Purpose and Goals  Provide better quality of life for the residents with chronic mental illnesses (Schizophrenia, Dementia, Parkinson’s disease).  Provide Rehabilitative services for residents with injuries (bone fractures resulting from a fall).  Create a homey environment for the patients in need of physical and emotional

Setting and Residents 

The Madison Center is located on 161 Bakers Ridge road.



The location of the Center offers residents the proper tranquility and serenity that are necessary for a speedy recovery and general well-being.



Older Adults that are in need or long-term, short-term or rehabilitative services.



The residents come from various cultural backgrounds and different areas of the United States.

The services provided by the Madison Healthcare Center are oriented towards:

Development of new skills and interests.



Maintaining current mental functioning in order to prevent decline.

My Role  One-on-one visits with residents  Assist during scheduled activities  Report unusual behaviors (depressed or apathetic mood)  Report any improvements (participation and initiative)  Encourage social interaction  Plan and carry-out recreational activities  Listen to the residents needs and accommodate feasible requests  Conduct activity sessions reared towards “reminiscing”
