To Parents / Guardians: This report is issued for the purpose of keeping you informed on the progress of your child in the Target Program. It will be used in determining his/her eligibility for continued placement.

OVERALL ASSESSMENT: A total of fourteen or more marks in the “M” column indicates an Overall Assessment of “Meets the essential requirements.” (M) M – Meets the essential requirements D – Does not meet the essential requirements.
