Background  The Madison organization offers high quality nursing, medical and rehabilitative services to their patients. Purpose and Goals  Provide better quality of life for the residents with chronic mental illnesses (Schizophrenia, Dementia, Parkinson’s disease).  Provide Rehabilitative services for residents with injuries (bone fractures resulting from a fall).  Create a homey environment for the patients in need of physical and emotional

Setting and Residents 

The Madison Center is located on 161 Bakers Ridge road.



The location of the Center offers residents the proper tranquility and serenity that are necessary for a speedy recovery and general well-being.



Older Adults that are in need or long-term, short-term or rehabilitative services.



The residents come from various cultural backgrounds and different areas of the United States.

Services offered at the Center 

The services provided by the Madison Healthcare Center are oriented towards:



Development of new skills and interests.



Maintaining current mental functioning in order to prevent decline.

My Role  One-on-one visits with residents  Assist during scheduled activities  Report unusual behaviors (depressed or apathetic mood)  Report any improvements (participation and initiative)  Encourage social interaction  Plan and carry-out recreational activities  Listen to the residents needs and accommodate feasible requests  Conduct activity sessions reared towards “reminiscing”

The Madison Center staff and management is committed to giving their residents the best quality care possible by providing physical, emotional and psychological support to each resident in order to insure them a meaningful experience during their “golden” years.
