Why is your patient getting this medication Prevention and treatment of postoperative n/v.

For IV meds, compatibility with IV drips and/or solutions Administer undiluted postop if n/v occur shortly after surgery. Administer over at least 30 sec and preferably over 2-5 min

Mechanism of action and indications (Why med ordered) Blocks the effects of serotonin at 5-ht receptor sites located in vagal nerve terminals and the chemoreceptor trigger zone in the CNS. Decreases incidence and severity of nausea and vomiting

Nursing Implications (what to focus on) Contraindications/warnings/interactions Hypersensitivity. Use cautiously in liver impairment; abdominal surgery.

Interactions with other patient drugs, OTC or herbal medicines (ask patient specifically) May be affected by drugs altering the activity of liver enzymes.

Nursing Process- Assessment (Pre-administration assessment) Assess for nausea, vomiting, abdominal distention, and bowel sounds prior to and following administration. Assess patient for extrapyramidal effects periodically

Common side effects Headache, constipation, diarrhea

Lab value alterations caused by medicine May cause transient increase in serum bilirubin, AST, and ALT levels. Be sure to teach the patient the following about this medication Take as directed. Advise patient to notify health care professional immediately if involuntary movement of eyes, face, or limbs occur Assessment Evaluation Why would you hold or not give this med? Check after giving Patient reports that nausea has resolved and Improvement of postoperative they are no longer vomiting. n/v due to radiation therapy.
