Republic of the Philippines ) Province of Pangasinan ) Mangatarem for Urbiztondo ) S.S AFFIDAVIT OF EXPLANATION I, HILARIO BUENO, Filipino citizen, of legal age, married, and a resident of Centro Toma, Bani, Pangasinan, Philippines after having been duly sworn to an oath, in accordance with law, do hereby depose and say;

That I was born on 14 January 1959 in Mangatarem, Pangasinan;

2. That my biological mother is GERTRUDES ESTABILLO BUENO; 3. That my true and correct full name is HILARIO BUENO as shown in my CERTIFICATE OF LIVE BIRTH REGISTRY NO. 204;

4. That as per my CERTIFICATE OF LIVE BIRTH REGISTRY NO. 204, my biological father is unknown; hence, I am using the maiden last name of my biological mother;

5. That I am a holder of OFFICE OF THE SENIOR CITIZENS AFFAIRS (OSCA) identification document of Bani, Pangasinan;

6. That my recorded full name in the said OSCA identification document is HILARIO E. BUENO instead of HILARIO BUENO only;

7. That the discrepancy in the above-stated documents as to my true and correct full name was caused by an error inadvertently done by me while preparing the said OSCA identification document;

8. That I am executing this affidavit to explain the said discrepancy in the above-stated documents and to correct my true and correct full name in my OSCA identification document from HILARIO E. BUENO, to HILARIO BUENO;

9. That I am executing this affidavit to declare the truth of the foregoing statements; and 10. That I am executing this affidavit for whatever legal intents and purposes this affidavit may serve.
