Consent Form: I ____________________________________ , voluntarily accept to be part of this study, “A KAP Study on the type of Treatment Approach opted for Diabetes and its complications in Urban Slums, Nandyal”. I was informed about the study in my own language, as to which type of treatment I was opting for the diabetes. I was also informed about inclusion into this study would not change in the treatment nor cause any physical harm. I was not influenced by any person or my family members to be part of this study.
