer makes first request to get car repeated (but tells not to turn on); later asks for it again, and says to turn on; Pfeifer's communication attempts sound like transcript from repeater; when communicating w/ Palmer (Picciotto says Palmer is Bravo 2?) Turi and King: in charge of safety; should be doing something to assess situation; are just mulling around in lobby; at one point Turi says something about view of FDR, might be assessing angle, situation from there, but goes on to just point out that West St side is not safe to exit;

After first collapse, no maydays: a couple of evac orders by Pfeifer, but they leave scene and don't do much to notify people in building; should have repeated maydays; later on street, asks O'Flaherty if knows about people/men in Tower 1 (event though that's where Pfeifer was and O'Flaherty had just escaped from Marriott, was reporting people trapped there); No collapse zone established (Downey) Should talk to 1 survivor 13 Truck (maybe we've done this) McKinsey Report • • • • • • • • •

Does not bring up lack of collapse zone; no one should have been allowed back/in area except for specific rescue, response; not just generally allowed; Even outside command center was too close; should have been a couple of blocks away (safety, perspective) Accountability: still not good: people still rush to scene w/o being assigned; people get reported on riding list as going overtime who aren't there (changing tours) P. 33 Says Ganci, Feehan and "other members" of incident command went back; not true, no one else; ESU has basically no coordination/ relationship with Spec Op units (though almost always overlap in assignments; Rather than getting extra Hazmat units, have train 21 ladder co's as backup hazmat units; Larry Stack, had been fighting for Battalion Car Radios that would record everything on every channel; only in a few cars; P. 73: remarks again that there was staging, people just didn't follow procedure of reporting. Should have emphasized 1) lack of maydays; and 2) no establishment of collapse zone (why wasn't the construction of the building etc studied after '93; should have had details like this listed on sids card).
