Applicant (Referral) Documentary Requirements • For applicants WITH an existing credit card (must be a Principal cardholder of another bank), submit a photocopy of one (1) valid ID with photo and signature [e.g., Company ID, Driver’s License, Passport, Professional Regulation Commission (PRC) ID, SSS, BIR ID, Postal ID, etc.] and latest credit card statement of account. • For applicants WITHOUT an existing credit card, submit a photocopy of one (1) valid ID with photo and signature [e.g., Company ID, Driver’s License, Passport, Professional Regulation Commission (PRC) ID, SSS, BIR ID, Postal ID, etc.] and any ONE of the following: (1) Latest ITR/Certificate of Compensation Payment/Tax Withheld (BIR Form 2316); (2) Certificate of Employment; or (3) Latest three (3) months payslips. The EastWest Member-Get-Member (“MGM”) Promo is open to all EastWest Principal Cardholders in good credit standing*, herein referred to as “Referrer”. Employees of EastWest and Corporate Cardholders are not qualified to join the Promo.

2. Promo period is from November 5, 2018 to March 31, 2019. 3. EastWest Principal Cardholders can join the MGM Promo by asking their relatives and friends, herein referred to as “Referral”, to apply for an EastWest Mastercard credit card. 4. The complete information of the Referrer must be clearly indicated on the MGM Application Form. Blank, incomplete, invalid and unreadable Referrer information will not be counted in favor of the Referrer despite approval of the application of the Referral. Referrers should provide the following information for proper tagging of Referrals: a) Complete name (as it appears on the EastWest credit card) b) 1st 6 digits and last 4 digits of their EastWest credit card number c) E-mail address 5. The Referrer must ensure that his/her Referral meets all of the following criteria: a) Must not have an existing principal EastWest credit card or a cancelled EastWest credit card within the promo period. 6. To qualify for the Promo, the Referral’s application form must be: a) received within the promo period; b) the prescribed MGM application form, completely filled out and signed; and c) submitted with complete documentary requirements via e-mail to: [email protected] 7.

9. The Referrer must be in good credit standing* throughout the duration of the promo period and upon crediting of the Referrer Incentive. Otherwise, the Referrer Incentive will be forfeited in favor of the Bank. 10. In the event that the Referral is referred by two (2) or more Referrers, the Referrer Incentive shall be awarded to the Referrer whose Referral application was approved first. The Bank shall strictly enforce a one (1) Referrer Incentive and Referral Welcome Gift policy per valid approved Referral. 11. Referrers will be notified of their Referral’s application status through their e-mail address on record. Referrer Incentive will be posted on the Referrer’s credit card Statement of Account (SOA) on the next billing cycle from the card approval date of their Referral. 12. Only signed MGM Application Forms with complete documentary requirements will be processed. Incomplete Referral applications will be cancelled. Request for reprocessing of cancelled/declined applications will not be accommodated. Instead, the Referrer shall be required to re-submit the duly accomplished MGM Application Form with documentary requirements. All submitted MGM Application Forms and documentary requirements shall belong to EastWest and shall not be returned. 13. Disputes in respect of the Referrer’s eligibility, coverage of the promo dates, fulfillment, etc., shall be resolved by EastWest at its sole discretion with prior approval from DTI.

B. Referral Incentive 1. Newly approved Referrals under the EastWest Member-GetMember (“MGM”) Program shall be eligible to redeem FREE SM Gift Pass/es provided that the approved Referral is “new-to-bank”. New-to-bank means the approved Referral does not have any existing EastWest principal credit card or cancelled EastWest principal credit card within the promo period. 2. Promo period is from November 1, 2018 to March 31, 2019. 3. The value of the FREE SM Gift Pass/es to be given away shall depend on the Mastercard credit card type issued by EastWest under the MGM Promo subject to the corresponding spend requirement as stated in the table below: Mastercard Credit Card Type Classic Mastercard

4. The approved Referral will qualify for the FREE SM Gift Pass/es once he/she reaches the minimum spend requirement within ninety (90) days from the date of card approval, which shall be indicated in the Welcome Letter to be inserted in the welcome pack of the newly approved EastWest Mastercard credit card. 5. Upon reaching the minimum spend requirement, a Complimentary Gift Letter shall be sent to the qualified Referral’s billing address on record within forty-five (45) days. Redemption of the FREE SM Gift Pass/es is until sixty (60) days from the date of the Complimentary Gift Letter. 6. Only straight/retail transactions, merchant installment transactions (where the principal amount is considered), online purchases, mail/telephone order (MOTO) transactions and Quick Bills/auto-charge transactions are eligible under the MGM Promo. Cash advance availments (whether via POS terminal or manual voice authorizations), Insta-Cash, Balance Transfer, Convert-to-Installment transactions, fees and charges are not considered qualified spend for the Promo. 7. To redeem the FREE SM Gift Pass/es, the Referral must: a) Proceed to the SM Store Customer Service Counter at any SM Store nationwide; b) Present his/her new EastWest Mastercard credit card and valid ID; and c) Surrender the original Complimentary Gift Letter. Note: The credit card account must be in good credit standing* at the time of redemption.

8. Upon redemption, the EastWest Mastercard credit card will be validated against the Complimentary Gift Letter presented. The credit card details on the letter must match with the EastWest Mastercard credit card presented. 9. A qualified Referral can only redeem once. The Complimentary Gift Letter can only be used once. 10. Qualified Referral must be responsible for his/her Complimentary Gift Letter. Lost or destroyed Complimentary Gift Letters shall not be replaced. Tampered Complimentary Gift Letters shall not be accepted. 11. In case a Referral is proven by EastWest to have made an invalid transaction, submitted invalid redemption documents not within the terms of item numbers 6 and 7 of the Promo Terms and Conditions and/or has made multiple redemptions, EastWest shall have the right to charge the value of the SM Gift Pass/es redeemed under this Promo to the Referral’s EastWest Mastercard credit card even if the charging results to a negative balance on his/her EastWest Mastercard credit card. 12. This Promo may not be used in conjunction with other EastWest acquisition/activation promos. In case the approved Referral qualifies for more than one promo, he/she can only avail of one. Multiple or invalid promo availment may result to charging the full cost of the reward to the approved Referral’s account, at EastWest’s discretion. 13. The approved Referral understands and agrees that, as a condition for receiving the SM Gift Pass/es under this Program, he/she shall retain his/her EastWest Mastercard credit card until its next anniversary date. In the event that the approved Referral cancels his/her EastWest Mastercard credit card prior to its next anniversary date, he/she shall be charged through his/her EastWest Mastercard credit card an amount not exceeding the value of the SM Gift Pass/es redeemed. 14. The issuance and use of EastWest credit card are subject to the Bank’s credit policies and the Terms and Conditions Governing the Issuance and Use of EastWest Credit Cards. Card type issuance is subject to the existing Marketing program guidelines, Credit policies and procedures of EastWest. 15. Disputes in respect of the approved Referral’s eligibility, coverage of the promo dates, fulfillment, etc., shall be resolved by EastWest at its sole discretion with prior approval from DTI. *Cardholders in good credit standing are those whose accounts are not delinquent, not under investigation due to suspected fraudulent activities, those whose EastWest credit cards are not reported lost or stolen and those who have not, otherwise, violated any of the Terms and Conditions Governing the Issuance and Use of EastWest Credit Cards.
