AFFIDAVIT OF EXPLANATION

(Relating to discrepancy in the entries on Certificates of Births)

I, ANGELITA BODLAT BEWANG, of legal age, Filipino, widower, and a resident of P7

Brgy. Soriano, Cabadbaran City, Agusan del Norte, after having been duly sworn to in accordance with law, depose and state:

1. THAT I have been living together with the late Julius C. Aguilar as husband and wife under the same family home, continuously and without any interruption for a period of at least seven (7) years at the time of his death; 2. THAT to process the documentation relating to the accident which result to his untimely death, I have to acquire both his birth and death certificates; 3. THAT to this end, reasonable and diligent efforts have been exerted to find and locate the parents and other close relatives of the late , Julius C. Aguilar, but the same proved to be futile; 4. THAT I firmly believed to the best of my knowledge that Julius C. Aguilar has no known living parents or relative who can obtain his birth and death certificates; 5. THAT in view of the foregoing, it is only I which may be considered as the sole living relative of Julius C. Aguilar by virtue of our common-law marriage relation. 6. THAT I execute this Affidavit to attest to the truth of the foregoing facts and to support my action to obtain, acquire, and receive from the City of Cabadbaran and City of Butuan, Julius C. Aguilar’s birth certificate and death certificate respectively. IN WITNESS WHEREOF, I have hereunto set my hand this December 18, 2018 in Butuan City, Philippines. AIZA P.
