House Rules and Regulations Of The Biopolis Building ACCESS AND OPERATING HOURS 1. The Biopolis building is a twenty-four (24) hours operation together with its Engineering department. However the Admin Office Hours will only be until six in the evening (6:00 pm); 2. Visitors/ Guests of Tenants (a) must present and surrender a valid issued identification card (ID) with their recent picture for log-in purposes; (b) must ONLY be verified by the Authorized representative in exchange for a visitor’s card; (c) Visitor’s Pass MUST ALWAYS BE WORN AT ALL TIMES within the building premises; 3. ANY AND ALL KINDS OF FIREARMS AND OTHER SIMILAR WEAPONS ARE STRICTLY PROHIBITED ANYWHERE WITHIN THE BUILDING PREMISES. Furthermore, ANY AND ALL REQUEST to be reconsidered and or be exempted with the above mentioned item, WILL NOT BE ENTERTAINED. 4. DRESS CODE: STRICTLY NO WEARING OF REAVILING TOP, SHIRTS with PROFOUND LANGUAGE(s), MINI SKIRTS & SHORT SHORTS, MID RIB BLOUSES, TUBES and SLIPPERS. Inappropriately dressed are not permitted to enter within the building premises; 5. All Tenants and Employees are required to maintain proper attire including company issued identification cards (ID); Note: It does not authorize anybody, in the absence of company uniform(s) to wear anything at-will during business operations and work schedule and even office visit purposes 6. All tenant and their employees, visitors and guests are expected to extend courtesy, respect, obedience and preserve peace, order, and harmony at all times with co-employees, peers, subordinate and superior regardless of its race, color, status, sex, nationality, religion, citizenship, ancestral origin, disability & medical condition, age and marital status.

USE OF ELEVATORS ALL Elevators are designated as “NON SMOKING” AREA There are six (6) passenger elevators serving from the Ground Level to 11 th floor. One (1) elevator is designated as the Service Elevator will serve as the Cargo Elevator. It is strictly prohibited to tamper/play with the elevator control buttons, light switches, doors, etc. other that for selecting the appropriate/destination floor level A. Passenger Elevators The Elevators shall be for the exclusive use of the Tenants, Employees, Guests and Visitors for their transport to their designated floor level(s) B. Service Elevators [Open to all Floor(s)] 1. Transport using cart with wheels may be required for large or heavy items shall only be allowed within service elevator, 2. Baggage, Luggage, Furniture, Equipment, Boxes, Supplies, Goods, Packaged delivery of food, and other heavy or voluminous articles that are to be delivered to the Tenant’s Unit(s) shall enter the building premises using the ramp area for proper inspection, and are only allowed to use designated service elevator or fire exit stairs 3. Any form of garbage or litter shall only be transported using the Service Elevator and is Strictly Prohibited from the Passenger Elevators, 4. Use of the designated service elevator for HAULING shall be based on the scheduled time as preferred by garbage haulers, its authorize representatives or any of the engineers, duly acknowledge by the same. All deliver/pullouts must be at the lower ground loading and unloading area and shall be supported with an accomplished INWARD form and/or OUTWARD form with DESIGNATED PURPOSE to be secured from the Engineering department 5. When heavy and/or voluminous articles are to be delivered from ground floor thru the service elevator, protective padding or board must be provided by the user 6. Equipment or articles in excess of the maximum load capacity of the elevators shall not be allowed, 7. All Damage(s) to the building common area cause by the moving or carrying of articles shall be paid for by the requesting tenant. Violation is subject to penalty computed on the amount of the items (x50%) of the whole amount of the damaged area/item. In cases that the Tenant’s food is being delivered (A)Food must be packed individually per tenant “one is to one” (1:1) and must be properly endorsed in the Admin Office accompanied with a Letter indicating the NAME and TIME of Delivery thereon. (B)Food that are ordered in the fast food chains ie., Jollibee, Mcdo, Kfc, and alike and even with Mr. Tea Café is strictly prohibited to access any elevator. Any ordered food and or beverages must only be picked up within the Reception Area.

AGAIN, ANY CONSIDERATION will not be given to anyone’s FAILURE TO ABIDE

SECURITY AND SAFETY 1. Security, maintenance and janitorial services are provided with a primary function to serve the common areas of the building, however, The tenant shall be the sole responsible for the security inside his office unit and leased premises and likewise the sole responsible for the Conduct and Behavior of their Employees, Contractors and Visitors provided that the Building house rules are strictly implemented and followed; 2. Gold Quest Premiere Resources Inc.(GQPRI) and its owners, staff/ employees, and other business related to GQPRI shall not be HELD liable in any matter to the Tenant for any negligence, loss and or damages arising from the admission, exclusion or ejection of any person to or from the office unit or the building itself; 3. The Property Manager and its Authorized representative shall have the right of access to any unit for the purpose of making inspection ( as stated in the signed lease of contract). Such entry shall be made during reasonable time or hour, except in cases of emergency and with the least disturbance and inconvenience to the Tenants;

4. All tenants shall be responsible for maintaining the Smoke detector in each unit/ leased premises to ensure it is FUNCTIONING at all times . Tampering and/or removing of any signage(s), tag(s) or any fire safety equipment or device placed within the leased premises is STRICTLY PROHIBITED, however, in cases of intentional tampering and or removing shall automatically be Subject for Termination of Contract of Lease; 5. All tenants are obliged to prove the required fully-charged fire extinguisher/s at all times within the Leased Premises except within the Common Area(s) ; 6. Only Authorized representatives of GQPRI are allowed to access all electrical/meter rooms, machine rooms, maintenance room and alike.

CCTV The Closed Circuit Television Monitors (CCTV), has been designated to monitor the various important systems of the building from one location. Access is limited to authorize personnel only. In cases of unexpected events that might occur, the interested party accompanied by a request letter will only be entertained by the Authorize Admin representative to re-view the incident date. However, Producing a copy of the re-view video is STRICTLY PROHIBITED for the safety purposes of the Building’s and other tenant’s interest.

GENERATOR Generator sets are provided to ensure an emergency power back up; such as elevators, lightings along the hallways, Corridors, and each of the tenant’s unit during power outages.

DISPLAYS, NOTICE, POSTERS NO signage(s), advertisements, posters, or notices shall be inscribed, posted, painted on or displayed in any common area of the building including the circulation thereof within the leased premise(s) without securing written permission addressed to the Building Property Manager and or Admin Manager/Legal Supervisor. WASTE COLLECTION AND DISPOSAL GUIDELINES Proper garbage Disposal under BIOP-SM-010 DATED September 18, 2017 has been properly disseminated and acknowledged by the Tenants.

USE OF LEASED PREMISES A. Cleanliness/Sanitation All tenants should maintain their respective units in good and sanitary condition. No rubbish, refuse or garbage shall be allowed to accumulate nor shall any fire hazard be allowed to exist in any part thereof. Tenants shall be bound at all times to follow the rules and regulations promulgated by the GQPRI for the disposal of rubbish and maintenance of cleanliness and sanitation within the building premises. ( Please refer to BIOP-SM-010)

B. Nuisance All tenants shall use and maintain their leased premises in a peaceful and reasonably quiet manner. Tenants shall refrain from any noise or loud acts that would disturb nor annoy the peace of the premises and its other tenants . No immoral, improper, or unlawful behavior and physical violence shall be allowed neither be tolerated within the building premises.

C. Vendor and Solicitations 1. Vendors are Strictly Prohibited from entering and selling their products within the building premises. 2. Solicitations and alike are likewise prohibited inside the premises.

D. Prohibited Items GQPRI strictly prohibits the bringing in and storage of liquid petroleum gas products, toxic items, and flammable/inflammable products or items potentially hazardous nature inside the building

E. Utilities, Repairs and Alteration(s) In cases that upon the inspection of units, altered units without the knowledge of the Admin Office shall automatically be penalized, NO RECONSIDERATION SHALL BE GIVEN to any deviators thereof.

F. Pets GQPRI strictly prohibits the bringing of PETS within the building premises.

COMMON AREAS General Limitations Gold Quest Premiere Resources Inc. - The Biopolis is a Smoke-Free building (a)

Violators caught Smoking in the common areas inside the building through Any Authorized Representative from the Admin Office and Engineering Department, as well as the Duty on Guard has the authority to confiscate the ID of the Tenant, its Employee, and or its guest and visitor and shall be subject to pay the of violation fine, amounting to Php 10,000.00.

NOTE: Notice or Warning shall not apply to any deviators (b)

The Tenants are likewise PROHIBITED IN SMOKING within the common Toilets, Elevator lobbies, Reception area, Fire Exits and other Common areas,

(c)

Nothing shall be done in any unit or in any common area or limited areas, which will impair the structural integrity or aesthetic design of the building specifically those which would change the exterior or façade of the building,

(d)

Any damage or injury caused to any of the common areas and facilities through the willful or negligent act of a Tenant concerned shall be charged on their end. Should repair be impossible, the Tenant shall replace or fully compensate for the damaged area or facility,

(e)

Nothing shall be placed neither stored in the common areas specially FIRE EXITS,

(f)

Dusting Out of rugs, throwing of cigarette butts and the like from windows or by beating them on the exterior of the building is STRICTLY PROHIBITED,

(1)

Open Space Areas (a) Strictly NO DOUBLE PARKING or FLOATING of waiting vehicles within the access areas, and Repair Work Vehicles ALL DELIVERIES are strictly given only five (5) minutes to unload

cargo(s) (b) LOADING AND UNLOADING FOR ALL TRANSPORTING VEHICLES must only be DONE AT THE DESIGNATED LOBBY AREA

(c) Indiscriminate blowing of car horns within the premises of GQPRI is strictly prohibited (d) Gambling, drinking of alcoholic beverages, and loitering within the open areas of GQPRI is strictly prohibited

Toilet Area/Comfort Rooms (a) The use of comfort rooms, at all times should be utilized with proper care as a courtesy to the next user. Throwing of unnecessary rubbish in the urinal bowl and alike, and Stepping on the Toilet Bowl is STRICTLY PROHIBITED, (b) No Urine Pee and or Water floor droplets in the comfort rooms including the PWD should be observed to prevent the accumulation of disgusting foul odor and possible accidents, (c) All water faucets should be utilized with extra care to avoid breakage and malfunction, (d) WASHING OF ANY UTENSILS such as coffee cup, teaspoons, plates, bowls, and other alike used for eating and drinking IS STRICTLY PROHIBITED. (e) STRICTLY SMOKING and LOCKING the doors because of smoking inside the COMMON TOILET AREA IS STRICTLY PROHIBITED

(3)Reception Area, Corridors, Isles, Open Spaces and Emergency/Fire Exits (a) RECEPTION AREA: Strictly Authorized Personnel are allowed behind the Lobby Desk, unauthorized visitor or guests or tenants are prohibited to go beyond the desk, (b) Strictly no eating of ICE CREAM and food alike that might cause drip on the floors of the grand lobby to prevent stain on the floors (c) Coffee and other hot beverages Without Lid/Cover shall not be allowed within the premises of the building (d) These areas should be kept clean at all times, open and safe from all passerby and tenants. (e) No obstruction of any kind is allowed to be placed in any areas without the approval from the Property Manager, Engineering Department, and Admin Manager/Legal Supervisor (f) BYSTANDER(S) is/are STRICTLY PROHIBITED (g) Littering, Spitting and Loitering with in the Reception Areas, Corridors, Isles, Open Spaces and Emergency/Fire Exits areas is Strictly Prohibited (h) The emergency/ fire exits must be kept closed at all times and no one should be allowed for daily usage unless otherwise approved and authorized by the Property Manager

Tenants occupying an entire floor must ensure that the layout of furniture and files does not obstruct the way to the fire exit door

All Existing MEMO(s) duly received and has been disseminated must be strictly followed FAILURE to do so does not excuse any one from its/ their liability;

All MEMO(s) shall be updated from time to time supported by a written receiving paper, signed by the Authorized Representative;

(3)

All MEMO(s) that are to be updated from time to time is rooted to a case-to-case basis and its necessity;

(4)

HOUSE RULES shall be amended as the need arises without prejudice to the Tenants, in which they are expected to abide by all the existing and amended rules;

(5)

Receiving Letters that must be received duly by the Authorized Signatory before producing a copy thereof to avoid miscommunication.

ADDITIONAL PROVISIONS 1.

ALL GLASS WINDOWS FROM THE FIFTH (5TH) FLOOR UP TO THE ELEVENTH (11TH) FLOOR SHOULD BE CLOSED AT ALL TIMES.

Any violation reported by our

Authorized Representative from the Admin Office and Engineering Department, as well as the Duty on Guard, regarding opened windows found within your lease premises shall be equivalent to a violation fee of five thousand pesos (Php 5,000.00) 2. For reiteration that “The Biopolis is a Smoke-free Building”, the penalty for triggered smoke detector shall now be equivalent to one hundred thousand pesos (Php 100,000.00). Moreover, we will also change the penalty for violators caught smoking, instead of just the violators paying the amount of ten thousand pesos (Php 10,000.00), the penalty shall now be charged with ten thousand pesos (Php 10,000.00) for every stick of cigarette buds that our Authorized Representative will find within the area of your lease premises. 3. (For non-restaurant tenants) Cooking is not allowed inside the building, therefore ALL COOKING UTENSILS AND RAW FOOD WILL NOT BE ALLOWED TO ENTER INSIDE THE BUILDING PREMISES. Every cooking utensils and raw food found by our authorized representative inside your lease premises shall be charge with five thousand pesos (Php 5,000.00) per piece.
