STA BRIGIDA, MANSALAY, ORIENTAL MINDORO, hereby freely and spontaneously declares and makes manifest that: I am the surviving spouse of the late JOHNNY DELOS REYES JR., who died on 03 Novemeber 2006 at BRGY. STA BRIGIDA, MANSALAY, ORIENTAL MINDORO.
