This has two parts: a survey and a question and answer. Be honest in responding to questions and information sought. Cite issues that will help improve the current situation. COMMUNICATION I received an initial orientation that briefed me what can I expect from my work. I received appropriate feedback from my immediate supervisor about what I did well and also what I needed to improve on. There was teamwork among my coworkers. My opinion and ideas were considered at work. I received information on changes affecting work. TRAINING AND PERSONAL DEVELOPMENT I understood the procedures of my job.
