Dear Madam: The undersigned respectfully informs your office that Ms. Apple Peige C. Margallo had only availed the Booster Shot of Hepatitis B Vaccination conducted by the Health Management Research Group Incorporated last July 11, 2009. Moreover, the fees amounting to P 1950.00 were already charged to her assessment for the 1st Semester of S.Y. Furthermore, the student claimed that the Hepatitis B Vaccination Fee had been charged in her current assessment for the 1st Semester S.Y 2009-2010. In this connection, may request that the said remaining vaccination fee be deducted from the total assessment of this 1st Semester of S.Y. 2009-2010. Thank you and more power.
