Does not bring up lack of collapse zone; no one should have been allowed back/in area except for specific rescue, response; not just generally allowed; Even outside command center was too close; should have been a couple of blocks away (safety, perspective) Accountability: still not good: people still rush to scene w/o being assigned; people get reported on riding list as going overtime who aren't there (changing tours) P. 33 Says Ganci, Feehan and "other members" of incident command went back; not true, no one else; ESU has basically no coordination/ relationship with Spec Op units (though almost always overlap in assignments; Rather than getting extra Hazmat units, have train 21 ladder co's as backup hazmat units; Larry Stack, had been fighting for Battalion Car Radios that would record everything on every channel; only in a few cars; P. 73: remarks again that there was staging, people just didn't follow procedure of reporting.
