DIRECTIONS: 1. Use this worksheet for encoding SReA TEST scores. 2. One the first column, key in the test-taker’s (pupil) name. Type the FAMILY NAME first, then the FIRST NAME and then, MIDDLE NAME 3. Accomplish the succeeding cells by clicking on the most appropriate option provide. For each Item in every domain, “O” means the item was accomplished by the pupil. ‘1” means the item was accomplished. Shaded cells need not be filled-up.
