1. That I was born on August 4, 1966 in Bato, Leyte; 2. That in my birth certificate my name is spelled as "Maria Elena T. Alias"; 3. That I have been using "Ma. Elena T. Alias" to which I have been used to in my office records and other public documents; 5. That the names belong to one and the same person; 6. That I am executing this affidavit in order to attest to the truth of foregoing circumstances and for the purpose of setting the records straight and declaring that “Maria Elena T. Alias” and “Ma.
