1.0 Purpose/Scope This ADW Emergency Management Plan (EMP) outlines the procedures and resources necessary for initiating the response to emergency incidents within Alaska Drilling and Wells (ADW) operations on the North Slope. The EMP is designed to comply with the requirements of the BPXA Incident Management System (IMS). To assist ADW personnel in meeting these requirements, the EMP includes the following: •

Criteria for classifying the level of incident severity;



Established notification protocols for the given level of incident;



Current contact information for ADW, ADW Contractors and other affected parties;



Roles and Responsibilities for ADW personnel involved in the response effort;



Outline of ADW responsibilities within the Incident Management System. .

3.0 General Requirements The ADW Emergency Management Plan has been prepared to assist ADW staff during the initial stages an emergency incident. The EMP describes how ADW elements fit within the broader BPXA Incident Management System (IMS), as described in the IMS manual, Volume 1. The ADW Emergency Management Plan does not replace the Arctic Well Control Contingency Plan (AWCCP), the Blowout Contingency Capping Plan (BOCP) or any field-specific Oil Discharge Prevention and Contingency Plans (ODPCP’s).

4.0 Key Responsibilities Roles and Responsibilities for affected ADW personnel are defined in Appendix 1 – Roles and Responsibilities.

5.0 Procedure/Process 1.

The ADW Leadership Team shall review the EMP annually. The contact information will be reviewed every six (6) months and updated as required.

NS Drilling HSE Advisors will include the Emergency Management Plan in their six-month Rig Audit inspections.

3.

All updates will be coordinated via the Document Control Process. All paper copies of the EMP, along with all related attachments, are considered uncontrolled documents. 1.0 OPERATING STATEMENT The ADW Emergency Management Plan (EMP) has been prepared for BPXA Alaska Drilling & Wells (ADW) operations to provide guidance regarding the initial actions required at the onset of an ADW emergency incident. It is not a regulatory document. The order of priority in an emergency situation shall be: • • • •

Protection of life Protection of the environment Protection of equipment and assets Protection of the well

The priority in any emergency situation shall be to safeguard life. No work shall commence or continue unless appropriate safety precautions have been drafted, reviewed, approved and implemented. This document is controlled and shall be reviewed and updated per the defined intervals established by the designated ADW Authority. Hardcopies of this plan will be distributed to all relevant end-users, including: • ADW staff • ADW facilities & locations (i.e. drilling / workover rigs, well service units, support operations, etc.) • ADW Contractors • Affected Anchorage / North Slope BPXA staff • External users, as required Hardcopies are valid only at the time of printing and are to be considered uncontrolled. 2.0 INTRODUCTION 2.1 Overview The goals of the ADW Emergency Management Plan (‘EMP’) are: •

To provide specific guidance related to the necessary notifications to initiate the required level of the Incident Response Organization (IRO) and;



To define the roles and responsibilities for ADW positions within the BPXA Incident Management framework.

The EMP has been prepared to assist any affected ADW or ADW Contractor personnel during the initial stages of an emergency incident. It is not intended as a replacement of the specific Contingency Plans: •

ADW Blowout Contingency Plan – Well Capping



Asset specific Spill Prevention, Control and Countermeasure Plan

2.2 Scope This EMP is intended to cover all ADW related facilities and operations, including: • Drilling facilities and operations; •

Workover facilities and operations;



All additional well-based facilities and operations not covered by the above (i.e. well servicing and support, etc.).

2.3 Personnel Safety The Wellsite Leader’s first and highest priority during the initial stages of an incident is to provide for the safety of personnel. If necessary, personnel shall be evacuated to a designated Safe Area. The Wellsite Leader shall account for all on-site ADW and contractor personnel. If simultaneous operations are in progress on the drill site or pad, personnel involved in those operations shall be notified of the situation and evacuated as necessary.

2.4 Primacy of ADW Emergency Management Plan For all incidents taking place within the BPXA ADW area of operations, this Emergency Management Plan shall take precedence over any other plan(s), including earlier revisions of the ADW EMP and any contractor’s specific emergency response plans.

2.5 Initial Response The EMP classifies well-based emergency incidents according to the appropriate response level, following established and accepted procedures. It provides a decision-based guideline on how to respond to an incident by means of a four (4) step process: 1. Recognizing the level of emergency; 2. Prioritizing of actions; 3. Defining roles & responsibilities; 4. Initial response actions. The Initial On-Scene Commander (‘IOC’) is responsible for assessing the level of incident severity and initiating the appropriate notification process. Following BPXA protocol, ADW incidents will be identified as either Level ‘0’ or Level ‘1’. Refer to ‘Section 3.2 – Emergency Level Criteria’ for further details. Based on the Incident Level determination, the IOC will make the required notifications, as detailed in ‘Section 3 – Emergency Notification Process’.

2.6 Roles and Responsibilities The roles and responsibilities for specific ADW personnel, within the Incident Command System framework, are defined in ‘Appendix 1 – ADW Roles & Responsibilities’. All affected ADW personnel are expected to be familiar with the requirements of this Emergency Management Plan and in particular, the roles and responsibilities for which they may be assigned.

2.7 Communications & Documentation During any incident, information is generated and communicated from many sources. Effective reporting is essential to ensure factual information reaches the appropriate party in the shortest time and to provide assurance to BPXA and ADW management that appropriate resources are engaged and response actions are being initiated. Do not speculate regarding unknown situations or conditions. Do not speculate concerning status of personnel. Direct communication is the preferred method, rather than via a second party; contact the specific resource required to address specific needs. In general, communications shall be directed through a single point of contact. All requests from external news media shall be directed to the appropriate team leader. Ensure all incident-specific communications regarding decisions made, any actions taken and all items requiring tracking, are recorded / logged in the ICS Form 214, Unit Log (Appendix 4 – Forms). In addition to the Unit Log, an effort should be made to collect and archive any additional communications that may not be logged otherwise, i.e. general communications, etc. Refer to ‘General Roles & Responsibilities / Checklist’ for ADW Documentation Leader.

This manual contains reference copies of the most frequently used forms (refer Appendix 4 – Forms). The WSL should make copies of these forms and include in a file folder / clipboard to be kept in the rig ‘Go Bag’ (refer Appendix 1 – Roles and Responsibilities for WSL).

2.8 Decision Making Prior to proceeding with any field intervention operations taken to establish source control, the Alaska VP Drilling and Completions (or designee) must concur with and approve of the proposed activity. This will ensure that appropriate resources have been involved with the proposed plan and consensus has been achieved.

3.0 EMERGENCY NOTIFICATION PROCESS 3.1 Initial On-Scene Commander Anyone who first recognizes an event taking place that poses a risk to or endangers personnel, the environment or facilities, shall assume the role of Initial On-scene Commander (‘IOC’). The individual serving in the IOC capacity will maintain that role until properly relieved of that duty by the appointed On-Scene Commander (‘OC’). For ADW operations, the IOC role will typically be filled by a Wellsite Leader (‘WSL’). An exception to this case may occur during some well service or remediation operations, i.e. slickline operations, when a WSL is not normally present. It is important for any person who may be called on to serve as IOC to be familiar with the requirements, roles and responsibilities of the IOC position. For purposes of this document, the term ‘IOC’ will be used exclusively to mean the relevant individual, whether the ADW WSL or the ADW Contractor Supervisor.

3.2 Basic Requirements During a Level ‘0’ or Level ‘1’ emergency incident, as defined below, the IOC has four (4) initial responsibilities: •

Ensuring the safety of all personnel;



Securing the well / facility, if safely possible;



Securing ignition sources, if safely possible;



Accounting for all personnel and assess any injuries. +++ RULE #1 - THE SAFETY OF ALL PERSONNEL IS PARAMOUNT +++

3.3 Emergency Level Criteria Once the basic requirements noted above have been addressed, the IOC must assess the situation and determine what level of criticality the incident is, or may become in the nearterm. Based on the incident level determined, the IOC will initiate a defined series of notifications, with the number and priority of these notifications determined by the severity of the incident. Refer to the appropriate notification chart after deciding what level of emergency exists. If you are uncertain as to the proper level determination, assume a ‘worst case’ scenario; to optimize a timely response and secure resources, it is better to operate under the assumption that it is Level 1. If the situation later proves manageable under Level 0 criteria and the situation dictates, the level can be relaxed.

3.3.1 Level “0” - Definition A Level “0” incident is one that can be mitigated using local (i.e. readily available) ADW resources and does not require activation of the Incident Management Team (IMT) or ADW Anchorage Response Team. Examples of Level “0” incidents include: •

Rig evacuation with false alarm;



Recordable injury;



Potential loss of primary well control;



Taking a kick / Circulating out kick;



Severe lost circulation (i.e. difficult to maintain adequate fluid supply);



Venting gas

3.3.2 Level “1” – Definition A Level “1” emergency is a well control event which requires the assistance of resources beyond those available locally, i.e. North Slope, using ADW resources. For Level 1 incidents, the affected Area Manager OSM / ISM Area Manager will typically activate the Incident Management Team (IMT). The ADW response then becomes integral to the IMT. The ADW Anchorage / North Slope Response Teams may subsequently mobilize the necessary resources, as required. Examples of Level “1” incidents include: •

A major accident (emergency support required)



A worsening well control situation with a potential for escalation;



An uncontrolled spill with a continuous release impacting the environment;



Rig evacuation with a confirmed H2S release, fire, smoke;



A loss of well control requiring outside resources to regain control;



A catastrophic spill impacting people, environment, facilities, etc, or;



A fire or explosion which impacts people, environment, facilities, etc.

3.4 Emergency Notification Process If a Level “0” Incident is declared, the IOC must make the notifications detailed in Flowchart A – ‘Level ‘0’ Notification Path’. Contact numbers for these notifications are provided on the chart. If a Level “1” Incident is declared, the IOC must make the notifications detailed in Flowchart B – ‘Level ‘1’ Notification Path’. Contact numbers for these notifications are provided on the chart. Additional contact information is provided in ‘Appendix 2 – Contact Information’. * If unable to reach a contact on the Notification Chart proceed to the next contact. For example, if unable to contact the Affected ADW WTL then proceed to contact the Affected ADW Manager.

3.5 Variance in Notification Requirements For incidents involving Liberty, there is a variance in the notification chain. The Liberty WTL will notify the Liberty WPM who will then notify the affected ADW Manager.

3.6 Additional Information Required for Notifications Regardless of incident level, the following information should be readily at hand and provided in the notifications: •

Location – pad / drillsite and well #



Nature of emergency – well control, gas release, fire, injury



Injuries – number and extent



Fluid types and volumes



Supervisor name



Number of people on location



Other work on the drill site or pad – RWO, drilling, well servicing, etc.



On-scene contact point – office or cell phone # / Harmony # / pager # / radio channel that will be monitored



Weather and wind direction at location

In addition, the IOC should be prepared to provide the following data to the ADW Anchorage Response Team and the EOC (Emergency Operations Center): •

3.7 Escalation from Level “0” to Level “1” At any time during an Emergency Incident, the IOC or OC may change the event level status from Level “0” to Level “1”. This may be based upon changing well conditions, revised resource needs or other factors. If the Level is upgraded, the notification process shall be repeated using the path defined in Flowchart B – ‘Level ‘1’ Notification Path’ and each contact informed of the change in event level.

3.8 Duties After Completing Notifications After the required notifications have been made, the affected ADW personnel should refer to ‘Appendix 1 - Roles and Responsibilities’ for additional position-specific duty requirements within the Incident Response Organization.

4.0 BPXA INCIDENT COMMAND SYSTEM 4.1 Overview The Incident Command System utilized by BPXA is structured as the Incident Response Organization (IRO). The IRO provides for efficient modular activation and deactivation of resources, as required to respond to incidents of variable duration and severity. This flexibility to expand or contract the response organization to meet changing needs is a primary strength of the IRO. This system is further detailed in the ‘BPXA Incident Management System (IMS) Manual, Volume 1’. It is important to note that the structures and personnel described herein should be considered examples of the typical response organization; the actual organization may vary. For example, if a well control incident escalates from a Level “0” Incident to a Level “1” Incident, additional components of the IRO may be activated as required by the incident response. At the onset of any ADW emergency incident the Wellsite Leader, or incident observer, assumes the role of IOC and makes the required notifications to BPXA management and specified agencies (refer Flowchart A – Level ‘0’ Notifications / Flowchart B - Level ‘1’ Notifications). Depending upon the severity of the event and the level of response required, the affected North Slope OSM / OIM may initiate the Incident Management Team (‘IMT’). With the initiation of the IMT, the ADW response will become integrated within the broader BPXA Incident Management System. This section describes how the ADW response interacts within the overall IMT response. It also defines the structure and roles of the ADW response teams.

4.2 ADW Management Team The following ADW Organization Chart (Figure 4-1) represents the structure of the Alaska Drilling and Wells Management Team as of May 2010. The individuals currently assigned to these positions, along with their contact information, is available in Appendix 2 – Contact Information.

4.3 IRO Structure for ADW Incidents The BPXA IRO structure, for incidents involving ADW, typically consists of two (2) primary elements: • •

Incident Management Team (IMT) Business Support Team (BST)

Operating within these IRO teams, ADW will provide two (2) response teams: • •

North Slope Response Team (NSRT) Anchorage Response Team (ART)

During ADW-related incidents, ADW personnel will typically be involved at two (2) locations: 1. North Slope – Located at the affected site, operating as the ADW North Slope Response Team (NSRT). The NSRT is integrated in, and provides supports to, the IMT. 2. Anchorage - Located at the Anchorage Drilling Operations Center (DOC), operating as the ADW Anchorage Response Team (ART). The ART provides technical support for the IMT / NSRT operations and interfaces with BST. Within the established BPXA protocol, the IRO follows a well-defined chain of command. This allows response efforts to be maximized and minimizes confusion and duplication of duties. During any Level 1 ADW related incident, the IMT is tasked with implementing the approved Source Control plans at the affected location. Both the NSRT and the ART support the IMT by providing the ADW technical expertise and resources necessary to develop, evaluate and propose viable options that meet the risk mitigation criteria. Any plan for Source Control, with regards to well kill or well capping operations, shall be chosen with the concurrence of the ART and based on final approval of the Alaska VP Drilling and Completions (or designee). The following sections reflect typical staffing models that ADW personnel will utilize during the initial and possibly subsequent stages of an emergency incident. They are intended to provide the user with the tools and guidance needed to assign specific roles and establish the required communications with the various component elements of the IRO. The structure for a typical Incident Response Organization (IRO) is illustrated in Figure 4-2; the basic structure for ADW elements within the IRO structure is illustrated in Figure 4-3. These are located on the following pages.

4.3.1 Incident Management Team (IMT) The Incident Management Team (‘IMT’) is the ‘front line’ unit, operating in close proximity to the affected location. The IMT will be based at the incident command post, located in the affected field office or designated emergency operations center, on the North Slope. The IMT is tasked with the regaining and maintaining safe and effective control of the given incident at the source, utilizing the coordinated resources of the support teams. The ADW North Slope Response Team (‘NSRT’) is an integrated component of the IMT (as shown in Figure 4-6 – ADW NSRT Organization, located on page 4-19). The NSRT will provide Source Control support and ADW expertise at the affected location. The ADW Anchorage Response Team (‘ART’) provides peripheral support to the IMT for Source Control, Planning, Logistics, or other areas as required (as shown in Figure 4-4 – ADW Anchorage Response Team Organization, located on page 4-15). Mobilization of ADW Response Teams Upon initiation of a Level 1 Incident response, the Affected ADW Manager (or designee) assumes the role of NSRT Leader and begins contacting the individuals required to staff the NSRT and the ADW element of the IMT: • Notifies the Unaffected ADW Manager, who mobilizes the ART (refer Section 4.3.2 – ADW Anchorage Response Team); • Mobilizes and supervises the NSRT (refer Section 4.3.3 – ADW North Slope Response Team); ADW Response Team Assignments With activation of the ADW Response Teams, the NSRT / ART Leaders (or designees) should fill in the staffing resource information on the ‘ADW Roles / Incident Management Team’ and ‘ADW Roles / Anchorage Response Team’ forms (refer Appendix 4 – Forms). These documents should be prominently posted at the NS Incident Command Post and Anchorage Control Center. As the response level changes, the corresponding changes in positions should be reflected in the posted staffing charts. A generic staffing sheet, for additional staff requirements, is included (refer Appendix 4 – Forms).

ADW Role Definitions – IMT / NSRT ADW Incident Leader

The ADW Incident Leader role is filled by the Alaska VP Drilling and Completions. The ADW Incident Leader serves a dual role: •

As a member of the BST, the position serves as the resident Source Control authority and expert and;



As the Alaska VP Drilling and Completions, the position has responsibility for oversight of all operations influencing ADW operations.

Source Control Officer

The Affected ADW Manager fills the position of Source Control Officer on the IMT and also serves as the ADW North Slope Response Team Leader, who is responsible for staffing and leading the ADW North Slope Response Team. Typically, this person will travel to the North Slope accompanied by several Drilling Engineers and other required support personnel. This position will coordinate with the Deputy Operations Section Chief (‘DOSC’) to ensure Source Control is appropriately staffed and the ADW Logistics Specialist (Tool Services Supervisor) is engaged with the Logistics Section. Initial On-scene Commander

The base definition of the Initial On-Scene Commander (‘IOC’) is that individual being the first responder to a given incident. Within ADW operations this is taken to be the affected Wellsite Leader (‘WSL’), but may be served by the first responder in the absence of a WSL. The IOC is in charge of the entire response until the tactical teams arrive, when the IOC is relieved of command by the incoming On-Scene Commander (OC). The priorities of the IOC are: 1) 2) 3) 4)

Safety of personnel; Minimization of environmental damage and; Protection of property; Initiation of the Incident Notification process.

The IOC is responsible for establishing response priorities prior to the arrival of the tactical teams in order to facilitate a smooth handover. When the tactical teams arrive, the IOC will conduct a debriefing with the tactical responders. When command is transferred to the OC, the IOC is relieved of this initial role and is assigned the role of deputy Source Control Branch Director (SCBD) in support of the resource management for this event. Source Control Branch Director (SCBD)

For Drilling / RWO operations, the Affected Operations Drilling Superintendent will typically assume the role of Source Control Branch Director (‘SCBD’); for Wells operations, a Senior Wells WSL / Engineer will serve as SCBD. The SCBD reports to the OC and is responsible for supervising and directing the planning for on-scene source control response operations. This position communicates directly with the OC to ensure source control tactical objectives, logistical needs and equipment requirements are properly communicated within the IMT structure.

Depending on the particular circumstances, the WSL may serve as the interim SCBD for the period after relief by the OC and prior to arrival and handover to the Affected Operations Drilling Superintendent / Sr. Wells WSL. For incident durations greater than 24 – 36 hours, the WSL will fill the role of deputy SCBD, providing relief capability Deputy Operations Section Chief (DOSC)

The North Slope Well Operations TL typically assumes the role of Deputy Operations Section Chief. Based on the current staffing structure of the Well Operations TL, the “Senior” Well Operations TL will assume the role as Deputy Operations Section Chief with the IMT and the other will work as needed, supporting ADW. The position of Deputy Operations Section Chief is responsible for five primary functions: 1. Support Operations Section Chief; 2. Support the Command Staff; 3. Support the Planning Section Chief to develop an Incident Action Plan by assigning resources from the ADW North Slope Response Team; 4. Communicate with the Source Control Branch Director, adjust resources as necessary and; 5. Ensures the ADW Logistics Specialist from Tool Services is assigned to the Logistics Section. The Deputy Operations Section Chief provides technical support related to ADW operations. The DOSC has resources available via the ADW North Slope Response Team and ADW Anchorage Response Teams to provide required technical support. ADW Logistics Specialist

This position will typically be filled by the Drilling Tool Service Supervisor (or designate) and report directly to the Logistics Section Chief in the IMT. The Logistics Section is the resource support mechanism for the organization, providing services and support to all the organization components involved in the incident. The ADW representative in the Logistics Section is responsible for coordinating Drilling & Wells material and equipment requirements with the rest of the response organization, ensuring the required resources are located and received in a timely fashion. ADW Technical Specialist

This position provides technical expertise, related to any ADW incident, to the Planning section. The role will be filled by an ADW Senior Drilling Engineer (or designate) and reports to the Planning Section Chief.

4.3.2 ADW Anchorage Response Team (ART) The ADW Anchorage Response Team (ART) is structured much like the traditional Incident Command System (ICS) structure. As discussed in Section 4.3.1 – IMT, the ART does not take a lead role in directing incidentrelated operations, but supports the IMT by developing, assessing and presenting viable options for Source Control. It receives direction from the Deputy Operations Section Chief (DOSC). Activation and Mobilization of ART Upon notification of a Level 1 incident, the Affected ADW Manager notifies the Unaffected ADW Manager, who mobilizes the ART. The Unaffected ADW Manager assumes the role of ART Leader. The ART Leader, or designee, will notify the individuals required to staff the ART, as described in ‘Table 4b - ART Staffing Matrix’, along with any other resources that be required. Mobilization of ART Control Center Upon initiation of a Level 1 Incident response, the ART Leader (Unaffected ADW Manager) will notify BP Security in the Anchorage main building, who will mobilize Conference Room B & C as the ADW Anchorage Response Team Control Center and allow access for the team members. The ART will operate from this location. In addition to Conference Room B & C, the ADW ‘iCenter’, located on the South end of the 7th floor is available as a satellite area for collaborative use. ADW Role Definitions - ART ADW Response Leader The Alaska VP Drilling and Completions (or designate) serves both the Business Support Team (BST) and the ART as the ADW Incident Response Leader, with liaison responsibility between ADW and BPXA management and serves as a member of the BST. ADW Anchorage Response Team Leader The Anchorage Response Team Leader is typically the Unaffected ADW Manager and will organize the response team and coordinate its activities. The Response Team Leader will assign the roles of Operations Section Chief and Planning Section Chief to available qualified personnel and assist them in assigning qualified personnel to complete their staffs. HSE Advisor An HSE Advisor will provide support to the Response Team Leader and the Section Chiefs as required. They will be responsible for coordinating efforts regarding any permits required. They are responsible for communications with the NS HSE Advisors. Documentation Leader(s) The Documentation Leaders are responsible for maintaining current information on the Status Boards for the BST and the ADW Anchorage Response Team. Key duties include updating situation status, tracking open and closed action items. The Communications Officer, Section Chiefs, and the ADW Anchorage Response Team Leader will keep the Documentation Leaders advised of developments to be documented. Two (2) Documentation Leaders will be required, although this may be covered by one (1), depending on the situation. Communications Officer The Communications Officer will be the single point of contact between the Anchorage Response Team and the North Slope Response Team, coordinating communications with the North Slope Response Team, the IMT Information Officer, and the Anchorage Response Team Leader. All official communications between teams and all communications requesting information will be directed through the communications officer. Operations and Planning Technical Support The Operations and Planning Technical Support role was formally the position held by the unaffected operations drilling superintendent. This position is now full time on the North Slope so this position was altered for a senior engineer or ETL. The main responsibilities are to help the operations and planning sections of the ART where needed and work as directed by the ART Leader.

Resource Team Representative The Resource Team Representative position will typically be a facilities, reservoir or production engineers. This position provides the relevant technical expertise and support for the given incident.

4.3.3 ADW North Slope Response Team (NSRT) The primary function of the ADW North Slope Response Team (NSRT) is to provide the specific technical expertise and resources necessary in support of the IMT response. The NSRT also assists in implementing both short- and long-term Source Control plans at the field level, under the direction of the IMT. The ADW NSRT is lead by the Affected ADW Manager, who also serves as Source Control Officer within the IMT. As NSRT Leader, the Affected ADW Manager will travel to the North Slope with the North Slope Response Team, which typically includes: • Affected ADW WTL • Affected ADW Operations Drilling Superintendent • Drilling Engineer(s) • Additional ADW resources, as required • 3rd Party Representatives, as required He / she will coordinate with the Deputy Operations Section Chief to ensure the following positions are appropriately staffed: • Source Control Branch Director • ADW Technical Specialist assigned to the Planning Section to assist in the development of the Incident Action Plan (IAP) • ADW Logistics Specialist (Tool Service) is functioning in the Logistic Section This team will add resources as necessary throughout an incident to ensure adequate ADW technical resources are available. Figure 4-6 depicts the ADW NRST organization. Activation and Mobilization of NSRT Upon initiation of a Level 1 incident, the Affected ADW Manager (or designee) will assume the role of NSRT Leader and begin contacting the individuals required for staffing the NSRT, as described in ‘Table 4c – NSRT Staffing Matrix’. Based on the circumstances of the particular incident, this team will either travel to the affected location, i.e. North Slope, as required or be on stand-by while events are monitored. Once an accurate headcount for team members is available, the NSRT Leader should inform the BST, so travel arrangements can be secured. In the event the NSRT should mobilize to the North Slope, the NSRT Leader (or designee) should fill in the staffing resource information on the ‘ADW Roles / NSRT / Incident Management Team’ form (refer Appendix 4 – Forms). This document should then be posted in a prominent location at the Incident Command Post, providing a visible source of staffing information for active participants. As the response level changes, the corresponding changes in positions should be reflected in the posted staffing charts.

4.3.4 Business Support Team (BST) The BST is located in Anchorage and operates from the Crisis Center. The main functions of the BST are to provide support to the IMT and to communicate with external agencies and stakeholders. The BST is staffed by BPXA personnel outside of ADW staff and, as such, will not be detailed in this document. The BST will typically be activated via the BP Naperville Notification Center. Naperville Notification Center The BP Naperville Notification Center (NNC) has been established to serve as a central clearinghouse for the higher-level notifications required for integrating all required corporate resources necessary to successfully manage an emergency incident. The NNC receives reports ranging from routine to major crisis incidents. After a Level ‘1’ incident, the notification protocol has the Affected OSM / OIM notifying the NNC. The NNC operator receives the necessary information from the caller and then forwards that information to the Notification Center Crisis Coordinator and other appropriate Crisis Management contacts. The NNC will then notify the Anchorage Duty Officer. Examples of incidents reported to the Notification Center include: • Fire / Explosion • Well Control Incidents within Level ‘1’ definitions • Health/Toxicology Emergency • Security Emergency or Threat • Release/Spill • Chemtrec-related Figure 4-3 illustrates the Naperville Notification Center and how it interfaces with the BPXA Incident Response.

ADW Emergency Management Plan - Duty Roster Duty Roster will be updated weekly and emailed by Rachel Brooks on Thursdays to the distribution list. You are responsible to cover your week(s) on the roster. If you need to change a week, coordinate with a person in your column, and communicate changes to Rachel two weeks prior to distribution. The EMP duty roster does not imply "Delegation of Authority". The roster provides initial contingent manning for the ADW Anchorage Response Team only. The EMP duty roster, Initial Incident Response Leader (IRL) will be notified by the NS Well Operations TL (Well Servicing) for a Level 1 incident. The initial IRL will then notify the initial Anchorage response team leader (ARTL) and Initial communications officer (CO) and proceed to the office. The i-center (722) is the muster room until conference rooms B&C are set up, which is initiated by the ARTL. The expectation of the duty roster staff: a) be within an hour from the office and respond to the initial level 1 incident notification, b) notify other EMP duty roster staff, c) muster in the office, set up and staff the Anchorage Response Team, assist the NS IMT. EMP duty roster staff are expected to both understand and fulfill their respective roles as described in the EMP roles and responsibilities. The current ADW EMP version is located under the emergency response tab on the ADW home pages. The weekly Duty Roster rotation starts 0700hrs on Monday morning. ADW Incident Response Leader ADW Incident Response Leader This position is activated in the event of an ADW related incident. The Alaska VP of GENERAL ROLES AND RESPONSIBILITIES:

Drilling and Completions organizes and manages the ADW response within the framework of the Incident Command System. This position works closely with the ADW Business Support Team (BST) and the ADW Anchorage and North Slope Response Team Leaders to develop and approve strategic objectives and source control priorities. When an incident results in activation of the Anchorage BST, the Alaska VP of Drilling and Completions serves a dual role as this position is also a member of the BST, serving as the resident Source Control authority and expert. The Alaska VP of Drilling and Completions will work with the BST to mitigate and minimize impacts of the incident. CHECK LIST: Ensure required notifications have been made. If Level ‘0’, situation will be controlled by ADW resources - no additional support resources required. Source Control Branch Director - IMT

The Affected Operations Drilling Superintendent (ODS) will assume the role of Source Control Branch Director (SCBD) for Drilling or RWO related incidents. For Wells Group incidents, a Senior Wells WSL will serve as Source Control Branch Director, unless otherwise assigned by the Well Operations TL. Note - If the Affected Operations Drilling Superintendent is traveling to the North Slope, the WSL may serve as interim Source Control Branch Director upon relief of his Initial On-Scene Commander duties. The Affected Operations Drilling Superintendent will assume the role upon proper handover from the WSL. ADW Technical Specialist - IMT

The primary function of this position is to provide ADW technical expertise to the IMT Planning Section The Deputy Operations Section Chief (DOSC) will assign this position to the Planning Section. An ADW Representative on the slope will staff it initially. Once the ADW North Slope Response Team is in place, the position will revert to an ADW Senior Drilling or Wells Engineer (based on type of incident). This position communicates with the Deputy Operations Section Chief, ADW North Slope and Anchorage Response Team to provide input into the Planning Section of the IMT. CHECKLIST Report immediately to the Emergency Operations Center and meet with the Planning Section Chief for briefing. Communicate regularly with Deputy Operations Section Chief and ADW North Slope and Anchorage Response Teams to provide information/updates on well status to the Planning Section Chief. ‰ Relay wellbore diagram and status of well to planning section. ‰

Ensure situation unit in planning section has latest source control information for display in the Information Center.

Provide input into the development of the Incident Action Plan (IAP) in coordination with the Deputy Operations Section Chief and both response teams.

ADW Logistics Specialist GENERAL ROLES AND RESPONSIBILITIES - IMT The ADW Logistics Specialist is responsible for coordinating ADW logistical requirements with the rest of the IMT response organization, while ensuring all materials are located and received in a timely fashion. This position will be filled by the Drilling Tool Service Supervisor (or designate) and reports directly to the Logistics Section Chief in the IMT. The Logistics Section is the support mechanism for the organization. Logistics provides services and support systems to all the organization components involved in the incident. Appendix 4 – Form 3 Unit Log, ICS Form 214 / 214a Instructions and guidance for documentation: The Unit log, ICS Forms 214 and 214a, are the standard forms used to provide a documented record of events and activities during an emergency. It is possible that a single document may be drafted for an emergency but as an event escalates it is prudent that each role assignee initiates a log specific for their activities and tasks. This natural expansion of multiple logs is to ensure that each assigned role will create a diary of actions and responses specific to their roles and responsibilities. It is recommended that a new log is initiated at the beginning of each tour and is closed out at the end of each tour. Events, activities and actions shall be recorded factually as they occur or at the earliest possible opportunity. The following guidelines provide the Unit Log user (‘drafter’) with general guidance as to what to incorporate into this log: •

The Unit Log ICS Form 214 / 214a is a living document, chronicling the events and activities associated with the well control event, or other specified emergency incident, and the specific individual role assignment.



Documentation is expected to take place as events occur or at the earliest possible opportunity.



Documentation of events, actions and responses are required to create an audit trail for future reviews and lessons learned analysis.



The drafter must also understand that this form may become a legal document in the event of subsequent litigation and care must be taken that all events and activities are accurately documented.



For each entry the drafter should provide detail of the “who, what, where, why, when and how”. Accuracy and detail are paramount. The drafter must also sequentially number the form pages as well.



Regular submission of ICS Form 214 / 214a to the IMT (Incident Management Team) via fax or email will be defined at the onset of an event.



In the event that the drafter is instructed to write something into the log by anyone it is important to identify these sections with the person’s name who gave the directive to write the section.



When a drafter is relieved of duty, for whatever reason, it is recommended that a ‘statement of relief’ be posted immediately after the last entry by the outgoing drafter. The relief shall initiate new forms for each tour.

Blank sample ICS Forms 214 and 214a are provided for initial event use. Copy blank forms for additional copies as needed.

Staffing & Role Assignment Forms The following forms are recommended for use at each ADW emergency operation center, both ART and NSRT / IMT. These sheets are to be filled in by the ADW management at each site and posted in a prominent location. This provides a valuable interfacing / communicating tool for all incident respondents and additional new assignees, as the event evolves. There is one each staffing form for ART and NSRT / IMT that reflect the primary roles. Both forms can be modified to meet the specifics of a given incident. There is a third generic form that will allow for additional staffing as the event dictates. Utilization of these forms is only a recommended practice for use during the initial stages of an event, when activities are not fully defined and personnel may still be mobilizing. Once the Emergency Operation Center is established and fully functional, event boards may be substituted.
