PHN vacancies will be filled from this list. Qualified applicants will remain on the eligible list for 6 months and then must reapply. 'Public Health Nurse - Immunization Project Associate, is the only exception; please see separate announcement to apply directly for that position. Summary of Practice: PHNs provide direct care and health promotion services for individuals, families and community groups in King County. They perform complex assessments, education, linkages to community resources, and case management services in patient homes, clinics and other community settings. PHNs provide nursing services to low income and diverse client populations. This may include services to children with special health care needs, abused/neglected children, teen parents, and families with complex social concerns. In communicable disease they are involved with prevention and intervention. Current openings:

PHN - Kent/South County; Family Planning: 1.0 FTE; 8 hours/Monday – Friday, 5 days a week schedule; Family Planning / DSHS Community Service Office. Provides vacation coverage and backfill primarily at the 5 south county DSHS CSO offices. Based at Kent DSHS CSO. PHN - White Center; 1.0 FTE; Maternity Support Services (office); Monday – Friday. Applicants bilingual in English and Spanish desired. This vacancy open to internal candidates only. EXAMPLES OF DUTIES Duties of Specific Programs: There are a number of specialized programs in which PHNs may provide services. PHNs may be assigned duties in multiple programs depending on their local office needs. Maternity Support Services (MSS) PHN's provide office and/or home visits to

pregnant and postpartum women and their infants, children & families. Responsibilities include family assessment, care planning, case management, education/support, documentation, case management, and coordination of care with community providers. PHN's in the MSS program may also be assigned duties in CSHCN and EIP. Children with Special Health Care Needs (CSHCN) PHN’s provide home, office & phone visits to families that have a child(ren) birth to 18 years of age that have complex medical or developmental conditions. Responsibilities include family assessment, care planning, case management, education/support, documentation, case management, and coordination of care with community providers. Early Intervention Program (EIP) & Early Family Support System (EFSS) In these programs PHN's work with clients referred by the Division of Children and Family Services/Child Protective Services. These nurses provide nursing services to individuals and families with a goal of reducing the risk of child abuse and neglect, identification and referral for health and developmental problems, improving parenting & family functioning, improving health status and preventing health problems. Nurse Family Partnership (NFP) This project targets first time young mothers, in the NFP project, for intensive home visiting services with a goal of improving health and social functioning of the mothers and their babies. Components of the program include a limited caseload of 25 families per PHN who are followed with a defined schedule of home visits using structured protocols for a duration of 2 ½ 3 years. The selected candidate for this project will participate in training on the Nurse Family Partnership Program in Denver. The project also includes a very specific supervisory component of weekly individual and team meetings with project staff and supervisors. Responsibilities include caseload management, assessment, care planning, documentation, case management, and patient education. Duties may include group education and maternity support office visits. Desired Qualifications: Maternal-child public health nursing experience and NCAST certification and lactation training preferred. Experience working with multidisciplinary, collaborative programs strongly preferred. Spanish bilingual and bicultural person preferred. PHN's in NFP may be assigned duties in CSHCN and MSS programs. Family Planning Services (FP) PHN's provide care primarily at the Community Service Office (CSO). Nurses provide birth control counseling, pregnancy testing, pregnancy options counseling and emergency contraception. Youth Health Services (YHS) PHNs function as a member of an interdisciplinary, interagency team working with teens in local school districts. The PHN participates in reproductive health and psychosocial assessments, does intensive outreach at schools, develops outreach materials and acts as a liaison between the Youth Health Center (YHC) and other social and health agencies. This position often represents the Youth Health Center and Public Health to the community and when working with families. This position provides services at the YHC including birth control counseling, pregnancy testing, pregnancy options counseling and emergency contraception. Regional Health Outreach Services (RHO) PHN's provide nursing consultation in the Child Care Health Program directed toward improving health status, preventing health problems and promoting the safety and emotional well being of children in child care sites (including licensed home child care programs) within King County. MOM's Plus The MOM's Plus program provides intensive home visiting services to an

aggregate population of low income clients in King County who may be homeless and/or have problems with substance abuse. Some visits will occur in in nontraditional settings such as shelters or treatment centers. PHNs in MOM's Plus complete all assessments for both program and health department requirements. PHNs in MOM's Plus work closely with the MOM's team in county correctional facilities and collaborate treatment services in south county, maintain positive relationships with community partners, drug/mental health courts and attorneys. Foster Care Passport program is a collaborative service provided by DSHS Children's Administration and contracted staff from PH. These PHN's gather, interpret and record health history information (back to birth) on children in out-of-home placements in Washington. This information in shared with the child's provider, (foster parent's, adoptive parents, social worker's) to improve the child's health and educational status. Tuberculosis (TB) PHN provides case management services to patients with active or latent disease, care coordination, referral and follow-up, contact investigation work, medication management, ongoing physical and psychosocial assessment to individuals and families. These nurses provide information to patients, community members and other healthcare providers regarding TB. The PHN may also participate in research projects that seek to improve the health and treatment of TB patients. PHN provides Communicable Disease Investigation. Working for Communicable Disease Control, Epidemiology and Immunization Section. Major areas of responsibility include: performing case and outbreak investigations and coordinating section responsibilities relating to the tracking of perinatal hepatitis B and the implementation of disease control measures; assist in developing disease investigation protocols and in assuring that questionnaires are current, complete, accurate, and reflect best practices; provide information and technical assistance to other section staff and departmental programs, health care providers and the public; participate in section's response during outbreaks and emergencies which may involve after hours work. White Center Early Learning Initiative (WCELI) PHNs deliver intensive home visiting services to at-risk families living in the White Center Early Learning Initiative (WCELI) catchment area. Families will be enrolled pre and post natally and will receive home visits twice a month until their infants reach 2 years of age. Responsibilities include caseload management, assessment, care planning, documentation, case management, and patient education. Services will be coordinated with other WCELI home visiting services including First Steps and doula services; and will incorporate Promoting First Relationships and the Partners in Parenting Education curricula.

General Duties: Obtain and document health history and make nursing diagnosis based on nursing assessment. Documentation may require electronic charting; TREC2, WIC, CIMS. Provide expertise regarding public health nursing scope, practice and policy at many levels. Provide support services to clients that links/refers, to other Health Department staff and community providers/partners, while assuring access and continuity of care. Provide program planning, monitoring local work plans and progress toward program

goals. Participate in quality assurance activities to meet laws, regulations, policies and procedures. Provide training, orientation and education to staff, clients, students and the community. May provide birth control and pregnancy options counseling, including emergency contraception. Travel throughout King County. QUALIFICATIONS Licensing/Certification Requirements: Washington State Registered Nurse License in good standing. Bachelors of Science in Nursing (BSN) from an accredited National League of Nursing (NLN) university or equivalent. Qualifications: Knowledge of public health nursing principles and processes; human systems; infectious disease management and control; health care system structure and function. Knowledge of child and adult learning and health education techniques and principles. Knowledge of research techniques and principles. Knowledge of personal safety standards, safety techniques and principles. Demonstrated abilities performing comprehensive nursing assessments and successfully engaging clients in a plan of care. Demonstrated ability to work in homes and community settings without on-site supervision; communicating effectively with patients and their families and with other health and human services staff; developing and maintaining therapeutic relationships with patients; and working with culturally and socio-economically diverse populations. Demonstrated ability providing reproductive health education, services, community outreach, and working with community partners and agencies. Demonstrated ability working in a fast paced environment independently, multi tasking, demonstrating attention to detail, crisis intervention and problem solving skills and effective time management. Demonstrated ability completing medical record documentation. problem-solving; and case management. Demonstrated organizational and leadership abilities. Willing to provide options counseling.

Desired Qualifications: Skilled in using information management systems and other health care technology. Background in Public Health or experience related to the special program employment request. Bilingual / Spanish (Immunization PHN) Experience related to immunizations, group presentations and education. Necessary Special Requirements: The selected candidate must pass a thorough background investigation. PHN's who provide direct services in CHS under a DSHS service agreement will go through background checks every three years. Employees are required to protect the privacy and security of protected health information as defined in State and Federal Law Employees are required to adhere to OSHA/WISHA guidelines including but not limited to completing their mandatory trainings on time. Washington State Driver's License or the ability to travel to work locations throughout King County with no public transportation in a timely fashion.
