Mechanism of action and indications Management of hypertension, CHF and Reduction of risk of death or development of CHF following MI. Slowed progression of left ventricular overt heart failure. Prevention of migraine headaches. Lowering blood pressure, decreased afterload (CHF), decreased development of overt heart failure. Increased survival after MI.
Interactions with other patient drugs, OTC, or herbal medicines (ask patient specifically) diuretics, antihypertensives, nitrates, phenothiazines, alcohol surgery, general anesthesia, potassium supplements, potassium-sparing diuretics, indomethacin, salt substitutes, cyclosporine, NSAIDS, antacids, lithium, digoxin. Nursing Implications (what to focus on) Contraindications/warnings/interactions Hypersensitivity, cross-sensitivity among ACE inhibitors may occur. Pregnancy. Exercise extreme caution in Family history of angioedema. GERI: Renal impairment, hepatic impairment, hypovolemia, hypoatremia, concurrent diuretic therapy, aortic stenosis/ hypertrophic cardiomyopathy, cerebrovascular or cariac insufficiency. Common side effects dizziness, fatigue, headache, insomnia, weakness, cough, eosinophilic pneumonitis, hypotension, angina pectoris, tachycardia, taste disturbances, anorexia, diarrhea, hepatotoxiciy (rare) nausea, proteinuria, impotence, renal failure, rashes, hyperkalemia, Agranulocytosis, angioedema, fever. Be sure to teach the patient the following about this medication Avoid salt substitutes or high level potassium/sodium foods. Notify if have rash mouth sores, sore throat, fever, swelling of hands or feet; irregular heart beat, chest pain, dry cougy, hoarseness, swelling of face, eyes, lips, or tongue, difficulty swallowing or breathing occur.
