Not monitoring I.G. being delivered. Faulty Instrument Hatches and purge stacks not properly tight and gaskets not checked.
Recommended Actions or Procedures Control of access inside the Cargo Tanks and proper monitoring. Check Cargo Tanks by Visual and audible means to ensure there is no personnel inside. Verify cargo hatches are tight and gasket properly sealed. Verify spade blind is removed and I.G. branch valve is open on COT to be inerted. All other COT I.G. branch valve must be closed. Ensure I.G. being delivered is less than 5% by volume. Calibrate Instrument before use. Take oxygen reading on various levels. Verify cargo hatches and purge stacks are tight and gasket properly sealed.
