POTENTIAL HAZARDS

Refer KOC Standard for Shutdown System Doc. No. KOC.I. 004; & HSEMS Procedure - Doc. No. SA.KOC.004 “Permit to Work”.



Take a Cold Work Permit & other associated permits from Asset Owner.



Plan the work involving personnel responsible for preparation of ESD Functional Testing.



Conduct Tool Box Talk.



Maintain a radio communication between worker at Control Room / ESD panel & person to verify the status of ESD valves in the field.

Lack of Updated Documents & List of ESD Valves



Keep updated lists of all ESD Valves & BDVs (Blow Down Valves) indicating its normal as well as ESD status.

Shutdown of Facility / System / Equipment



Take advance approval from Asset Owner & line management before carrying out functional testing of ESD System.

Wrong Identity of ESD Switches (ESD Level 1, 2, 3 & 4)



Ensure the identity of Switches for the respective ESD Level before actuating it.

Failure of ESD to Actuate in manual or auto mode



Refer Cause & Effect Diagram.



Investigate the problem and rectify it.



Note the ESD Valve # in the list and inform Control Room.



Find out the reason and rectify the defect.



Note the BDV # in the list and inform Control Room.



Find out the reason and rectify the defect.



Note the ESD / BDV # in the list and inform Control Room.



Find out the reason and rectify the defect.



Verify the normal as well as emergency situation status of ESD Valves & BDV individually from the site & Control Room.



Note the ESD / BDV # for which the location & control room status is not matching.



Find out the reason and rectify the defect



Ensure proper wind up & housekeeping at worksite.



Normalize ESD System after evaluating the hazard.
