Klaricid (Suspension) Generic name Clarithromycin Action Binds to the 50S of bacterial ribosome, blocking protein synthesis; bacterio static or bactericidal, depending on the concentration. Dosage 7.5 mg/kg bid up to a max 500 mg bid. Usual duration: 5-10 days. Indication Upper & lower resp tract, acute otitis media, skin & soft tissue infections. Disseminated or localized mycobacterial infections due to Mycobacterium avium or Mycobacterium intracellulare. Side effect • Headache • Rash ( in children) Nursing Consideration . • The safety and effectiveness of the ER form haven’t been established for treating other infections for which the original form has been approved. • Obtain specimen for the culture and sensitivity test before giving. Begin therapy awaiting results. • Monitor patients for super infection. • Giving Clarithromycin with a drug metabolized by CP3A may increase drug levels and prolong therapeutic and adverse effect.

Paracetamol Action Thought to produce analgesia by blocking pain impulses by inhibiting synthesis of prostaglandin in the CNS or of the other substances that sensitize pain receptors to stimulation. The drug may relieve fever through central action in the hypothalamic heat regulating center. Dosage Cap Adult & childn >12 yr 1 cap. Syr Childn 7-12 yr 5-10 mL, 2-6 yr 2.5-5 mL. Drops 1-2 yr 1.2-1.8 mL, 6 mth-1 yr 0.6-1.2 mL, <6 mth 0.3-0.6 mL. All to be taken tid-qid.

Indication Fever & pain due to common cold, flu, measles, teething discomfort & post-vaccination reactions. Side effect • Jaundice • Rash, urticaria • Hypoglycemia Nursing Consideration • Many OTC and prescription products contain acetaminophen; be aware of this when calculating total daily dose. • Use liquid form for children and patients who have difficulty swallowing. • In children, don’t exceed five doses in 24 hours.
