History: ชายไทยคู่อายุ 65 ปี อาชีพ ข้าราชการบำานาญ Cc: ปัสสาวะน้อยลงและเหนื่อย Present illness:ชายไทยคู่อายุ 65 ปี เป็นความดันโลหิตสูงมา 7 ปีได้รับยา กลุ่มACE-inhibitor เพื่อลดความดันมาตลอด ยังควบคุมได้ไม่ดี เนื่องจากผูป้ ่วยชอบทานอาหารรสเค็มจัด ความดันโลหิตอยู่ในช่วง 140/90-160/100 mmHg 3 วันก่อนมารพ. หกล้ม ข้อเท้าซ้ายแพลง ได้ซื้อยาแก้ปวดข้อและเอ็นอักเสบ จากร้านขายยามากิน จำานวน 5 ชุด อาการดีขึ้น วันนี้รู้สกึ อ่อนเพลีย เหนื่อยง่าย หอบ สังเกตว่าปัสสาวะลดลงมาก ท้องอืด และหน้าบวมขึ้น ไม่มีไข้ Past history: ไม่เป็นเบาหวาน ไขมันในเลือดปกติ ไม่ดื่มเหล้า หรือสูบบุหรี2่
Pertinent subjective data • มีประวัติ Hypertension (ควบคุมได้ไม่ดี) treated with ACE-Inhibitors • สงสัยว่ากินยา NSAIDs เพ่ ือรักษาอาการปวดข้อและเอ็นอักเสบ
• ปั สสาวะน้อยลง หน้าบวมขึ้น และเหน่ ือย

Hypertension vs kidneys disease • Benign (BP > 150/90 mmHg) – Thickening afferent arterioles, ischemic injury to glomeruli and tubules – Physical examination : nonspecific symptomatology (e.g., headaches, weakness, palpitations), cardiac hypertrophy, and possibly signs of congestive heart failure • Malignant (Sudden elevation of BP, diastolic BP >130 mmHg) – Developed from long-standing or untreated hypertension, markedly elevated BP and rapidly declining renal function – Leading to ischemia and infarction of glomerular and tubular – Physical examination : neurologic (dizziness, headache, blurring of vision, altered states of consciousness, and focal or generalized seizures), Cardiac decompensation and renal failure 8

Hyaline cast • A relatively transparent renal cast composed of proteinaceous material deriv ed from disintegration of cells; seen in patients with renal disease or transiently with exercise, fever, congestive heart failure, and diuretic therapy.

Epithelial cast • A cast that contains epithelial cells and their remnants; occurs most frequently in renal tubules and urine as a marker for re nal tubular necrosis.

Definition of Acute Renal Failure • Acute renal failure is an abrupt decline in glomerular and tubular function, resulting in the failure of the kidneys to excrete nitrogenous waste products and to maintain fluid and electrolyte homeostasis. • Azotemia is a consistent feature of acute renal failure, oliguria is not.

-Urine may be brown with pigmented granular casts, renal tubular epithelial cells and epithelial casts
-The urine often contains red blood cells ,white blood cells, and white blood cell casts.

Renin Angiotensin Aldosterone System
Angiotensin II ออกฤทธิ์ได้หลายตำาแหน่งดังนี้ 1. กระตุน ้ ชัน้ Zona glomerulosa ของต่อมหมวกไต ให้สร้างและหลั่ง
Aldosterone ให้มกี ารดูดกลับโซเดียมพร้อมกับโพแทสเซียมออกมาทางปัสสาวะ 2. กระตุน้ กล้ามเนือ้ เรียบทีผ่ นังหลอดเลือดทั้ง afferent และefferent arteriole ทำาให้หลอดเลือดบีบตัว เพิ่มความต้านทานให้ความดันเลือดสูงขึ้น แต่เลือดเข้ามาเลี้ยงไตได้น้อยลง ( affernt arteriole ไวต่อ AII มากกว่า efferent arteriole ) 3. กระตุน้ mesangial cell ให้หดตัว ลดสัมประสิทธิ์ในการกรองที่ glomerulus 4. มีผลต่อ cell ผนังหลอดไต ทำาให้ดูดกลับโซเดียมมากขึ้น 5. กระตุน้ การสร้าง Prostaglandin ในไต 6. กระตุน้ thirst center ในสมอง ทำาให้กระหายนำ้า

การวัด GFR
Inulin ถูกกรองออกได้ดีที่โกลเมอรูรัส ไม่ถูกดูดกลับ ไม่ถูกหลั่งในหลอดไต และไม่เป็นอันตรายต่อเนื้อเยื่อใด ดังนั้น Clearanceของinulinจึงช่วยบอกGFRได้ 100% ข้อจำำกัด คือ โดยปกติไม่พบสารนี้ในเลือด ต้องให้เข้าทางหลอดเลือดช้า ๆ จนระดับคงที่ในเลือดและในปัสสาวะซึ่งเป็นการยุ่งยากทำาให้ไม่นิยมใช้ในทางคลินิก Creatinine กรองได้ดีในโกลเมอรูรัส ไม่ถูกดูดกลับ ถูกหลั่งในหลอดไตได้บ้าง 5-10% อีกทั้งยังเป็นสารที่มีอยู่แล้วเป็นปกติในร่างกาย จึงนิยมนำามาใช้ในทางคลินิกมากกว่า แม้ว่า Clearanceของ creatinine สูงกว่าGFRที่เป็นจริงแต่อนุโลมให้ถือว่ามีค่าเท่ากับ GFR ได้ (ทางคลินิกนิยมเจาะเลือดหา plasma creatinine เป็นตัวบอก GFR)
