Brand Name: Flagyl Generic Name: Metronidazole Indications: mild pain, fever Drug Classification: Anti-infectives, anti-protozoals, antiulcer agents Mechanism of Action: Disrupts DNA and protein synthesis in susceptible organisms. Most notable for activity against anaerobic bacteria, including: Bacteroides, Chlostridium. In addition, is active against Trichomonas vaginalis, Entamoeba hystolystica, Gardia lamblia, H. Pylori, and Chlostridium difficile. Dosage: Adult: Anaeroboc infections – 7.5 mg/kg q 6 hr. Trichomoniasis – 250 mg q 8 hr for 7 days or single 2-g dose or 1 g bid for 1 day. Amebiasis – 500-750 mg q 8 hr for 5-10 days. H. pylori – 250 mg qid or 500 mg bid for 1-2 wks. Bacterial vaginoses – 750 mg od as ER tablets for 7 days. Children: Trichomoniasis – 5 mg/kg q 8 hr for 7-10days. Amebiasis – 11.6-16.7 mg/kg q 8 hr for 5-10 days. Special Precautions: History of blood dyscrasias, history of seizures or neurologic problems, severe hepatic impairement, pregnancy, lactation, children. Pregnancy Risk Category: B Adverse Reactions: CNS: Seizures, dizziness, headache EENT: tearing (topical only)

Derm: Rash, urticaria; topical only – burning, mild dryness, skin irritation, transient redness GI: Abdominal pain, anorexia, nausea, diarrhea, dry mouth, furry tongue, glossitis, unpleasant taste, vomiting Hemat: Leucopenia Local: Phlebitis at IV site Neuro: Peripheral neuropathy Misc: superinfection. Contraindications: Hypersensitivity, hypersensitivity to parabens (topical only), first trimester of pregnancy. Forms: Tablets – 250 mg, 500 mg; ER tablets – 750 mg; Capsules – 375 mg; Powder for injection – 500 mg vials; Topical gel – 0.75% in 28.4-g tubes; Topical cream – 0.75% in 28.4-g tubes; Topical lotion – 0.75% in 59-ml bottle; Vaginal gel – 0.75% in 70-g tubes. Nursing Responsibilities:  Assess patient for infection at the beginning of and throughout therapy.  Obtain specimens for culture and sensitivity before initiating therapy.  Monitor neurologic status during and after IV infusions.  Monitor intake and output and daily weight  Administer with food or milk to minimize GI irritation. Tablets may be crushed for patients with difficulty in swallowing.
