MARINE CORPS SEMPER FI PHILIPPINES, INCORPORATED (An NGO of 51st Marine Reserve Battalion, Marine Forces Reserve Central, Philippine Marine Corps)

A non-stock and non-profit association under the laws of the Republic of the Philippines, an association duly registered with the Securities and Exchange Commission, with SEC Certificate of Registration No CN 200928202. ; PRIMARY PURPOSE: * To contribute to the development of the country and the general welfare of the people by providing basic services though the activated marine reserve units in the country by strengthening its capabilities, resources, training and equipments to be effective in providing basic services to the Filipino people in the Area of Community Relations, Disaster Response and Community Development Related Projects. * The Association has regular activities on the following; *Public Affairs like, Symposium/Public Forum, Press, Radio and TV Releases, Advocacy and Information Campaigns and Community Dialogue. * Civil Affairs like; Medical/Dental/Optical Civic Action Program (MEDOCAP), Disaster Operations and Equipment Transfers, Management and Control of Evacuees and Dislocated Persons, *Environmental Concerns (Ex. MONETARY DONATIONS Members of Marine Corps Semper Fi Phils., Inc is an all-volunteer disaster response group that is also involve in various community development and service projects. As such, we are not receiving any funding from the government or private sector. We exist through the voluntary contributions of its members, fundraising and donations. We rely on the support of businesses, individuals, LGU’s and others to help us perform our job effectively. Though we have a list of needed items, monetary donations are also needed for other things like, insurance, training, transportation allowance, gas, etc. We do appreciate any donations you or your company or organization would like to provide. Any donations made, either in cash or in kind is duly receipted and recorded in our books of accounts, in which a yearly accounting is made and reported to the Securities and Exchange Commission and other regulatory agencies of the government, as provided by law.

Stand by to support disaster, relief and rehabilitation efforts of NAVFORCEN during the calamities that struck Southern Leyte on December 2003.

Information Caravan and Medical-Dental Mission on March 12, 2004 in Olango Island, Lapulapu City. This activity was initiated by AGIO 7 in cooperation with Philippine Information Agency 7. Close to 600 residents availed of the medical-dental mission.

Information Caravan and Medical-Dental Mission on May 14, 2004 in Barangay Casuntingan, Mandaue City held at Cabancalan I Elementary School. This activity was initiated by AGIO 7 in cooperation with Philippine Information Agency 7. Close to 200 patients were treated.

4)

Medical Dental CIVAC at Barangay Buagsong, Cordova on May 18, 2004. The activity was initiated by HQS, Naval Forces Central Philippines and supported by HQS, Naval Reserve Center Eastern Visayas. A total of 324 patients were treated as follows; Dental – 85, Medical – 180, Circumcisions – 59.

Information Caravan with Medical Dental Mission conducted at Poblacion, Minglanilla, Cebu on 04 to 05 August 2004. The activity was initiated by Association of Government Information Officers 7 (AGIO 7) in cooperation with Philippine Information Agency 7. There were 712 Medical and 172 Dental patients treated. We also conducted Operation Linis or Street cleaning in the major streets and thoroughfares of the municipality.

Medical Dental Mission at Barangay Pitalo, San Fernando, Cebu in coordination with the Association of Government Information Officers 7 (AGIO 7) on 14 January 05. A total of 862 patients were provided with free medical and dental assistance with medicines.

Information Caravan with Medical Dental Mission conducted at Barangay Don Andres Soriano, Toledo City on 19 May 2005. This activity was initiated by Association of Government Information Officers 7 (AGIO 7) in cooperation with Philippine Information Agency 7. There were 204 Medical and 122 Dental, 32 Urinalysis, 52 Blood Typing, and 107 Operation Tuli. A total of 588 patients were treated and provided free medicines.

Information Caravan with Medical – Dental Mission in Antequera, Bohol on 26 August 2005. A total of 339 Medical/Pediatric, 81 Dental and 119 Optical with a grand total of 539 patients/beneficiaries were treated and provided with free medicines.

Information Caravan with Medical and Dental Mission in partnership with Association of Government Information Officers - 7 at Barangay Poblacion, Dauis, Bohol for the period of 3 days from 02 to 04 February 2006.

Information Caravan with Medical and Dental Mission upon the invitation and in partnership with the Office of the Cebu Vice Provincial Governor, the Mayor of the Municipality of Cordova, Cebu and the Association of Government Information Officers – 7 (AGIO 7) at Barangay Cogon, Cordova, Cebu on 11 February 2006.

Medical and Dental Mission upon the invitation and in partnership with the Office of the Cebu Vice Provincial Governor, and the Mayor of the Municipality of Poro, Camotes Island, Cebu at Barangay CagCagan, Poro, Cebu for the period of 3 days, from 17 to 19 April 2006.

Environmental Concerns (Ex. Tree planting, Environmental Coastal Clean Up 1)

Environmental Coastal Clean-up Activity in coordination with the local government unit of Talisay City on 5 September 2004. The activity was conducted at the coastal areas of Barangays of Poblacion and Cansojong. In connection with the observance of the 25TH National Reservist’s Week Celebration

Mangrove Tree Planting Activity at Barangay Buagsong, Cordova, Cebu on September 2004. Some five hundred (500) mangrove seedlings were planted in the shores of the said barangay. In connection with the observance of the 25th National Reservist’s Week Celebration.

Repair in the Housing Project at Eversley Child, Sanitarium, Jagobiao, Mandaue City in coordination with the Gawad Kalinga Foundation on 30 January 2005.

Repair in the Housing Project at Eversley Child, Sanitarium, Jagobiao, Mandaue City in coordination with the Gawad Kalinga Foundation on 27 February 2005.

Invitation was made by the Gawad Kalinga Foundation in making the necessary preparation for the launching of it’s Free Housing on 23 July 2005.

Cleanliness and Beautification Projects (Brigada Eskwela) 1)

Annual Alay Lakad held at the City of Cebu from Plaza Independencia to Cebu City Sports Complex on October 23, 2005.

Conducted Operation Balik Aral Linis Program (DEP-ED Brigada Eskwela) at Lapu-Lapu City Central School, Lapu-Lapu City on 21 May 2005. In coordination with the principal of the school, Mrs Teresita Bahandi. Four (4) rooms were repaired, one (1) room minor demolished, five (5) rooms were painted, eight (8) Tables and Desk were painted and 2 hectares school surrounding was cleaned.

Other cause oriented activities, Humanitarian Assistance and Disaster Relief (HA/DR) Operations

Relief and rescue operations, crowd control, firefighting, perimeter security against lawless elements that preys on helpless fire victims, evacuation of residents and their personal belongings during the fire that gutted Sitios Sto Nino, San Vicente Ferrer and Sitio Silangan Uno in Barangay Tinago and Silangan Dos in Barangay Tejero in the City of Cebu on 12 November 05. There were 170 families that were left homeless. An estimated P4.25M in properties went up in smoke. The Bureau of Fire Protection tapped the fire at 11:29AM. The community also lost a chapel and the Cebu City Nutrition Scholar building. Evacuation and transfer of equipments and supplies of Naval Supply Depot to include the more than 20 drums of bunker fuel to the basketball court adjacent to the PNSLAI Cebu Branch

Humanitarian and Civic Assistance (HCA) Projects 1)

I)

Disaster Response and Rescue Operations at Barangay Ginsaugon, St. Bernard, Southern Leyte in connection with the Mudslide Tragedy for the period 21 to 28 February 2006 to assist and participate in the rescue, retrieval, relief and rehabilitation operations to include loading, unloading, stocking, transfer, issuance and distribution of donated relief items from truck, warehouse, site area, chopper and vice versa. The Unit was attached to the 31st Marine Expeditionary Unit, Marine Expeditionary Unit Service Support Group 31, Marine Forces Pacific, United States Marine Corps.

BUNDLES OF JOY Activity at Sitio Inanuran Island, Matabao, Tubigon, Bohol on 19 December 04. Beneficiaries of the activity are the islands of Inanuran and Bilang-Bilangan and the coastal barangay’s of Panaytayon, Matabao and Palayan. A total of 151 indigent families were the beneficiaries of the said activity. This unit repacked and distributed a total of 235 plastic bags containing assorted clothes, biscuits, canned goods, noodles, toys, candies, etc.

Blood Letting Activities, Crowd Control and Others 1)

Crowd Control for 2004 Sinulog Mardi Gras.

2)

Appointed as Commission on Elections Deputy on May 10, 2004 National and Local Election.

3)

Crowd Control/Perimeter Security in the Inaugural Ceremony of Her Excellency Gloria Macapagal Arroyo as President of the Republic of the Philippines at Cebu Provincial Capitol on June 30, 2004.

4)

Blood Letting Activity on 29 August 2004 conducted by the Medical personnel of Regional Blood Coordinating Council (F) Inc. (RBCC). The activity was conducted at the University of Cebu-METC/Capt. Susifatro U. Abella Gym. A total of 30 personnel of this unit donated blood which totaled to 13,500 CC of various types. In connection with the observance of the 25TH National Reservist Week Celebration.

Blood Letting Activity conducted by Medical personnel of Chong Hua Hospital. The activity was conducted at the University of Cebu-METC/Capt. Susifatro U. Abella Gym, Alumnos, Mambaling, Cebu City on 12 December 04. A total of 8 personnel of this unit donated blood which totaled to 4,000 CC of various types. A total of 34,650 CC of blood were donated.

Crowd Control during the Grand Sinulog Mardi Gras 2005 on 16 January 2005. In connection with the observance of the yearly traditional feast of Senior Santo De Cebu.

Participated in the parade at Cebu City Sport Complex, Abellana, Cebu City on 12 June 05. In connection with the observance of the Philippine Independence Day

8)

Blood Letting Activity held at Abella gym at the University of Cebu, Alumnos Mambaling Campus on 09 July 2005. A total of 13 personnel donated blood which totaled 5,850 CC of various blood types.

Crowd Control Augmentation ICOW the celebration of the traditional feast of Senor Santo Niño De Cebu “Grand Sinulog Mardi Gras 2006” for the period of 3 days, from 14 to 16 January 2006.

Conducted 30 days Ready Reserve Organizational Training (RROT) CL 01-2004

Conducted 30 days Disaster Response and Rescue Unit (DRRU) Training CL 01-2004 for the period 07 August 2004 to 18 September 2004.

Refresher Training and Educational/Observation Trip to HQS NAVRESCOM in Intramuros Manila, HQS Philippine Marine Corps in Fort Bonifacio, Taguig City and Marine Base in Ternate, Cavite on 18 to 26 March 2005. The purpose of the trip is to acquaint the reservist, visit various PN/Marine units and meet the personnel directly administering the marine reservist. Various subjects were also taken up with Briefing, Lectures and Practical Exercises being done at The Basic School of the Philippine Marine Training Center in Ternate, Cavite like Combat Formations, Heliborne Operations, Grenade Throwing Techniques, Amphibious Operations, Manual of Arms and Chanting. The knowledge learned will help in disseminating the proper and correct procedure to other reservist considering that the main core of our Unit Activity is CMO and Training.

On the Job Training (OJT) at Naval Operating Base Mactan, Headquarters Naval Forces Central, Canjulao, Lapulapu City on Base Security and Guard Mounting. This was conducted on 10, 17 and 24 December 2005.
