WELCOME TO REST Mission & History: In 1979, when a man freezes to death on the streets of Uptown, a group of neighborhood residents respond with a commitment to provide emergency overnight shelter for homeless men and women. Their commitment to prevent any further tragedies was the beginning of REST. Today, REST provides shelter, permanent housing and social service programs to hundreds of homeless men and women each day. Since its inception, REST has sheltered tens of thousands of individuals and placed thousands of people into housing. Mission REST works collaboratively to provide safe, quality, permanent and emergency housing options and supportive social services to adult men and women who are homeless in Chicago. REST assists these individuals to develop their employment and life skills to achieve the transition to independent living. REST is committed to: An individual’s right to housing. The dignity of individuals and their right to freedom of choice when fully informed of options. The diversity of individuals, including the need for a diverse staff, board, volunteer base and community with resulting talents and resources from which REST can benefit. The sharing of knowledge and experience and working cooperatively with service providers and others to more effectively address the problems of homelessness. REST SHELTER RULES The following rules apply to all clients during their stay in the shelter... any clarification of rules can me answered by staff. For questions regarding the shelter rules, please ask you case manager and attending staff. 1.) 2.) 3.) 4.) 5.) 6.) 7.) 8.) 9.) 10.) 11.) 12.)

REST SHELTER SERVICES -Support/Day Center Located in the same physical space as the Men’s Shelter [941 W Lawrence.] , the Support Center offers REST’s clients daytime shelter from 8:00 a.m. to 8:00 p.m. The Support Center, which acts as an entry point into REST’s other programs, offers reading materials, games, TV and beverages. Clients are to sign the membership list at the front desk upon arrival, and must have stayed the night before at one of the overnight shelters or the warming center. For questions regarding the support/day center, please ask you case manager and attending staff. -Shelter/Program Membership To transition from the Warming center to becoming a member, clients must sign the membership list, located at the support-center, till a membership spot opens up. When you become a member, you will receive a temporary membership card and will be assigned a case manager. In order to obtain a membership card, you must complete a number of support group meetings alloted by your case manager. For questions regarding the shelter/program membership, please ask you case manager and attending staff. -Warming Center In addition to the Men’s and Women’s Shelters, a seasonal Warming Center (generally open 40 weeks per year) provides overnight shelter for another 65 men. For entry, clients are to sign in at the day center at 4-5pm in order to obtain a ticket for the shelter later that night, and a “lottery” is held if there not enough beds at time of entry. You are to sign in every night in order to keep you bedspace. To transition from the Warming center to becoming a member, clients must sign the membership list, located at the support-center, till a membership spot opens up. For questions regarding the warming center, please ask you case manager and attending staff. -Overnight Shelters REST operates two overnight Emergency Shelters that accommodate 60 men and 40 women. In REST’s shelters, open 365 days a year, clients receive: a safe place to sleep, a shower, and a nightly meal. For entry, clients are to sign in at the day center at 4-5pm in order to obtain a ticket for the shelter later that night, and a “lottery” is held if there not enough beds at 6pm. You are to sign in every night in order to keep you bedspace, with the exception of a three consecutive day grace period, with a total of 7 days out per month. For questions regarding the overnight shelters, please ask you case manager and attending staff. -Shower and Meal Services Shower services are as follows: Shower services start at entry to the overnight shelter, and go on till 11pm. Meal serivces are as follows: Breakfast, from 7:00am till 7:30am [with exceptions] Dinner Meal: 9:00pm till :9:30 [with exceptions] The People's Church of Chicago, [also located at 941 W Lawrence] serves from noon till 1:30pm [with exceptions] in the support center. For questions regarding shower and meal services, please ask you desk attendant.

REST PROGRAM SERVICES -General REST’s Social Service Program includes individualized case management, referrals (including on-site mental health assessments and job preparation), Life-Skills classes, support groups, and a medical clinic. REST case managers work with each client to develop a “big picture” plan that has goals designed to increase a client’s skills or income, increase their self-sufficiency, and ultimately to place them into permanent housing. The RESTCare health clinic is staffed by volunteer doctors and nurses and offers access to medical health services. The hours are as follows: Social Service Program: Monday-Friday, 9am-11:30pm, 1pm-4:30pm Health Care Clinic: Tuesdays, 6pm-8pm -Social Services Upon obtaining a temporary membership card, you will be assigned a case manager. REST case managers work with each client to develop a “big picture” plan that has goals designed to increase a client’s skills or income, increase their self-sufficiency, and ultimately to place them into permanent housing. In order to keep membership and access to these services, each member must complete a number of support group meetings alloted by your case manager. For questions regarding social services, please ask you case manager. -Housing Program REST’s Housing program rents privately-owned apartments to give its clients a home of their own. The Housing Program caters to individuals with chronic mental illness, chronic substance abuse or both. Eligibility for the program requires participation in case management before and during housing placement. The Housing Program is supported by a grant from the U.S. Department of Housing and Urban Development (HUD). A client’s rent is calculated at 30 percent of their income, if they have no income, they pay no rent. For questions regarding qualifications, stages, and housing rules, please ask you case manager. -Support Groups In order to keep membership and access to social services, each member must complete a number of support group meetings alloted by your case manager. These meetings range from 12-Step [AA, NA, CA] meetings, to community meetings, including Art, Health, Fitness, and general supportive groups. For questions regarding support groups and schedules, please ask you case manager.

REST HOUSING STAGES AND RULES REST’s Permanent Housing program rents privately-owned apartments to give its clients a home of their own. The Permanent Housing Program caters to individuals with chronic mental illness, chronic substance abuse or both. Eligibility for the program requires participation in case management before and during housing placement. The Permanent Housing Program is supported by a grant from the U.S. Department of Housing and Urban Development (HUD). A client’s rent is calculated at 30 percent of their income, if they have no income, they pay no rent. For questions regarding housing stages and rules, please ask you case manager.
